Endoscopic and stroboscopic description of adults with paradoxical vocal fold dysfunction.

The purpose of this study was to determine if endoscopic and stroboscopic parameters of voice were normal between attacks of paradoxical vocal fold dysfunction (PVFD). Fifty adults (38 females, 12 males) and 54 adult controls (40 females, 14 males) were examined via endoscopy with and without stroboscopy. Endoscopy indicated paradoxical adduction of the folds during the respiratory cycle of all 50 participants with PVFD, although they were asymptomatic. Atypical laryngeal configurations were observed including abnormality of the anterior-posterior dimension and ventricular fold medialization i

Platelet Rich Plasma (PRP) Treatment in Recalcitrant, Bilateral Phono-traumatic Lesions.

OBJECTIVE: Mature vocal fold nodules and fibrous phono-traumatic lesions that have failed standard voice therapy can be challenging. Autologous platelet-rich plasma (PRP) is rich in growth factors and easily obtained from a simple blood draw. Growth factor injection into the vocal folds may reverse the scar and inflammation in recalcitrant, bilateral phono-traumatic lesions such as mature vocal fold nodules. This study reports the short-term (3months) voice results after PRP injection in 11 subjects with recalcitrant, bilateral phono-traumatic lesions that have failed voice therapy. MATERIALS

Treatment of vocal fold scar by carbon dioxide laser and collagen injection: retrospective study on 12 patients.

Vocal fold scars are the result of injury to the vocal fold lamina propria. This condition leads to an impaired vibration and usually to poor voice quality. The purpose of our study was to compare the pre- and posttreatment voice assessment scores in patients treated by CO(2) laser-assisted freeing of the scar followed by collagen injection. A group of 12 patients (10 women and 2 men) with vocal fold scars was studied retrospectively. Voice assessment was based on stroboscopy and on perceptual scores using the Grade (G) of the GRABS scale, subjective evaluation by the use of the Voice Handicap

Does Even Low-Grade Dysphonia Warrant Voice Center Referral?

OBJECTIVE: Data regarding the referral of dysphonic patients to specialty voice clinics are limited. The objective of this study is to examine the relationship between low perceptual dysphonia severity and subtle laryngeal findings to discern if this can help guide referral. STUDY DESIGN: This is a retrospective chart review. METHODS: The charts of 94 patients presenting with a primary complaint of hoarseness to a single laryngologist over a 1-year period at a tertiary care, interdisciplinary voice center were analyzed. Patients were stratified by clinician perceptual rating of dysphonia sev

Surgery for voice.

Preoperative assessment in a multidisciplinary voice clinic, using stroboscopy, is essential. In the treatment of benign vocal fold mucosal disorders precise phonosurgical techniques, using high magnification which respects the multilayered structured of the vocal fold, are mandatory. A paralysed vocal fold can be assessed by electromyography and medialization thyroplasty procedures offer excellent 'fine tuning' of the voice.

[Pharmacotherapy of voice and articulation disorders in aphasia].

The phonation and articulation disorders in a group of aphasics with aphonia/dysarthrophonia could be considerably reduced by the use of medication. The group consisted of 10 apoplectic patients whose resulting aphasia could not be classified because of the vocal impairment. Extrapyramidal motion disorders were proved by laryngoscopy and stroboscopy. A definite improvement of phonation and articulation was observed after L-dopa medication.

The use of the pulse dye laser in the treatment of vocal fold scar: a preliminary study.

OBJECTIVES/HYPOTHESIS: Dysphonia due to vocal fold scarring is a challenging problem to the laryngologist. Vocal fold scarring after radiation, phonosurgery, and laser cordectomy causes moderate to severe dysphonia. Surgical attempts at scar removal and voice restoration have limited success. Pulsed dye laser (PDL) treatment has been shown to be effective in softening scarred skin by serial office treatments. The objective of this preliminary study was to evaluate the use of the PDL in the management of patients with established vocal fold scar. STUDY DESIGN: This is an Institutional Review B

Preoperative Voice Characteristics in Thyroid Patients.

INTRODUCTION: Thyroid surgery outcomes have evolved from mortality control strategies to morbidity control measures. Most vocal outcomes research in thyroid surgery are focused on recurrent nerve anatomic and functional preservation. However, there are likely multiple causes of vocal dysfunction in thyroid patients. We prospectively analyzed preoperative patients with thyroid disease to define preoperative vocal characteristics of this population. OBJECTIVE: Quantify vocal and laryngeal baseline conditions in thyroid surgical patients. METHODS: Prospective study of preoperative stroboscopy f

Qualification of a quantitative laryngeal imaging system using videostroboscopy and videokymography.

OBJECTIVES: We sought to determine whether full-cycle glottal width measurements could be obtained with a quantitative laryngeal imaging system using videostroboscopy, and whether glottal width and vocal fold length measurements were repeatable and reliable. METHODS: Synthetic vocal folds were phonated on a laboratory bench, and dynamic images were obtained in repeated trials by use of videostroboscopy and videokymography (VKG) with an imaging system equipped with a 2-point laser projection device for measuring absolute dimensions. Video images were also obtained with an industrial videoscope

High-speed imaging used to detect vocal fold paresis: a case report.

High-speed imaging has been used to study vocal fold vibration and has been shown to provide additional information in aid of our understanding of pathologic vocal fold vibration. This is the first case report of vocal fold paresis diagnosed by high-speed imaging. An 18-year-old girl presented with intermittent voice loss that had been present for 4 years. The patient had been seen by other otolaryngologists and had been given proton pump inhibitors without any improvement in her voice. Her voice was diplophonic. The patient was examined by rigid stroboscopy and was found to have a predominant

Common practices of voice therapists in the evaluation of patients.

This study surveys voice therapists regarding common diagnostic practices in patients referred for therapy with the diagnosis of muscle tension dysphonia (broadly defined as the "hyperfunctional" component of the dysphonia). Through postings on the e-mail list of the ASHA special interest division on voice, speech pathologists with at least 3 years' experience in stroboscopy and acoustic instrumentation were invited to complete the survey. Results from 53 completed surveys demonstrated that voice quality and patient self-perception are the sole assessments performed by all therapists. Voice qu

Ansa cervicalis-to-recurrent laryngeal nerve anastomosis for unilateral vocal fold paralysis: experience of a single institution.

OBJECTIVES: One treatment option for unilateral vocal fold paralysis (UVFP) is ansa cervicalis-to-recurrent laryngeal nerve (ansa-RLN) anastomosis to provide reinnervation to the affected vocal fold. The advantages of this treatment approach are that it 1) provides vocal fold tone, bulk, and tension, 2) is technically simple, and 3) does not preclude other medialization procedures. We present all patients who have undergone ansa-RLN anastomosis for UVFP at our institution. METHODS: An Institutional Review Board-approved retrospective chart review was performed to include all patients who had

Perceptual and laboratory assessment of dysphonia.

The voice laboratory is an essential tool in the voice clinic. It provides a functional diagnosis of disturbed voice production, demonstrating the deviant characteristics, limitations, and possibilities for change. As voice is multidimensional, several aspects need to be documented, quantified, and analyzed: perception, stroboscopy, aerodynamics, acoustics, self-evaluation by the patient, and in specific cases, physiologic signals, such as electroglottography, flow glottography, nasometry, and electromyography. Effectiveness and outcome studies rely on such data.

[Technique of thyroid gland operation: prevention of lesions of the superior laryngeal nerve].

The anatomical variant of the external branch of the superior laryngeal nerve is at risk during thyroid surgery. We recommend a careful dissection of the upper thyroid pole strictly avoiding even minor mass ligatures and all vascular structures have to be selectively identified. Of a consecutive series of 161 upper pole dissections the superior laryngeal nerve was found in 31% and no postoperative injury was detected by video stroboscopy.

Diagnosis and treatment of intracordal cysts.

Seventy-one cases of intracordal cysts are reported. They were all diagnosed between 1970 and 1980: 53 were epidermoid and 18 retention cysts. They caused longstanding dysphonia characterized by lowered tonality, huskiness, desonorization, and vocal strain. Clinical signs were often discrete and stroboscopy revealed a lack of vibrations on the cystic cord. Surgery was followed by vocal re-education and there was a high percentage of good vocal results.
