Skin Excursion with Cannula Lipodissection and the Anatomical Implications of Superficial Musculoaponeurotic System Movement in the Tumescent Face Lift.

BACKGROUND: The tumescent face lift uses the concept of subcutaneous cannula lipodissection of the neck, as opposed to wide skin undermining. Lipodissection mobilizes soft tissue while preserving perforating neurovascular branches. The purpose of this study is two-fold: (1) to compare the skin recruitment with subcutaneous lipodissection of the neck alone versus wide undermining and (2) to examine the effect of superiorly oriented superficial musculoaponeurotic system (SMAS) movement on jowl excursion in the tumescent face lift. METHODS: A cadaveric study was performed on five fresh tissue ca

Suprafibromuscular facelifting with periosteal suspension of the superficial musculoaponeurotic system and fat pad of Bichat rotation. Tightening the net.

OBJECTIVE: To design a facelifting technique that improves the safety of the facial nerve in extended facelifting; improves methods of fixation of the elevation of the nasolabial folds, the melolabial folds, the corner of the mouth and the malar fat pad; and augments the malar and submalar areas without implants. DESIGN: After a retrospective review of previous modified "composite" technique facelift results (307 patients over 4 years), a suprafibromuscular facelift technique was evolved through 22 fresh cadaver dissections. The resulting technique was applied to 73 patients, 61 females and 1

Histological insights into the nasal superficial musculoaponeurotic system implicating to oral and maxillofacial surgery - A preliminary study from cadavers.

BACKGROUND/PURPOSE: The superficial musculoaponeurotic system (SMAS) represents a pivotal component of midfacial soft tissue architecture, with significant implications for both aesthetic and reconstructive interventions. However, the histological characteristics of the nasal SMAS remain inadequately elucidated, particularly within Asian populations. This study aimed to characterize the histological features of the nasal SMAS in adult Vietnamese cadavers and to assess its potential relevance to oral and maxillofacial surgical procedures involving the midface. MATERIALS AND METHODS: Histologic

Extended Superficial Musculoaponeurotic System Face-Lift With Vertical Vector for Asian Patients: A 3-Dimensional Analysis of Facial Contour.

In this study, the authors report the experience of extended superficial musculoaponeurotic system (SMAS) face-lift with the vertical vector in Asian ethnicity and investigate the 3-dimensional change of facial contour. A total of 32 patients with Korean ethnicity underwent extended SMAS face-lift with vertical vector from 2015 to 2018. Patients with aging face were included for the study subjects, whereas those who were diagnosed with any craniofacial deformity were excluded. Using 3-dimensional photogrammetry, surface contour analysis was performed in the cheek region to calculate the mean,

Comparing the Safety and Efficacy of Superficial Musculoaponeurotic System and Deep Plane Facelift Techniques: A Systematic Review and Meta-analysis.

BACKGROUND: Facelift surgery (rhytidectomy) addresses skin laxity, soft tissue descent, and volume loss, with techniques such as the superficial musculoaponeurotic system (SMAS) and deep plane facelifts offering distinct advantages. However, the optimal technique remains debated due to differences in complication rates and aesthetic outcomes. OBJECTIVE: The aim of the study was to compare complication rates and aesthetic outcomes of modern facelift techniques. METHODS: A systematic search of databases, including MEDLINE, Embase, Cochrane Library, CINAHL, and LILACS, was conducted up to May 2

A Novel Approach to Medial Orbital Wall Reconstruction After Orbital Exenteration Using the Nasal Superficial Musculoaponeurotic System Flap: A Case Report.

We describe a novel application of the nasal Superficial Musculoaponeurotic System flap for medial orbital wall reconstruction following orbital exenteration. A 64-year-old woman underwent type IIa exenteration for moderately differentiated squamous cell carcinoma of the right medial canthus, requiring resection of the lamina papyracea. A pedicled nasal Superficial Musculoaponeurotic System flap incorporating perichondrium and based on the dorsal nasal artery was harvested through a midline nasal incision to close a 1.5-cm communication between the orbit and nasal cavity. To provide additional

Selective creation of thermal injury zones in the superficial musculoaponeurotic system using intense ultrasound therapy: a new target for noninvasive facial rejuvenation.

OBJECTIVES: To transcutaneously deliver intense ultrasound (IUS) energy to target the facial superficial musculoaponeurotic system (SMAS), to produce discrete thermal injury zones (TIZs) in the SMAS, and to demonstrate the relative sparing of adjacent nontargeted layers superficial and deep to the SMAS layer. METHODS: In 6 unfixed human cadaveric specimens, the SMAS layer was visualized and targeted using the ultrasound imaging component of the IUS device. Using 2 IUS handpieces, 202 exposure lines were delivered bilaterally in multiple facial regions by varying combinations of power and expo

CORE High-SMAS Extended Deep Plane Facelift Technique: How it Evolved.

Extended deep plane techniques have become the benchmark of facelift surgery. Yet these techniques assume release of the medial most of zygomatic retaining ligaments, which makes them rather complex and highly dependent on training and experience. In this article, we will review and describe the adapted extended high-superficial musculoaponeurotic system (SMAS) approach of the author and discuss the obstacles that can be encountered mastering this technique.

Complete Platysma Muscle Suspension in Deep-Plane Face-Lift Surgery.

Deep-plane face-lift surgery is not new, having originally been described over 30 years ago; however, the technique has seen a recent surge in popularity. Although proponents emphasize its ability to deliver very natural results, critics often cite the possibility of additional risk of the procedure because of its technically challenging dissection in proximity to branches of the facial nerve and other critical structures. These risks are perhaps greatest when operating in what have historically been described as the "danger zones," particularly when releasing the zygomatic retaining ligaments
