BACKGROUND: The nerve root sedimentation sign is a magnetic resonance (MR) sign, shown to be present in central lumbar spinal stenosis. The lack of sedimentation of the nerve roots to the dorsal part of the dural sac is consistent with the positive nerve root sedimentation sign.
PURPOSE: To validate the reliability of the nerve root sedimentation sign in diagnosis of different grades of lumbar spinal canal stenosis.
MATERIAL AND METHODS: This study was a retrospective review of 101 consecutive MR imaging (MRI) studies obtained on patients with clinically suspected lumbar canal stenosis. Base

The purpose of this study was to define the spinal root origins of the lower subscapular nerve and the amounts of participating nerve fibers from each spinal root and to discuss the clinical implications. Using a method of separating the nerve fascicles that traces the particular nerve fibers at the intrafascicular level, the spinal root origins of the lower subscapular nerve appeared as two types. The first type comprised 76.9% and was composed of the C5, C6, and C7 roots; the second type comprised 23.1% and was composed of the C6 and C7 roots. The number of nerve fibers was 357.2 +/- 139.7 (

Twelve cervical spines from C2 to T1 were harvested from embalmed cadavers to study the location of the nerve root on plain radiographs. After removal of the soft tissue, the spinal nerves just lateral to the transverse processes were exposed and injected with lead oxide. Plain radiographs including anteroposterior (AP) and left and right oblique views were taken. Angular and linear measurements were performed directly on the radiographs. Results showed that the average frontal angle of the nerve root for all levels was 155 degrees on the AP view, 108 degrees on the foraminal side of the obliq

We describe a technique of magnetic coil (MC) stimulation of the thoracic spinal nerves and roots in 12 normal subjects and a patient with diabetes mellitus. We kept the MC flat against the vertebral column in the midline over T-7, T-8, and T-9 spinous processes and obtained compound muscle action potentials from the upper rectus abdominis, external oblique, and intercostal muscles. We obtained mean latencies to these muscles after stimulation in the posterior axillary line. We noted that the onset latencies remained fixed despite increasing the intensity of stimulation from 30% to 100% and on

Spinal nerve root compression occurs commonly in conditions such as herniated nucleus pulposus, spinal stenosis, and trauma. However, the pathophysiology of the symptoms and signs related to spinal nerve root compression is poorly understood. The purpose of the present study was to assess and compare effects of various pressures and durations of acute compression on spinal nerve root conduction in the pig cauda equina. Efferent conduction (compound motor action potentials) and afferent conduction (compound nerve action potentials) were monitored during compression for 2 or 4 hours with compres

OBJECTIVE: Redundant nerve roots (RNR) of the lumbar cauda equina are an important imaging presentation associated with lumbar spinal stenosis (LSS) and considered a potentially powerful prognostic indicator of poor functional recovery after surgery for symptomatic LSS. However, factors contributing to RNR remain undefined. This study aimed to investigate the effects of sagittal and transverse stenosis of the lumbar spinal canal on RNR formation.
METHODS: A retrospective case-control study was conducted in 124 LSS patients over 18 years of age. They were divided into an RNR group and a non-RN

Primary central nervous system lymphoma is an extranodal lymphoma originating in the brain, spinal cord, and leptomeninges, which rarely spreads outside the nervous system. Neurolymphomatosis is the term used to describe lymphomatous involvement of the cranial nerves (except optic nerve) and peripheral nerve roots or plexus. We report a case of a 23-year-old man presented with progressive weakness of all 4 limbs. F-FDG PET/CT revealed FDG-avid thickening along the multiple cranial nerve roots, spinal nerve roots, and brain parenchymal lesion. Core biopsy from the peripheral nerve root demonstr

A case is presented of an avulsion injury of the left brachial plexus in a woman who--after surgical reconstruction of the plexus--developed a liquid fistula from a giant pseudocyst localized in this area. A prolonged persistence of the pseudocyst resulted in a severe pain syndrome and symptoms of vascular disorders in the upper limb. Since results of local treatment (punctures, pressure dressings) were unsatisfactory, the authors decided to close the liquid fistula by suturing the damaged dural sac by means of laminectomy. The necessity of particular caution when operating in the area of inte

STUDY DESIGN: A lumbar radiculopathy model investigated pain behavioral responses after nerve root reinjury.
OBJECTIVES: To gain a further understanding of central sensitization and neuroinflammation associated with chronic lumbar radiculopathy after repeated nerve root injury.
SUMMARY OF BACKGROUND DATA: The pathophysiologic mechanisms associated with chronic radicular pain remain obscure. It has been hypothesized that lumbar root injury produces neuroimmunologic and neurochemical changes, sensitizing the spinal cord and causing pain responses to manifest with greater intensity and longer d

STUDY DESIGN: This study investigated the use of mechanically elicited electromyograms during the placement of pedicle screws in 89 patients undergoing surgery for spinal stenosis.
SUMMARY OF BACKGROUND DATA: Several methods for monitoring nerve roots are available. However, mechanically elicited electromyograms may be more sensitive to mechanical irritation of nerve roots by pedicle screws than by other methods.
METHODS: Mechanically elicited electromyograms were recorded in muscle groups innervated by cervical or lumbar nerve roots. Confirmation of surgical activity with the level of the e

OBJECTIVE: Redundant nerve roots (RNRs) seen in conjunction with lumbar spinal stenosis (LSS) are well-described radiographic findings. Several studies suggest their presence may be a negative prognostic indicator of postoperative outcome. Our hypothesis was that severe RNR (informally known as the spaghetti sign [SS]) can serve as a reliable marker of LSS that would benefit from surgical decompression. We sought to evaluate a grading scale for RNR, characterize the association with stenosis, and investigate the clinical implications of RNR.
METHODS: We conducted a retrospective chart review

OBJECTIVE: Spinal cord stimulation (SCS) has been used as a minimally invasive and effective treatment modality for various chronic pain disorders, with the main target being stimulation of the dorsal columns; however, certain neuropathic pain areas involve dermatomes that are suboptimally covered by SCS. Stimulation of the spinal nerve roots has the advantage of targeting one or several dermatomes at the same time. The aim of this systematic review is to investigate the efficacy of spinal nerve root stimulation (SNRS) for chronic pain disorders.

Pain originating from spinal nerve roots demonstrates multiple pathogeneses. Distinctions in the patho-anatomy, biomechanics, and pathophysiology of spinal nerve roots contribute to pathology, diagnosis, and management of root-related pain. Root-related pain can emerge from the tension events in the dura mater and nerve tissue associated with primary disc related disorders. Conversely, secondary disc-related degeneration can produce compression on the nerve roots. This compression can result in chemical and mechanical consequences imposed on the nervous tissue within the spinal canal, lateral

BACKGROUND: Spinal cord compression secondary to nerve root hypertrophy is often attributed to hereditary neuropathies. However, to avoid misdiagnosis, rare immune-mediated neuropathy such as chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) should not be overlooked. This report presents a case of multilevel nerve root hypertrophy leading to significant cord compression from CIDP.
CASE DESCRIPTION: We report a 56-year-old gentleman with type two diabetes mellitus who presented with subacute cervical cord syndrome following a fall. Mixed upper and lower motor neuron features wer

Redundant nerve root syndrome (RNRS) is a phenomenon characterized by the presence of elongated, enlarged, tortuous nerve roots in the lumbar subarachnoid space. It is unclear whether RNRS is caused by spinal stenosis or causes these symptoms. This study evaluated the effects of lumbar epidural steroid injection (LESI) on patients with RNRS and assessed factors associated with RNRS. This retrospective observational cohort study was conducted at a single pain clinic of a university hospital. The medical records of 172 outpatients presenting with low back and/or leg pain from July to December 2014

STUDY DESIGN: Nerve conduction velocity was studied in the dog cauda equina subjected to chronic and, 1 week later, additional acute compression.
OBJECTIVES: To model the situation of spinal stenosis by first inducing a controlled, chronic compression injury and then to add compression at various pressure levels to the already compressed cauda equina.
SUMMARY OF BACKGROUND DATA: Previous models on nerve root and cauda equina compression have analyzed the effects of compression only on noninjured nerves. Clinically, the nerves that are compressed in spinal stenosis already are exposed to chro
