We present a modification to the widely used lead hand retraction system that allows for improved surgical access to a digit with a severe flexor contracture. A simple construct using surgical adhesive tape and a silk suture can be made and applied to the digit and lead hand. Our technique is simple and inexpensive to use, and we recommend its use to improve surgical access without causing unnecessary trauma to the digit.

OBJECTIVE: The aim of this study was to review the surgical transobturator vaginal tape inside-out (Gynecare TVT-O, Ethicon Inc., Somerville, NJ) technique as described by de Leval and to present the relevant anatomical conditions of the lower pelvis on the basis of corpse dissections after TVT-O placement.
STUDY DESIGN: In order to visualize the anatomical structures through which the tape runs, anatomical dissections of five corpses after TVT-O placement were performed. Furthermore, the dissections made possible to give a detailed description of the neighbouring neurovascular structures.

The goals of this study were to assess the efficacy of inside-out transobturator vaginal tape (TVT-O) as a treatment of urodynamic stress incontinence (USI) and to explore the possible factors determining surgical success and failure. Each woman had a 20-min pad test and urodynamic study including uroflowmetry, cystometry, and stress urethral pressure profile before and after treatment. Forty-six of the 54 women (85%) were cured of the disease, and two (4%) showed clinical improvement; TVT-O had failed to treat USI in six patients (11%) during the 9-month follow-up period. INTRODUCTION: Urinary retention is a frequent complication of repair of female urinary incontinence treated by tension-free vaginal tape (TVT). The authors report a modification of the original technique designed to decrease the risk of postoperative dysuria and retention.
MATERIAL AND METHODS: One hundred and twenty four patients with pure stress urinary incontinence were treated by TVT. In 74 patients (group 1), the TVT was placed according to the original Swedish technique. In 50 patients (group 2), no tension was applied to the suprapubic extremities of the TVT during the operation, but t

Reliable lateral decubitus positioning is critical for safe and effective lateral lumbar interbody fusion (LLIF); however, traditional patient stabilization with adhesive taping can be time-consuming, inconsistent, and associated with skin shear or breakdown, particularly in high body mass index (BMI) or elderly patients. In this technical report, we describe the use of a mechanical patient positioning system as a tape-free method of stabilization designed to provide counter-pressure at the iliac crest, thoracic cage, and/or greater trochanter while allowing for fine adjustments in alignment.

The tension-free vaginal tape (TVT) technique is now commonly used in the management of female genuine stress incontinence. Short operation time, small amount of local anesthetic, fast postoperative mobilization, brief hospital stay, little residual urinary volume and little need for sick leave make TVT a simple and well-accepted minimal invasive surgery for the treatment of stress incontinence. We became suspicious of the procedure due to the very expensive cost of the instrumentation. Moreover we think that this "blind" procedure may be less dangerous by using a more familiar instrument than
