Evaluation of Incise Drape Lift Using 2% Chlorhexidine Gluconate/70% Isopropyl Alcohol Preoperative Skin Preparations in a Human Volunteer Knee Model.

BACKGROUND: Before surgery, skin is prepped with antiseptics to reduce risk of surgical site infections. An incise drape can be used as an additional modality to immobilize any remaining bacteria. Good adhesion of this drape is critical for infection prevention. METHODS: This is a randomized controlled study using a human volunteer knee model (n = 30) to evaluate the adhesion performance of an incise drape comparing 2 skin preparations. A new investigational 2% chlorhexidine gluconate/70% isopropyl alcohol skin prep (prep A) was compared with an existing skin prep containing the same active a

Operative material left in the abdominal cavity during surgical procedures.

The authors present seven cases of surgical drape left in body cavities during surgical operations. The most common symptoms reported by these patients are analysed and the consequences of leaving a foreign body in the abdominal cavity are summarised. In the majority of cases, the time elapsed from the operation to the detection of the foreign body was an average of 17 months. In one case, the foreign body remained in the abdomen for 7 years. The most common symptom reported by patients was abdominal pain. Some patients also developed wound healing disorders with leakage of pus from the wound,

Comparison of oxygen supplementation by nasal cannula with suction versus air insufflation without suction under drapes during monitored anesthesia care in adult cataract surgery-A randomized non-inferiority trial.

BACKGROUND: Patients with multiple co-morbidities undergoing cataract surgery are at risk of hypoxia and hypercarbia secondary to the rebreathing of the accumulated carbon dioxide under the surgical drapes. They are also at risk of fire accidents due to the hyperoxic condition secondary to oxygen supplementation. OBJECTIVES: The main aim of the study was to determine the lowest level of hemoglobin oxygen saturation while providing medical air in comparison with oxygen. Our hypothesis is that providing medical air will be non-inferior to oxygen administration in preventing hypoxia and rebreath

Skin preparation for preventing infection following caesarean section.

BACKGROUND: The risk of maternal mortality and morbidity is higher after caesarean section than for vaginal birth. With increasing rates of caesarean section, it is important to minimise risks to the mother as much as possible. This review focused on different skin preparations to prevent infection. This is an update of a review last published in 2018. OBJECTIVES: To compare the effects of different antiseptic agents, different methods of application, or different forms of antiseptic used for preoperative skin preparation for preventing postcaesarean infection. SEARCH METHODS: For this updat

Sources of contamination in the operating room: A fluorescent particle powder study.

This study utilized fluorescent particle powder to investigate 2 potential sources of sterile field contamination in the operating room (OR): forced-air warming blankets and OR light manipulation. In part 1, sterile draping for knee replacement surgery was performed on a mannequin in a sterile OR, comparing field contamination with the forced-air warming on versus off during draping. In part 2, OR lights coated with fluorescent powder were manipulated over a sterile field. Proper operation of these devices may reduce the particle burden on the surgical field.

Perspectives on disposable and reusable surgical materials in laparoscopic surgery: a global survey amongst surgeons.

BACKGROUND: Surgical practices generate substantial waste due to disposable materials. Reusable alternatives have the potential to substantially reduce emissions, yet their implementation has been slow. This study investigates reasons for this by examining surgeons' perspectives on using disposable and reusable materials during laparoscopic cholecystectomy (LC). MATERIALS AND METHODS: A survey, containing both quantitative and qualitative questions, targeted surgeons' perceptions of sustainable healthcare in general and reusable versus disposable instruments (trocars, clip applicators, drapes

Wound protectors in reducing surgical site infections in lower gastrointestinal surgery: an updated meta-analysis.

BACKGROUND: Surgical site infection (SSI) is a common complication in gastrointestinal surgery. Wound protection devices are being increasingly used in the attempt to reduce infection rates. We performed a meta-analysis to determine if wound protectors reduce the incidence of SSIs in lower gastrointestinal surgery. METHODS: MEDLINE and EMBASE databases were searched between 1946 and 2016. Randomized controlled trials comparing wound protector versus no wound protector in lower gastrointestinal surgery were included. Our primary outcome was surgical site infection. Subgroup analysis was conduc

Handmade Draping System for Tabletop Microscopes in Wet Lab Training.

BACKGROUND: Wet lab training using a tabletop microscope and chicken wings is an effective way to master superfine surgical techniques, providing a wet environment with real blood vessels that closely replicates surgical conditions. However, concerns remain regarding soiling of expensive microscopes with proteins and blood from raw meat, which can be difficult to remove once dried and may compromise the optical system. Additionally, raw poultry introduces the risk of bacterial contamination such as Campylobacter spp., which is commonly found in retail poultry. Since the practice is often perfo

Use of Alexis retractor in thyroid and parathyroid surgery.

In thyroid and parathyroid surgical procedures, good visibility of the thyroid gland is essential. This is traditionally achieved by means of traction sutures of the dermo-plastysmal flaps to the drapes of the operative field, producing tension and even damage to the skin. The Alexis® retractor ring improves exposure and facilitates stable access to the thyroid, protecting the skin from injury during the intervention.

Temporalis fascia in the management of gold eyelid weight extrusion.

A patient with a traumatic facial nerve palsy and resulting paralytic lagophthalmos underwent surgical implantation of a gold weight load, which extruded 4 weeks after surgery. Cicatricial contraction of the anterior lamella developed, and a full-thickness skin graft was used to correct the cicatrix. A second gold weight loading procedure was then undertaken with a temporalis fascia drape added to reduce the risk of extrusion. One year after surgery, there is no sign of migration or extrusion. Gold weight loading has emerged as the standard in management of paralytic lagophthalmos, with extrus

Pharmaceutical Properties of a Tinted Formulation of a Biguanide Antiseptic Agent, Olanexidine Gluconate.

Olanexidine gluconate-containing preoperative antiseptic (OLG-C) is colorless, which makes it difficult to determine its area of application. To overcome this drawback, we realized a stable orange-tinted antiseptic (OLG-T) by adding new additives to OLG-C and investigated its pharmaceutical properties compared with OLG-C and povidone iodine (PVP-I). We evaluated the influence of the additives on the antimicrobial activity and adhesiveness of medical adhesives to OLG-T-applied skin by in vitro time-kill/ex vivo micropig skin assays and a peel test using excised micropig skin, respectively. In t

Modified Technique for Labral Reconstruction of Hip Joint Using Autologous Iliotibial Band (ITB)-Make Labral Reconstruction Stress-Free.

Current treatments for labral tear include surgical debridement, arthroscopic repair, and labral reconstruction. Although labral debridement and labral suture repair are suitable for most patients, labral reconstruction is the first treatment option when there is extensive labral degeneration or defect. Often, however, the labral degeneration or defect is only detected intraoperatively; therefore, the surgeon should always have a backup plan. The current labral reconstruction technique has shortcomings such as long operation time, difficult autograft harvesting, cumbersome graft preparation, a

Sterile stockinette in orthopaedic surgery: a possible pathway for infection.

BACKGROUND: The prevention of infection in joint replacement surgery is important. Preparing and draping the patient is fundamental to maintaining sterility of the operative field. One method of draping for total knee replacement (TKR) surgery involves cleaning the operative leg with antiseptic wash then covering the un-prepped foot with a sterile stockinette to sequester the foot from the sterile field. There are two main methods for applying this stockinette. This randomized, blinded control trial simulates stockinette application to assess if it prevents proximal microbial migration into th

COVID-19 and otologic surgery.

In this article, we aim to summarize the impacts of COVID-19 on the practice of otologic surgery. Cadaveric studies have indicated COVID-19 viral particles are present in the middle ear mucosa of infected hosts. Otologic procedures can generate significant amounts of droplets due to reliance on high-speed drills. Multiple guidelines have been developed to improve patient and provider safety peri-operatively. Particle dispersion can be mitigated during microscopic mastoidectomy by utilizing barrier drape techniques. The barrier drape may similarly be applied to the surgical exoscope. Endoscopic
