Using a bone allograft to fixate proximal interphalangeal joint arthrodesis.

UNLABELLED: Digital contractures are a very common deformity of the foot and ankle that require surgical correction. It has been shown that arthrodesis provides a better long-term result than arthroplasty of the interphalangeal joints. Arthroplasties usually require K-wire fixation that presents potential complications, such as pin tract infection. This study presents a new cadaveric bone matrix allograft to be used as rigid internal fixation for proximal interphalangeal joint arthrodesis. The purpose of using the allograft as a fixation device is to achieve solid bone fusion and avoid the pot

Comparison of double screw fixation versus tension-band fixation after olecranon osteotomy for complex distal humerus fractures.

INTRODUCTION: Supra- and intercondylar elbow fractures are surgically challenging given the intra-articular comminution. Various surgical approaches have been described. Among these-olecranon osteotomy-provides optimal joint exposure but is associated with several complications. Tension band wiring (TBW) is the most commonly used fixation method. We hypothesized that double screw fixation would lead to fewer surgical revisions than TBW (all causes included) after olecranon osteotomy. METHODS: We carried out a single-center retrospective study of 39 patients, treated between January 2007 and A

Radiographic parameters of nonunion in Vancouver type C periprosthetic femoral fractures treated with plate fixation.

BACKGROUND: Plate fixation for Vancouver type C periprosthetic femoral fractures presents a surgical challenge, and nonunion is a serious complication. The relationship between specific fixation constructs and fracture healing remains poorly understood. This study aimed to describe the radiographic characteristics of patients with nonunion following plate fixation for these fractures, with a focus on proximal fixation strategies. METHODS: This multicenter retrospective study included 18 patients who underwent plate osteosynthesis for Vancouver type C fractures between 2014 and 2022. Patients w

Device-specific resistance to in vivo displacement of stent-grafts implanted with maximum iliac fixation.

PURPOSE: To compare the in vivo device-specific downward displacement force of various externally supported endografts implanted with maximum iliac fixation. METHODS: Twenty female sheep had aneurysms created with a graft patch in the infrarenal aorta. In 12 animals, a fully supported modular bifurcated stent-graft [AneuRx (n=4), Talent (n=4), or Zenith (n=4)] was deployed; in the other 8, a bifurcated aortic graft was surgically anastomosed to the infrarenal aorta. All grafts were displaced in vivo by applying downward traction to a guidewire brought out both femoral arteries. The peak force

Surgically-induced deformation in biodegradable orthopaedic implant devices.

The biocompatibility and mechanical performance of biodegradable metals (e.g. magnesium, iron, and zinc-based alloys) in orthopaedic-targeted applications are contingent on limiting the rate of corrosion in vivo throughout the bone healing. Concurrently, the surgical procedure for the implantation of internal bone fixation devices may impart plastic deformation to the device, potentially altering the corrosion rate of the device. However, the potential effect of the surgical implantation procedure on the mechanochemical performance of metallic degradable orthopaedic devices in vivo remains lar

Proximal Tibiofibular Joint Stabilization Using an Adjustable-Length Suspensory Fixation Device.

BACKGROUND: Proximal tibiofibular joint (PTFJ) injuries are rare injuries, and the optimal treatment is poorly understood. Surgical treatment options for PTFJ instability include reconstruction with allograft, stabilization with an adjustable-ength suspensory fixation device, open reduction and internal fixation with a screw, arthrodesis, and proximal fibular resection. INDICATIONS: In acute first-time PTFJ dislocations, nonoperative treatment may be trialed after closed reduction. Operative treatment is indicated for first-time dislocation with concomitant operative injury, an irreducible di

[The status of the iridocorneal angle after the implantation of anterior chamber intraocular lenses with flexible supporting elements].

The study was undertaken to examine the status of the iridocorneal angle (ICA) 1 to 4 years after implantation of anterior chamber intraocular lens with flexible supporting devices designed by M. L. Dvali. Correct implantation and gonioscopic monitoring wherein were found to provide reliable lens fixation, as a rule, in the area of the scleral spur and, occasionally, to the anterior part of the ciliary body, if the latter was broad. Additional fixation of the supporting devices was attained by delicate commissures at their small extent at the sites of iris contact, which were detected in the m

Novel intramedullary device for lengthening transfemoral residual limbs.

BACKGROUND: Lower limb loss is a highly disabling medical condition that can severely impact a person's quality of life. Recovery becomes especially challenging if an amputee has a short residual limb, which can complicate proper prosthetic fitting, causing discomfort, difficulties in suspension, and reduced mobility. Current limb lengthening techniques such as the Ilizarov apparatus and external fixators are cumbersome, uncomfortable, and have high complication rates. In this study, we investigated the effectiveness of a novel limb-lengthening device that uses intramedullary bone lengthening

Complications of lesser metatarsal surgery.

Complications of lesser metatarsal surgery are the result of inappropriate procedure choice, poor surgical technique, inadequate fixation, or a lack of patient compliance. Postoperative complications include transfer lesions, malunion, delayed union, nonunion, displacement of fixation devices, and resultant digital contracture. Despite good results with treatment of tailor's bunion deformities, results of osteotomies of the central three metatarsals are inconsistent. Conservative care is preferred for treatment of plantar hyperkeratoses.

Patellar Tendon Reconstruction Using Hamstring Tendon and Adjustable Suspensory Cortical Fixation.

Chronic patellar tendon rupture is a rare injury; extensor mechanism impairment leads to great disability in daily life. The delayed diagnosis and reconstruction of chronic patellar tendon ruptures are technically challenging. Numerous surgical procedures have been described with bone-tendon-bone graft, hamstring graft, artificial ligament, and allograft. We describe safe, reproducible anatomic reconstruction of the patellar tendon using hamstring tendon and adjustable suspensory fixation. After harvesting of the hamstrings, the graft is prepared with 2 adjustable suspensory fixation devices.

[Mid-third femoral shaft fracture in a patient with hypophosphatemic rickets treated with a locking centromedullary nail].

Residual deformities of hypophosphatemic rickets can be approached with a corrective osteotomy and fixation with a centromedullary nail, external fixators, Ilizarov devices, Kirschner's nails, cast devices, and plates, including epiphysiodesis. These methods may be used in case of fracture. The purpose of this work is to discuss the management of a 16-year-old patient with X-linked hypophosphatemic rickets at the end-stage of growth, who presented with a shaft fracture of the right femur, treated with alignment osteotomy and fixation, and alignment osteotomy of the contralateral femur in a sin

Novel self-fixation chest drain device tested in a swine model of pneumo-hemothorax.

INTRODUCTION: Thoracic injuries account for 20-25% of trauma-related deaths. In cases of pneumothorax the insertion of a chest tube is mandatory but associated with high complication rates particularly when inserted under difficult conditions. The C-Lant is a novel chest-tube insertion device that provides integrated double fixation capabilities and can be used by responders with minimal experience. The aim of the study was to test the device in a large animal model. MATERIAL AND METHODS: Pneumothorax, tension pneumothorax, and hemothorax were induced in four white domestic female pigs. The C

Development and preclinical evaluation of a cable-clamp fixation device for a disrupted pubic symphysis.

BACKGROUND: Traumatic separation of the pubic symphysis can destabilize the pelvis and require surgical fixation to reduce symphyseal gapping. The traditional approach involves open reduction and the implantation of a steel symphyseal plate (SP) on the pubic bone to hold the reposition. Despite its widespread use, SP-fixation is often associated with implant failure caused by screw loosening or breakage. METHODS: To address the need for a more reliable surgical intervention, we developed and tested two titanium cable-clamp implants. The cable served as tensioning device while the clamp secure

Wrist fractures.

The benefits of a well-reduced and well-healed wrist fracture are predictable. This knowledge has prompted the development of myriad treatment methods and fixation devices. The choice of open versus closed reduction and the devices used to maintain that reduction could most often be determined with a well-defined and reproducible closed reduction maneuver. With this maneuver, the integrity of the critical volar ulnar corner of the radius is determined as is articular stepoff, metaphyseal comminution, and distal radionular joint (DRUJ) stability. Reconstruction of the subluxed or dislocated DRU
