Esophageal stethoscope in thoracoscopic interruption of patent ductus arteriosus.

There is a significant rate of residual or recurrent ductal patency after video-assisted thoracoscopic closure of patent ductus arteriosus. Between February 2000 and October 2004, this procedure was carried out on 145 consecutive patients in whom heart sounds were monitored intraoperatively with an esophageal stethoscope. Changes in continuous cardiac murmurs were recorded after placing the 1(st) and 2(nd) vascular clips. There was no ductal flow after clipping twice in 138 (95%) patients; in the other 7, residual flow was abolished at the 3(rd) attempt. All patients left the operating room wi

Development of a tele-stethoscope and its application in pediatric cardiology.

Over the years, many attempts have been made to develop special stethoscopes for the teaching of auscultation. The objective of this article is to report on the experience with the development and implementation of an electronic stethoscope and a virtual library of cardiac sounds. There were four stages to this project: (1) the building of the prototype to acquire, filter and amplify the cardiac sounds, (2) the development of a software program to record, reproduce and visualize them, (3) the testing of the prototype in a clinical scenario, and (4) the development of an internet site, to store

Chest auscultation - tradition and expectations Concerns and promises in the era of smart stethoscopes.

AI-assisted digital stethoscopes can classify heart and lung sounds with accuracy comparable to or exceeding that of the average clinician and help the physician in the diagnostic process. With this essay we want to discuss benefits and downsides of this new technology. Will proficiency in auscultation, which brings joy and pride to many stethoscope users, be suffering when interpretation of chest sounds is taken over by AI? And how will recommendations from AI in the diagnostic process influence decision making and the patient-doctor relationship? A tendency toward deskilling of auscultation

Are Simulation Stethoscopes a Useful Adjunct for Emergency Residents' Training on High-fidelity Mannequins?

INTRODUCTION: Emergency medicine residents use simulation training for many reasons, such as gaining experience with critically ill patients and becoming familiar with disease processes. Residents frequently criticize simulation training using current high-fidelity mannequins due to the poor quality of physical exam findings present, such as auscultatory findings, as it may lead them down an alternate diagnostic or therapeutic pathway. Recently wireless remote programmed stethoscopes (simulation stethoscopes) have been developed that allow wireless transmission of any sound to a stethoscope re

Comparison of conventional and sensor-based electronic stethoscopes in detecting cardiac murmurs of dogs.

BACKGROUND AND OBJECTIVE: Cardiac auscultation is one of the most important parts of the cardiological examination traditionally performed with acoustic stethoscopes. The aim of this study was to compare the sensitivities and the diagnostic capabilities of traditional and electronic stethoscopes in detecting canine heart murmurs. MATERIALS AND METHODS: The study was performed on 21 dogs referred for cardiologic examination with suspected heart murmurs. Six out of these dogs had cardiac murmurs bilaterally. Cardiac auscultation was performed independently by a final-year veterinary student (AB

Bacterial Assessment of Stethoscopes Used by Healthcare Workers at a Tertiary Care Government Hospital in Bharatpur, Nepal.

The transmission of healthcare-associated infections (HCAIs) in healthcare settings is a serious challenge in the medical fraternity. Medical devices, such as stethoscopes used by healthcare workers (HCWs), are likely to harbor a considerable number of pathogenic microbes, which may result in the transmission of HCAIs. This study sought to investigate bacterial contamination of stethoscopes used by HCWs at Bharatpur Hospital, Nepal. During the study period of 3 months from December 2019 to February 2020, a total of 87 stethoscopes were examined; bacterial pathogens were isolated and identified

Comparing the auscultatory accuracy of health care professionals using three different brands of stethoscopes on a simulator.

BACKGROUND: It is considered standard practice to use disposable or patient-dedicated stethoscopes to prevent cross-contamination between patients in contact precautions and others in their vicinity. The literature offers very little information regarding the quality of currently used stethoscopes. This study assessed the fidelity with which acoustics were perceived by a broad range of health care professionals using three brands of stethoscopes. METHODS: This prospective study used a simulation center and volunteer health care professionals to test the sound quality offered by three brands o

Cardiac auscultation training of medical students: a comparison of electronic sensor-based and acoustic stethoscopes.

BACKGROUND: To determine whether the use of an electronic, sensor based stethoscope affects the cardiac auscultation skills of undergraduate medical students. METHODS: Forty eight third year medical students were randomized to use either an electronic stethoscope, or a conventional acoustic stethoscope during clinical auscultation training. After a training period of four months, cardiac auscultation skills were evaluated using four patients with different cardiac murmurs. Two experienced cardiologists determined correct answers. The students completed a questionnaire for each patient. The th

From stethoscope to ultrasound: an Italian survey of nephrology ultrasound practice.

BACKGROUND: Ultrasound (US) is an increasingly utilized imaging technique in nephrology,  for diagnostic and interventional applications. In spite of growing interest, its implementation across Italian nephrology centers remains poorly documented. To address this gap, a cross-sectional study was conducted to explore the modalities of the training of expert nephrologists and current practices of diagnostic and interventional US procedures  performed by nephrologists in Italy. METHODS: We developed a 24-question electronic survey that collected data on (1) participants' background (years in US

Evaluation of a digital stethoscope for electrocardiographic recording in donkeys: Preliminary results.

The digital stethoscope (DS) is a cost-effective single-lead digital stethoscope that allows simultaneous electrocardiographic (ECG) and phonocardiographic recordings on a smartphone. Despite its application in small animals and horses, there are currently no studies on its use in donkeys. The aim of this study was to evaluate the use of a new smartphone-based DS device in recording ECG tracings in donkeys. Standard base-apex lead ECG (sECG) and single-lead DS ECG (dECG) were simultaneously recorded for at least 30 s. Both sECG and dECG tracings were analysed by the same operator, recording he

The Evolving Stethoscope: Insights Derived from Studying Phonocardiography in Trainees.

Phonocardiography (PCG) is used as an adjunct to teach cardiac auscultation and is now a function of PCG-capable stethoscopes (PCS). To evaluate the efficacy of PCG and PCS, the authors investigated the impact of providing PCG data and PCSs on how frequently murmurs, rubs, and gallops (MRGs) were correctly identified by third-year medical students. Following their internal medicine rotation, third-year medical students from the Georgetown University School of Medicine completed a standardized auscultation assessment. Sound files of 10 different MRGs with a corresponding clinical vignette and p

Stethoscope hygiene: A call to action. Recommendations to update the CDC guidelines.

Healthcare-acquired infections are a tremendous challenge to the US medical system. Stethoscopes touch many patients, but current guidance from the Centers for Disease Control and Prevention does not support disinfection between each patient. Stethoscopes are rarely disinfected between patients by healthcare providers. When cultured, even after disinfection, stethoscopes have high rates of pathogen contamination, identical to that of unwashed hands. The consequence of these practices may bode poorly in the coronavirus 2019 disease (COVID-19) pandemic. Alternatively, the CDC recommends the use
