This paper reports data on the first national survey of sun-protection measures of Australian secondary school students. A cross-sectional survey of a representative sample of 23,915 Australian school children in Years 7-12 was conducted in 1993. Students were questioned about usual sun-protection practices, beliefs about skin cancer and suntans, and sunburn history. The results showed differences in sun-protection behaviours as a function of 'year level', 'gender', 'skin type' and 'concern about getting cancer' among others. A diagnosis of primary adrenocortical insufficiency was made in a shapely, suntanned girl whose sole complaint was increasing pigmentation. Plasma cortisol was low in spite of markedly elevated levels of ACTH. Plasma cortisol, urinary 17-oxogenic steroids and urinary aldosterone did not respond to three days of ACTH stimulation. Addison's disease can be diagnosed and treated before development of anorexia, weight loss, weakness and other classical symptoms.

Synopsis This paper reviews current ideas concerning the reactions involved in sunburn, suntanning, premature ageing and carcinogenesis due to acute or chronic exposure to sunlight. The physiological mechanisms of protection involving melanin pigmentation, thickening of the epidermal horny layer and urocanic acid are briefly discussed. The importance of wavelengths from 290-315 nm (UV-B) for these reactions is emphasized as is the need to investigate further the involvement of longer wavelength (UV-A, 315-400 nm).

Risk and preventive factors for skin phototype are discussed. Skin phototype was firstly proposed by Fitzpatrick based on skin response of the Caucasian, whereas the Japanese skin type was proposed for Japanese skin by Satoh and Kawada. Since skin phototype is determined by a personal interview of sunburn and suntan experience, it has a limitation in accuracy for personal interview and racial difference. The skin phototype concept is practicable and useful for predicting individual's sensitivity to UV, risk and preventive factors, and choosing sunscreens even with the limitation.

The awareness of 160 subjects about the dangers of over-exposure to the sun in relation to development of malignant melanoma was surveyed using a questionnaire. The survey found that despite wide acknowledgement of the dangers of ultraviolet radiation to the skin, the desire to get a suntan was still very strong and insufficient protection was being used. Advice continued to be required on all aspects of sun protection and on the importance of regularly examining the skin for changes in moles, a health education activity in which nurses have the skills to become fully involved.

Three women successively using the same sunbed received superficial to medium partial thickness burns as a result of an ultraviolet filter slipping out of place. Whilst the longer-term effects of the various spectra of ultraviolet (UV) light continue to be evaluated, the near-immediate cutaneous and/or ocular sequelae of excess exposure are well known. Cutaneous burns are not, however, immediately manifest and it is concluded that suntanning devices should incorporate failsafe mechanisms activated by filter slippage or other malfunction.

Using a standardized open photopatch test technique, the phototoxic reactions produced by bergamot oil bergapten (5-methoxypsoralen) the active component of the oil, and xanthotoxin (8-methoxypsoralen) were studied. Citropten (5,7-dimethoxycoumarin) and bergapten (5-methoxypsoralen) are present in bergamot oil which is used as a cosmetic tanning product. The aim of this study was to quantify, using HPLC, the amount of citropten and bergapten in the skin after the application of suntan products (emulsion and oil formulations). Suntan is known to be protective against sunburn resulting from medium-wavelength ultraviolet radiation. Protection against UV-B by UV-A-induced pigmentation was studied by the evaluation of the erythema produced and by histologic alterations in the epidermis, including sunburn cell counts.
