A cross-modal association between somatosensory tactile sensation and parietal and occipital activities during Braille reading was initially discovered in tests with blind subjects, with sighted and blindfolded healthy subjects used as controls. However, the neural background of oral stereognosis remains unclear. In the present study, we investigated whether the parietal and occipital cortices are activated during shape discrimination by the mouth using functional near-infrared spectroscopy (fNIRS). Following presentation of the test piece shape, a sham discrimination trial without the test pi

Perioral and distal upper limb sensory dysfunction (cheiro-oral syndrome) has classically been attributed to cortical involvement. In previously reported cases of the syndrome, caused by stroke, however, the thalamus or brain stem has been the actual site of the lesion. We have studied two patients with infarct in the superficial middle cerebral artery territory involving the parietal operculum. Sensory involvement was purely subjective in the face, but severe hypoesthesia was present in the distal upper limb, involving mainly position sense, stereognosis, and graphesthesia. Temperature and pa

We studied the distribution of the median nerve somatosensory evoked potentials (SEPs) on the scalp of a 67-year-old woman with a localized old hematoma involving the posterior part of the right postcentral gyrus identified by MRI. The patient presented with an isolated loss of discriminative sensation (stereognosis, graphesthesia and position sense) of the left extremities. With respect to the SEPs on the right hemisphere, the only components remaining were the initial negativity (N20 with latency of about 20 msec) recorded in the parieto-occipital area, which exhibited polarity inversion acr

Oral form identification scores were obtained for nine anterior and nine posterior left-hemisphere brain lesion subjects exhibiting speech apraxic behavior and aphasia. An objective measure of speech apraxic difficulty and a standardized aphasia assessment were obtained for each subject. Despite the traditional association of a cortical sensory deficit including asterognosis in posterior brain-injured patients, oral form identification scores did not differ significantly for the two subject groups. However, a previously reported heterogeneity of performance was observed in the present data. Th

The Tactual Performance Test (TPT) is reviewed because of its widespread use in neuropsychology. The review indicates that: (1) each of the three TPT scores (Time, Memory, Location), while having considerable overlap in what is measured, contributes something unique; (2) test-retest reliability is adequate, although TPT Memory and Location are generally less reliable than TPT Time; (3) TPT Time has proven to be a sensitive and valid measure of dysfunction in heterogeneous populations of brain-damaged subjects; (4) subject variables of importance to TPT performance are age, IQ, sex, and, to a m

Mental rotation of objects improves when passive tactile information for the rotating object accompanies the imagined rotation (Wraga, Creem, & Proffitt, 2000). We examined this phenomenon further using a within-subjects paradigm involving handheld objects. In Experiment 1, participants imagined rotating an unseen object placed on their upturned palms. The participants were faster at mental rotation when the object was rotated on their palm than when the object remained stationary. Experiment 2 tested whether the performance advantage would endure when the participants received tactile informa

Subjects attempted to recognize simple line drawings of common objects using either touch or vision. In the touch condition, subjects explored raised line drawings using the distal pad of the index finger or the distal pads both of the index and of the middle fingers. In the visual condition, a computer-driven display was used to simulate tactual exploration. By moving an electronic pen over a digitizing tablet, the subject could explore a line drawing stored in memory; on the display screen a portion of the drawing appeared to move behind a stationary aperture, in concert with the movement of

A test of memory and learning with guaranteed initial stimulus-processing was validated and is here presented with norms for 70-79 year old and 80-89 year old community residents vs. nursing-home residents. The test procedure also allows incidental observation of stereognosis, object-naming, and left-right orientation, screens for depression, and elicits rapid retrieval of words from semantic categories--all common aspects of evaluation for dementia. A validation study showed a significant difference between 21 moderately impaired and 21 unimpaired elderly nursing home residents grouped by the

Inter- and intrahemispheric somatosensory functions were evaluated in blinded tests of 11 children with partial and complete agenesis of the corpus callosum (age range 2 years 8 months to 11 years 9 months) and compared with those of age- and sex-matched control individuals both normally developing and with neurological impairment. Tests included uni- and bimanual measures of stereognosis, haptic visual discrimination, kinaesthesis, and texture matching. Children with callosal absence displayed significant difficulties with many somatosensory functions, but most deficits were no greater than t

Tests for graphesthesia and for directional cutaneous kinesthesia (DCK) were performed on a large series of neurological patients and normal subjects, in addition to the standard tests for discriminative sensation. Defects in graphesthesia and DCK were found with lesions at all levels of the nervous system. Graphesthesia was more often and more severely affected than DCK. These functions when impaired were always associated with other sensory defects (directional joint kinesthesia, two-point discrimination, etc.) in different combinations. It appears DCK is probably the basis for graphesthesia

We report on a patient with multiple sclerosis (MS) who developed bilateral useless hand syndrome (UHS) and astereognosis. Clinical features of UHS in our patient are similar to limb-kinetic apraxia with astereognosis. Cervical T1-weighted magnetic resonance imaging revealed a gadolinium-enhanced lesion at the level of C3-C4, mainly involving the posterior cord. This patient suggests that posterior cord lesions at high cervical levels cause UHS and astereognosis in MS, involving the sensorimotor integration needed to execute complex finger movements.

The question as to whether a failure of recognition unrelated to impaired sensory processing or to disorder of naming can occur in the somato-sensory modality has been eagerly debated in the french neurology. Taking as an argument the fact that he had never observed a tactile agnosia in the absence of subtle sensory deficits Dejerine denied the localizing value of tactile agnosia (or asterognosis). Conversely Delay, 20 years later, identified tactile performances such as discrimination of texture and shapes, which he considered as a specific neocortical function, that were lost in parietal syn

We report a patient with a hemorrhagic lesion encroaching upon the posterior third of the corpus callosum but sparing the splenium. She showed marked difficulties in recognizing objects and shapes perceived through her left hand, while she could appreciate elementary sensorial features of items tactually presented to the same hand flawlessly. This picture, corresponding to classical descriptions of unilateral associative tactile agnosia, was associated with finger agnosia of the left hand. This very unusual case report can be interpreted as an instance of disconnection syndrome, and allows a d

1. Somatosensory evoked potentials are abnormal in a large percentage of patients with severe multiple sclerosis if amplitude, latency and right-left asymmetry are all considered. Mild disease has remained difficult to identify. This screening test employing two stimulus sites in the same arm did not improve the yield. 2. Stimulation of the median nerve at the elbow is not often likely to justify the associated discomfort, except in patients with known disease in the forearm. 3. Measuring the central conduction velocities from easily recorded low cervical potentials up to the parietal scalp de

BACKGROUND: Prevalence of cobalamin deficiency is high especially in older patients and an immediate therapy start is necessary to prevent irreversible neurological damages. Unfortunately, the diagnosis of cobalamin deficiency is difficult and at present, there is no consensus for diagnosis of this deficiency. Therefore, we aim to elucidate a meaningful diagnostic pathway by a case report with an initially misleading medical history.
CASE PRESENTATION: A 57 year-old Caucasian man suffering from dramatic myelosis of the cervical posterior columns. Apart from associated neurological symptoms (t

BACKGROUND: Useless hand syndrome (UHS) refers to clumsiness of complex finger movements and loss of manual dexterity due to cervical cord lesions, often associated with multiple sclerosis (MS). This condition may represent the first demyelinating event in MS or may occur in a patient with pre-existing MS.
METHODS: We report 3 patients with UHS, 2 of whom had not been previously diagnosed with MS. The history and physical examinations, EMG/NCV studies of the arms, and radiological findings are presented.
RESULTS: Presenting symptoms in all 3 cases included paresthesia and difficulty with man

Various concomitant disorders in 535 children with infantile hemiplegia were evaluated; 159 of the cases were particularly well documented. The right/left distribution of the hemiplegia was 56 to 44, the ratio of boys to girls 59 to 41. Severe impairments of hand function (lack of function, first grip) were found more frequently in right hemiplegics than left; the ratio of severe to slight disorders was approx. 3 to 1 in right hemiplegics and 3 to 2 in left hemiplegics. Impairment of hand function was closely related to the quality of sensitivity, the tests of this being based on two-point dis

Adults with mild mental retardation and equal-MA children and adults without mental retardation were required to (a) tactually examine single letters, two-letter words, bigrams, and Chinese characters with their right or left index finger and (b) indicate whether a visually presented stimulus was the tactually examined stimulus by saying "same" or "different." The left hand was significantly better for "same" responses and the right hand, for "different" responses, suggesting that hemispheric processing is dependent on information-processing requirements rather than type of stimulus.
