The objective of this case report is to describe the substantial sagittal correction of spinal hyperkyphosis alongside fracture fixation. In advanced ankylosing spondylitis (AS), the spine is usually fused, hyperkyphotic, and due to deformity, as well as improper bone remodeling, predisposed to fractures. These fractures, mostly unstable, require surgical treatment. The authors present fracture management with concomitant deformity correction at the fracture site and pedicle subtraction osteotomy (PSO) below the fracture, showing the benefits of performing the procedures with the patient in a

INTRODUCTION AND IMPORTANCE: A multi-level non-contiguous spinal fracture (MNSF) caused by a high-energy impact is a type of complex traumatic injury that is been frequently initially missed, and resulting in delayed diagnosis which adversely affects can result in spinal deformity and neurological deficit. This report describes the operative management of a patient with MNSF with spinal cord injury involving the cervical and thoracic vertebrae by cervical orthosis and posterior thoracic decompression and fusion.

AETIOPATHOGENESIS: Sacral fractures represent an often overlooked interface between spinal and pelvic ring injuries. They typically occur in high-energy trauma or in older patients with osteoporosis.
DIAGNOSTICS: Due to their complex anatomy and deep location, they are difficult to detect using conventional radiography; thus, computed tomography (CT) is considered the diagnostic gold standard and allows precise fracture analysis. Magnetic resonance imaging (MRI) is particularly valuable for insufficiency or stress-related fractures to detect early bone marrow edema. Biomechanically, the sacru

INTRODUCTION AND IMPORTANCE: Unstable pelvic ring injuries often occur in high energy traumas. Vertical sacrum fracture is an associated injury. This report describes a late spinal nerve compression that occurred following surgical reduction and fixation of pelvic ring injuries caused by traumatic L5-S1 disc herniation and malposition of the sacral fracture.
CASE PRESENTATION: A 61-year-old female presented with radiculopathy in her right leg after surgical reduction and fixation of a sacral and pelvic fracture. Physical examination revealed numbness and weakness of the right leg. Radiographi

INTRODUCTION: Epidemiological studies of fractures of the spine in children are all old, mostly single-centre, with series spanning periods of 5 to 20 years.
HYPOTHESIS: As lifestyle is constantly changing, notably with an increase in sports activities and improvements in the prevention of road and household accidents, epidemiology has likely changed.
OBJECTIVE: To update the description of spinal trauma in children and adolescents compared to the existing literature.
MATERIAL AND METHOD: A multicentre cross-sectional study of spinal fracture, dislocation and spinal cord injury without radi

STUDY DESIGN: The study retrospectively investigated 15 cases with multilevel noncontiguous spinal fractures (MNSF).
PURPOSE: To clarify the evaluation of true diagnosis and to plane the surgical treatment.
OVERVIEW OF LITERATURE: MNSF are defined as fractures of the vertebral column at more than one level. High-energy injuries caused MNSF, with an incidence ranging from 1.6% to 16.7%. MNSF may be misdiagnosed due to lack of detailed neurological and radiological examinations.
METHODS: Patients with metabolic, rheumatologic diseases and neoplasms were excluded. Despite the presence of a spi

OBJECTIVE: Osteoporotic compression fractures, which can lead to neurological complications in some cases, are an increasingly frequent occurrence. These lesions require decompression surgery with or without spinal stabilization procedures. In this article, we present the preliminary results obtained using open kyphoplasty, a new method of treating vertebral compression disorders.
METHODS: Sixteen patients were included in this prospective study, and a total of 17 vertebrae were treated. All of the patients had vertebral compression fractures associated with neurological disorders. The surgic

OBJECTIVES: We evaluated early results of osteoporotic spinal fractures treated with balloon kyphoplasty.
METHODS: The study included 16 patients (11 females, 5 males; mean age 66 years, range 55 to 75 years) who were treated with balloon kyphoplasty for osteoporotic spinal fractures. One patient had vertebral fractures at two levels. The mean duration from trauma to kyphoplasty was two months (range 0 to 4 months). The mean amount of cement used during the procedures was 4 ml (range 3 to 6 ml). Kyphoplasty was performed with a bipedicular approach in all the patients. Functional results were

Thirty-one patients with thoracolumbar burst fractures, seven untreated, 16 treated nonoperatively, and eight treated operatively, were analyzed retrospectively and followed up for 3 to 7 years. The initial and final degrees of neurologic deficit and the stenotic ratio of the spinal canal were recorded. Stenotic ratio significantly decreased from the first examination (range, 12.3%-74.5%; average, 26.2%) to the final followup (range, 5.4%-46.5%; average, 19.2%), but there were no differences of the percentage of remodeling between patients who were untreated and those treated nonoperatively an

STUDY DESIGN: A prospective, longitudinal study of multiply injured patients treated with segmental instrumentation for spinal fractures with a minimum 2-year follow-up.
OBJECTIVES: To determine whether urgent stabilization of spinal fractures in severely injured patients increases the risk of surgery compared with early treatment and historical results.
SUMMARY AND BACKGROUND DATA: Opinion in clinical studies is divided about whether operative treatment offers an advantage over nonoperative treatment in isolated spine fractures. Concomitant trauma is rarely discussed relative to decision ma

Generally osteoporotic compression fractures of the spine cause only localized pain and kyphosis without other significant complications. These fractures usually heal without difficulty by conservative treatment. However, it becomes to be accepted as a general concept that a small number of patients with osteoporotic fracture of the thoracolumbar spine result in delayed vertebral body collapse with neural compromise. We analyzed our series of osteoporotic-posttraumatic vertebral collapse. We had treated 51 consecutive patients with posttraumatic vertebral collapse following osteoporotic compre

We assessed narrowing of the spinal canal in 39 burst fractures and fracture-dislocations of thoracolumbar vertebrae treated by the AO Internal Spinal Fixator, using CT preoperatively and at various stages postoperatively. Computer-aided planimetry was used to measure the narrowing, and its restoration shortly after instrumentation, or at 15 months. The mean initial reduction of canal area was to 63.7% +/- 18.8% of normal; this was restored to a mean of 95.4% +/- 21.2% of normal when measured either soon after surgery or at 15 months (p < 0.001 for both groups). There was more improvement in c

Skeletal injuries may be overlooked in multiply-injured patients. Skeletal scintigraphy, a very sensitive indicator of skeletal injuries, may give complementary information. We therefore performed scintigraphic screening in 20 patients with multiple injuries. In all, 38 fractures were diagnosed by plain skeletal radiography, and were also visualized scintigraphically. Furthermore, scintigraphy revealed a fracture of the pelvis in one patient and a tibial plateau fracture in another. Scintigraphy was, however, negative in three patients; two with fractures of the transverse processes in the lum

Cotrel-Dubousset instrumentation (CDI) was used to stabilize thoracolumbar fractures in 48 consecutive patients at the Lucerne Spinal Center. The results of these patients were reviewed with respect to completion of healing, change in neurologic status, medical or hardware complications, and follow-up radiographic parameters. Twenty patients were braced and 28 were not braced after operation, depending on certain criteria. Two elderly patients died at 1 and 6 months after their injury, of medical complications. One other patient was lost to follow-up after 6 months. Of the remaining 45 patient

AIM: To study fracture rates and risk factors for fractures in patients with spinal cord injuries.
MATERIAL AND METHODS: A self-administered questionnaire was mailed to 646 members of the Danish Paraplegic Association and 1000 randomly selected normal controls. 438 patients (309 males, 129 females, 67.8%) and 654 controls (332 males, 322 females, 65.4%) returned the questionnaire. Median age in patients was 42, range 10-80 years, and in controls 43, range 19-93 years (2p = 0.25).
RESULTS: The crude fracture rate was 2% per year in patients and 1% per year in controls (RR = 2.0, P < 0.001). L

Osteoporosis and fragility play a significant role in the treatment and planning of patients with deformity secondary to osteoporotic vertebral fracture (OVF). The resulting deformity can present significant challenges for its management, both from a medical and surgical perspective. The need for a specific classification for these deformities, including the potential for the development of artificial intelligence and machine learning in predictive analysis, is emerging as a key point in the coming years. Relevant aspects in preoperative optimization and management of these patients are addres

OBJECTIVE: Falls from standing are common, particularly amongst the aging population, due to declining mobility, proprioception and vision. They are often complicated by fragility fractures, including vertebral fractures, that are associated with significant morbidity and may represent a pre-terminal condition with high one-year mortality rates.
PATIENTS AND METHODS: A retrospective review of the Trauma Audit and Research Network database for a major trauma centre was conducted for all patients admitted between January 2011 and December 2016. Patients with a spinal fracture and a confirmed fa

BACKGROUND: In patients with ankylosing spinal disease, including ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis, even low-impact trauma can lead to complex injuries. The injuries seem to be highly unstable and associated with greater mortality rates compared with the general spine trauma population.
METHODS: We reviewed the medical records of a consecutive series of 41 patients (34 men, 7 women) with ankylosing spinal disease and unstable traumatic spine injuries who were admitted to our department from 2007 to 2016.
RESULTS: The mean patient age was 73.4 ± 12.7 years.

Oesophageal lesions due to spinal fractures are very rare and easily overlooked. In this case report a woman was involved in a minor car accident, and she was primarily examined at the accident and emergency department with no signs of fractures. Ten days later, she was readmitted due to high infection counts and back pain. She had an unstable fracture of Th4 and a perforation of the oesophagus due to ankylosing fractures. The case emphasises the need to pay attention to a possible development of secondary oesophageal perforation in ankylosing spines and unstable fractures, and the need for a
