Unilateral hemilaminectomy: The surgical approach of choice for juxta-medullary spinal tumors.

BACKGROUND: The conventional laminectomy may precipitate numerous long-term complications such as the development of kypho-scoliosis, spinal instability, epidural fibrosis and loss of bony shield over the spinal cord, thus increasing the risk of a subsequent myelopathy. The present study evaluates the efficacy of a unilateral hemilaminectomy as the surgical approach of choice for excision of juxtamedullary spinal neoplasms. MATERIALS AND METHODS: From January 2001 to December 2010, 83 patients (male: female ratio: 58: 25, mean age 37.4 years, median age 45 years) harboring a juxta-medullary s

Primary germinoma of the spinal cord: a case report with 28-year follow-up and review of the literature.

Germ cell neoplasms occur in extra-gonadal midline locations of the retroperitoneum, mediastinum, pineal gland, areas of the suprasellar cistern, and rarely in the spinal cord. We recently reviewed a case of an unresectable lumbar spinal cord tumor in a 16-year-old female previously diagnosed as "metastatic poorly differentiated carcinoma." An extensive evaluation for a primary neoplasm at that time was unsuccessful and the patient was treated with local radiation therapy. Recently, additional histochemical and immunocytochemical studies were performed on the archival formalin-fixed paraffin-e

Neoplastic cauda equina syndrome: a neuroimaging-based review.

Cauda equina syndrome refers to dysfunction of the cauda equina, the collection of ventral and dorsal lumbar, sacral and coccygeal nerve roots that surround the filum terminale. This most commonly occurs as a result of compression by a herniated lumbosacral disc. However, the syndrome may also complicate metastatic cancer or a primary neoplasm within or infiltrating the spinal canal. An accurate and timely diagnosis is critical to avoid irreversible loss of neurological function. The clinician and radiologist must therefore be aware of the many possible causes to guide timely management. Here

Intradural spinal cord lesions: Gd-DTPA-enhanced MR imaging.

Twenty-five patients with suspected spinal cord neoplasms were studied with high-field-strength magnetic resonance (MR) imaging (1.5 T) before and after administration of gadolinium diethylenetriamine-pentaacetic acid (DTPA) (gadopentetate dimeglumine). Five patients had enhancing, nonneoplastic lesions, including spinal dural arteriovenous fistulas (AVFs), cord infarction, and chronic arachnoiditis. Fifteen patients had proved spinal cord neoplasms, 13 intramedullary and two extramedullary. Four of the intramedullary tumors were detected only after Gd-DTPA administration; in five others, cont

Neoplasms of the Cervical Spine: Diagnostic Workup and Clinical Decision-making Algorithms.

STUDY DESIGN: Narrative review. OBJECTIVE: To synthesize current evidence on diagnostics and clinical decision-making for patients with cervical spine neoplasms. BACKGROUND: Although the cervical spine is disproportionately affected by primary spinal tumors and is the predominant site for intramedullary spinal cord tumors, there remains a relative paucity of reviews focused specifically on cervical spine neoplasms. METHODS: PubMed and Scopus were searched using a combination of terms related to cervical spine neoplasms, classification schemas, diagnostic evaluation, staging, and clinical de

[Magnetic resonance imaging in spinal cord diseases: 127 cases].

Acute or chronic spinal cord syndromes with or without evidence of cord compression are a common diagnostic problem in neurology. Magnetic resonance imaging (MRI) was performed on 127 patients to assess the sensitivity of this examination in detecting local spinal cord lesions. The most common location noticed in this study were the cervico-thoracic (40.9%) and cervical regions (33.8%). A total spine examination accounted for 14.9% of the examinations performed, and the lumbar spine was evaluated in 10.2%. An increased signal corresponding to plaques of multiple sclerosis was the most frequent

A review of primary central nervous system neoplasms of cats.

Primary central nervous system (CNS) neoplasms are uncommonly diagnosed in cats. The majority of primary feline CNS neoplasms described in the veterinary literature consist of meningioma and glioma occurring mainly in the brain and less often in the spinal cord. Although most neoplasms can be diagnosed based on routine histologic evaluation, less typical tumors need to be further characterized using immunohistochemistry. This review compiles the relevant information about the most common primary CNS neoplasms of cats available in the veterinary literature, aiming to serve as a converging sourc

Meningeal Melanocytoma of the Lumbar Spinal Region: A Unique and Unusual Presentation.

Meningeal melanocytoma of the central nervous system is a rare primary melanocytic neoplasm arising from the leptomeninges' melanocytes. It chiefly occurs in the posterior cranial fossa and rarely involves the spinal cord. The literature is bereft of lumbar spinal involvement by meningeal melanocytomas. Herein, we present a 17-year-old male patient who was incidentally diagnosed with a lumbar spinal meningeal melanocytoma during an imaging workup for backache following a fall. It was an extramedullary intradural tumor and was amenable to complete surgical removal in the spinal region. This cas

Chondromyxoid fibroma of the lumbar facet joint: A case report.

Chondromyxoid fibroma (CMF) is a rarely documented benign osseous neoplasm, particularly with respect to its incidence in the lumbar spinal region. CMF predominantly manifests in vertebral bodies, exhibiting atypical emergence in ancillary anatomical sites. The present report describes, to the best of our knowledge, the second documented instance of CMF originating from the lumbar facet joint. The present case provides an example of CMF in the lumbar facet joint precipitating spinal canal stenosis, thereby engendering neurological manifestations in the lower extremities due to neoplastic proli

Spinal meningeal oligodendrogliomatosis in two boxer dogs.

Two Boxer dogs developed progressive ataxia in association with a neoplastic infiltration of the spinal leptomeninges. In the first dog, the leptomeningeal neoplasm encompassed the entire cord and the ventral aspect of the brainstem and extended bilaterally into the piriform lobes. In the second, the neoplasm surrounded the C1-C3 segments of the spinal cord and the brainstem without involvement of the brain or spinal cord parenchyma. In both dogs, the neoplastic cells had variably distinct cell borders, clear to eosinophilic cytoplasm, and a round to ovoid hyperchromatic nucleus. Neoplastic ce

Hemangiopericytoma of the spinal canal. Report of three cases.

Hemangiopericytoma is a vascular neoplasm consisting of capillaries outlined by an intact basement membrane that separates the endothelial cells of the capillaries from the spindle-shaped tumor cells in the extravascular area. These neoplasms are found in soft tissues but have rarely been shown to involve the spinal canal. This is a report of three such cases. Surgical removal of the tumor from the spinal canal was technically difficult. A high risk of recurrence has been reported but in these three cases adjunctive radiotherapy appeared to be of benefit in controlling the progression of the d

Coronal STIR sequence, a simple adjustment to routine MRI protocol for extra-spinal sciatica and sciatica-like symptoms

Abstract Background The value of adding coronal STIR images to MR imaging of sciatica aiming to detect extra-spinal abnormalities. Results Additional coronal STIR images detected extra-spinal abnormalities in 20% of the patients, thereby downgraded the normal studies from 21 to 13%. The extra-spinal abnormalities included bone abnormalities (36.4%), soft tissue abnormalities (4.5%), neurological abnormalities (2.3%), gynecological abnormalities (50%), and miscellaneous (6.8%). In 6.9% of patients, the extra-spinal abnormalities explained the patients’ pain and influenced their management. Extr

Non-enhancing pilocytic astrocytoma of the spinal cord.

Pilocytic astrocytomas are among the most common intramedullary spinal cord tumors in the pediatric age group. The presence of contrast enhancement is a major factor used to distinguish these tumors from other spinal cord lesions. We present a case of histologically proved non-enhancing intramedullary spinal cord pilocytic astrocytoma in a 12-year-old girl. This case represents an exception to the conventional wisdom that pediatric spinal neoplasms enhance with administration of intravenous contrast material.

Adrenal Cortical Adenoma in the Spinal Canal: A Case Report and Review of the Literature.

Ectopic adrenal cortical neoplasms of the spinal cord are extremely rare. To date only 10 such cases have been described. We present a case of a 46-year-old woman with lower back pain radiating to the right gluteal and posterior femoral regions, without a history of traumatic injury. Magnetic resonance imaging (MRI) of the thoracic and lumbar spine showed an intradural, extramedullary, well-circumscribed, contrast-enhancing lesion located in the T12-L1 region, hypo- to isointense on T2-weighted imaging, and isointense on T1. Disparities in reportable quality metrics by insurance status in the primary spine neoplasm population.

BACKGROUND CONTEXT: The Centers for Medicare and Medicaid Services (CMS) defines "adverse quality events" as the incidence of certain complications such as postsurgical hematoma or iatrogenic pneumothorax during an inpatient stay. Patient safety indicators (PSI) are a means to measure the incidence of these adverse events. When adverse events occur, reimbursement to the hospital decreases. The incidence of adverse quality events among patients hospitalized for primary spinal neoplasms is unknown. Similarly, it is unclear what the impact of insurance status is on adverse care quality among this

[Neuroradiology of primary and secondary spinal tumors].

The recent availability of spinal CT has modified the classic diagnostic algorithm of spinal and spinal cord tumors. The most important diagnostic signs given by traditional radiologic study of the spine (postural defects, signs of dysraphism, alterations of peduncles and neural foramina , soft paraspinal tissue changes, intra- and extra-vertebral calcifications, etc.), by myelography with hydrosoluble non ionic contrast medium and by spinal angiography are described. Their complementary indications and their up-to-date role in the diagnosis of site, size and nature of spinal neoplasms are dis

A "gullwing sign" on magnetic resonance imaging of extradural spinal tumors in dogs and cats allows prioritization of round cell neoplasia.

Extradural neoplasms are the most common spinal tumors in small animals. A bilobed appearance of ventral extradural spinal lesions ("gullwing sign") on MRI has been described with various conditions. The objective of this retrospective study was to determine if a "gullwing sign" is more common with certain types of extradural tumors. MRI studies of dogs and cats with extradural spinal neoplasms were reviewed for the presence of a "gullwing sign". Statistical analysis was performed to evaluate a possible relationship between tumor class and the presence of a "gullwing sign". Sixty-six cases wer

Developmental vascular anomalies of the spinal cord as a background for post radiation necrotizing myelopathy.

Radiation myelopathy is a well-known complication of irradiation therapy of neoplasms in the vicinity of the spinal cord. In the case presented myelopathy developed after therapeutic Co 60 irradiation of lingual carcinoma on the background of spinal vasculo-dysraphic malformation. The pathogenetic mechanism of necrotic myelopathy in this case is discussed.
