Effects of salicylate on intestinal absorption: in vitro and in vivo studies with enterotoxigenic micro-organisms.

This study was done to determine whether salicylate could affect alterations in intestinal absorption induced by preparations of enterotoxigenic micro-organisms. It was found in rats that salicylate increased intestinal monosaccharide uptake in vitro and reversed the inhibitory effects induced by cell-free preparations of Staphylococcus sp., Candida sp. and Klebsiella sp. In vivo, salicylates increased net water absorption in rat jejunum exposed to cell-free preparations of various micro-organisms. Increase in net fluid flux occurred after subcutaneous injection only with bacteria which stimul

Subcutaneous injection of metallic mercury.

Deliberate self-injection of metallic mercury into subcutaneous tissue is uncommon. A 41-year-old lady with a history of schizophrenia was admitted to our hospital after deliberate injection of metallic mercury into her right wrist and antecubital fossa. Physical examination was unremarkable except for the injection marks over right antecubital fossa and wrist. The presence of subcutaneous mercury deposits in her right elbow and wrist was confirmed by X-rays and ultrasound scan. Three days later, erythema, swelling, induration and tenderness were seen over the injection sites. At the operation

Safety, tolerability and pharmacokinetics of subcutaneous meropenem as an alternative to intravenous administration.

BACKGROUND: Subcutaneous delivery of antibiotics is a practical alternative to IV administration. Meropenem is commonly used to treat infections caused by resistant Gram-negative organisms. METHODS: This was a prospective, crossover self-controlled study in 11 stable inpatients established on meropenem. Participants received a single dose of subcutaneous meropenem, in 50 mL normal saline via gravity feed. Venous blood sampling was performed at baseline, 0.5, 1, 2, 4 and 8 h following the subcutaneous and IV doses. Antibiotic concentrations were measured using UPLC-MS/MS. Pharmacokinetic data

Is Oral Semaglutide a Good Fit for Patients After Metabolic Bariatric Surgery? A Biopharmaceutical Mechanistic Perspective.

Currently, GLP-1RAs are peptide drugs, typically administered by injection due to insufficient absorption, and only one GLP-1RA, semaglutide, is available as an orally administered drug. To overcome the absorption challenges of oral peptides, this drug product contains the absorption enhancer SNAC. As the tablet is eroded in the stomach, SNAC neutralizes the acidic gastric environment, thereby protecting the semaglutide from enzymatic degradation. Then, SNAC fluidizes the stomach lipidic membrane to increase semaglutide transcellular permeability across the gastric epithelium. It is necessary

Effect of serotonin treatment on intestinal transport in the rabbit.

The hormone serotonin (5-hydroxytryptamine) has been implicated as the cause of the diarrhea seen in many patients with the carcinoid syndrome. To determine whether serotonin is an intestinal secretagogue, the effect of serotonin on intestinal water and electrolyte transport was evaluated in the rabbit. Two weeks of daily subcutaneous injection of serotonin suspended in oil resulted in a blood serotonin level elevated to twice that of controls. Intestinal transport was studied in vivo by a perfusion technique. Serotonin treatment resulted in ileal secretion and decreased mid-jejunal absorption

Antivenom effect on lymphatic absorption and pharmacokinetics of coral snake venom using a large animal model.

Context: Historically, administration and dosing of antivenom (AV) have been guided primarily by physician judgment because of incomplete understanding of the envenomation process. As demonstrated previously, lymphatic absorption plays a major role in the availability and pharmacokinetics (PK) of coral snake venom injected subcutaneously, which suggests that absorption from subcutaneous tissue is the limiting step for venom bioavailability, supporting the notion that the bite site is an ongoing venom depot. This feature may underlie the recurrence phenomena reported in viperid envenomation tha

Rescue therapies for seizure emergencies: New modes of administration.

A subgroup of patients with drug-resistant epilepsy have seizure clusters, which are a part of the continuum of seizure emergencies that includes prolonged episodes and status epilepticus. When the patient or caregiver can identify the beginning of a cluster, the condition is amenable to certain treatments, an approach known as rescue therapy. Intravenous drug administration offers the fastest onset of action, but this route is usually not an option because most seizure clusters occur outside of a medical facility. Alternate routes of administration have been used or are proposed including rec

Maternal essential fatty acid supplementation increases zinc absorption in neonatal rats: relevance to the defect in zinc absorption in acrodermatitis enteropathica.

Pregnant zinc deficient and zinc adequate rats were injected subcutaneously with evening primrose oil throughout gestation and for 3 days post partum. The nursing pups were injected intragastrically with zinc-65 on day 3 of live and sacrifices 4 h later. The % of the total injected zinc recovered in the carcass (minus the gut and gut contents) was significantly increased in those pups nursed by mothers injected with evening primrose oil, regardless of their dietary zinc intake. The fatty acid composition of the total lipid extract of the gut and gut contents of the neonates with increased zinc

Overview of recombinant human hyaluronidase-facilitated subcutaneous infusion of IgG in primary immunodeficiencies.

Subcutaneous administration of immunoglobulin (IGSC) in a home setting, compared with intravenous administration, can improve patient quality of life. During IGSC, however, the subcutaneous extracellular matrix inhibits flow and fluid entry into the vascular compartment, which limits the amount of drug delivered. Recombinant human hyaluronidase (rHuPH20) increases the absorption and dispersion of infused fluids and drugs. Results from a Phase III, prospective, open-label, noncontrolled study of patients with primary immunodeficiencies indicated that IGSC infusion, facilitated by rHuPH20, is we

Subcutaneous emphysema, pneumomediastinum and pneumothorax complicating laparoscopic vagotomy. Report of two cases.

Two patients developed subcutaneous emphysema and pneumomediastinum during laparoscopic vagotomy. One of the patients also had a pneumothorax which produced a sudden increase in end-tidal carbon dioxide concentration preceding arterial oxygen desaturation. The pneumothorax was drained with an intercostal cannula. The patient required a twofold increase in minute ventilation to maintain normocarbia, probably because of the additional absorption of carbon dioxide through the pleural cavity. Despite the presence of a peritoneo-pleural communication, surgery was successfully completed. We believe

Sclerosing lipogranuloma of male genitalia. Review of 23 cases.

In 1950 the term "sclerosing lipogranuloma" was introduced to designate a praticular subcutaneous granulomatous reaction that was believed to be a "local reactive process following injury to adipose tissue." Subsequent reports have shown that exogenous lipid injections probably are responsible for this lesion. We have studied 23 cases involving penis, scrotum, spermatic cord, and perineum, nine of which had been previously reported. We have been able to identify paraffin hydrocarbons by means of infrared-absorption spectrophotometry in 21 of these cases. We belive this entity should be redefin

Disease activity in progressive multiple sclerosis can be effectively reduced by cladribine.

BACKGROUND: Evidence suggests people with non-relapsing deteriorating ("progressive") multiple sclerosis (pwPMS) may benefit from disease-modifying immune therapy (DMT). However, only one such treatment (ocrelizumab) has been licensed and is highly restricted to pwPMS suffering from the primary progressive phenotype. The difficulties assessing treatment outcome in pwPMS is one important reason for the lack of respective DMT. The concentration of neurofilaments in the cerebrospinal fluid (CSF) provides a biomarker of neuro-axonal damage, and both neurofilament light (NfL) and heavy chain (NfH)

Bioabsorbable System-Related Subcutaneous Swelling After Craniofacial Surgery.

BACKGROUND: Bioabsorbable systems have been commonly used in pediatric patients for primary cranioplasty and other related surgeries. However, subcutaneous swelling, a unique complication related to bioabsorbable osteosynthesis, is a concern. Differences in the incidence of subcutaneous swelling, depending on the bioabsorbable material used to construct the plate, are still unknown. METHODS: The authors retrospectively reviewed all incidences of subcutaneous swelling related to resorbable systems used during primary cranioplasty for patients with craniosynostosis at their hospital between 201

Insulin lispro: a fast-acting insulin analog.

Research has established the importance of maintaining blood glucose levels near normal in patients with type 1 (insulin-dependent) diabetes mellitus. Short-acting insulin analogs are designed to overcome the limitations of regular short-acting insulins. Compared with regular human insulin, the analog insulin lispro offers faster subcutaneous absorption, an earlier and greater insulin peak and a more rapid postpeak decrease. Insulin lispro begins to exert its effects within 15 minutes of subcutaneous administration, and peak levels occur 30 to 90 minutes after administration. Duration of activ

Improved nasal absorption of salmon calcitonin by powdery formulation with N-acetyl-L-cysteine as a mucolytic agent.

To establish a new formulation technology for the nasal delivery of peptide and protein drugs, we examined whether a mucolytic agent, N-acetyl-L-cysteine (NAC), could enhance the nasal absorption of a powder form of salmon calcitonin, a model peptide drug. We used ethylcellulose as an inert water-insoluble excipient. Various test formulations were prepared, and the effects on nasal absorbability were evaluated in rats and dogs. The powder formulation with NAC gave significant nasal absorption of SCT in both animal models, with absolute bioavailabilities of 30.0% in rats and 24.9% in dogs. Also

Subcutaneous Low-Density Foreign Bodies Detection via Grating-Based Multimodal X-ray Imaging.

Detecting low-density foreign bodies within soft tissues still stands for a serious challenge. Grating-based multimodal X-ray imaging typically has low hardware requirements while simultaneously providing three kinds of imaging information, i.e., absorption, phase-contrast, and dark-field. We aimed to explore the capacity of grating-based multimodal X-ray imaging technology for detecting common foreign bodies within subcutaneous tissues, and to assess the advantages as well as disadvantages of the three kinds of images obtained via grating-based X-ray multimodal technology in relation to diver

Oral delivery of insulin for treatment of diabetes: status quo, challenges and opportunities.

OBJECTIVES: Diabetes mellitus is characterised by progressive β-cell destruction and loss of function, or loss of ability of tissues to respond to insulin. Daily subcutaneous insulin injection is standard management for people with diabetes, although patient compliance is hard to achieve due to the inconvenience of injections, so other forms of delivery are being tested, including oral administration. This review summarises the developments in oral insulin administration. METHODS: The PubMed database was consulted to compile this review comparing conventional subcutaneous injection of insulin
