Optimizing Surgical Wound Care in Obstetrics and Gynecology.

IMPORTANCE: Obstetrics and gynecology (OB/GYN) accounts for at least half of all open abdominal surgeries performed. Rates of surgical wound complications after open procedures in OB/GYN range from 5% to 35%. Therefore, optimizing management of surgical wound complications has the potential to significantly reduce cost and morbidity. However, guidelines addressing best practices for wound care in OB/GYN are limited. OBJECTIVE: The objectives of this review are to describe the fundamentals of wound healing and to evaluate available evidence addressing surgical wound care. Based on these data,

Closed suction surgical wound drainage after orthopaedic surgery.

BACKGROUND: Closed suction drainage systems are frequently used to drain fluids, particularly blood, from surgical wounds. The aim of these systems is to reduce the occurrence of wound haematomas and infection. OBJECTIVES: To evaluate the effectiveness of closed suction drainage systems for orthopaedic surgery. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group specialised register (May 2001), MEDLINE (1996-May 2001) and references from articles. SELECTION CRITERIA: All randomised or quasi-randomised trials comparing the use of closed suction drainage systems with no d

Surveillance of surgical wound infections in cancer patients.

Despite the increasing number of therapeutic options available to cancer patients, surgery represents a mainstay of treatment. Surgical wound infections (SWIs) account for frequent patient morbidity, and the true incidence of these infections is probably underestimated. Little data exist that identify SWI rates among patients with cancer. Due to their disease, intensive treatment regimens, or both, profound immunosuppression is an all too frequent occurrence among cancer patients. Thus these patients may have a higher intrinsic risk for acquiring an SWI. Surveillance of SWIs coupled with repor

Toxic shock syndrome associated with surgical wound infections.

In 13 cases of toxic shock syndrome (TSS) associated with postoperative wound infections, clinical findings were similar to those observed in cases of TSS in menstruating women. While local signs of a surgical wounds infection were minimal, Staphylococcus aureus was recovered from cultures of wounds in 12 of 12 patients; multiple blood cultures were negative for 11 of 11 patients. Toxic shock syndrome due to surgical wound infections accompanied a wide variety of surgical procedures in both men and women and was not necessarily associated with menstruation in women. The median interval between

The Role of Perfumes in Surgical Wound Infections and Wound Healing: A Case-Control Study.

A common complication of post-surgical procedures is surgical site infections (SSIs), and wound healing can be gravely affected by these SSIs. Perfumes are known for their use in personal hygiene; however, their role in surgical wound healing and SSIs has not been thoroughly studied. The present study explores the post-operative usage of perfumes in the context of SSIs and wound healing. This was a case-control study conducted in Hail City's Tertiary Care Hospital in Saudi Arabia for 9 months (April-December). The participants were adults undergoing clean elective plastic surgeries. Sixty-thre

Negative-pressure wound therapy over surgically closed wounds in open fractures.

PURPOSE: To evaluate the outcome of open fracture surgery with negative-pressure wound therapy (NPWT) applied directly over surgical wounds. METHODS: Medical records of 10 men and 4 women aged 6 to 70 (mean, 43.7) years who underwent internal fixation, external fixation, or splint application for open fractures of the lower leg (n=7), foot and ankle (n=5), or knee (n=2), and NPWT were reviewed. The NPWT was applied directly over the surgical wound without a non-adherent contact layer. The foam was changed every 3 days. The standard negative pressure was 125 mm Hg. The NPWT was stopped when th

A Rat Surgical Skin Wound Model: An Approach for Wound Healing Studies and Biomaterial Evaluation.

Skin wound healing is a complex biological process involving coordinated phases of inflammation, proliferation, and remodeling. Given the need to better understand the underlying mechanisms of wound healing, excisional animal wound models are widely used to deepen the knowledge of this process and facilitate the development of novel therapeutic approaches. In such models, the induction of skin wounds using punch biopsies is frequently employed as it yields standardized lesions. However, this technique restricts the lesion to a circular shape, which may not be suitable for some experimental obj

Negative Pressure Wound Therapy in Facilitating Wound Healing After Surgical Decompression for Metastatic Spine Disease.

BACKGROUND: The risk of wound-related complications, including surgical site infections (SSIs), in patients undergoing surgery for metastatic spine disease (MSD) is high. Consequently, patients requiring wound revision surgery face delay in resuming oncological care and incur additional hospitalization. Recent reports suggest that negative pressure wound therapy (NPWT) applied on a closed wound at the time of surgery significantly reduces postoperative wound complications in degenerative spine disease and trauma setting. Here, we report a single institution experience with incisional NPWT in p

Aggressive Surgical Debridement With Primary Closure for Surgical Wound Dehiscence: A Retrospective Cohort Study Comparing Healing Outcomes to Negative Pressure Wound Therapy.

Surgical wound dehiscence (SWD) is a serious postoperative complication associated with significant morbidity, mortality and healthcare costs. Traditional management, including negative pressure wound therapy (NPWT), relies on secondary intention healing, which can be prolonged. This study evaluates a strategy of aggressive surgical debridement followed by primary wound closure (ASDC) to convert surgical wounds to primary intention healing. In this retrospective cohort study, we compared outcomes between ASDC and NPWT in 106 SWD patients (2013-2023). The ASDC group (n = 62) underwent radical d

Rigenera protocol in the treatment of surgical wound dehiscence.

The effective management of post-operative wounds is important to prevent potential complications such as surgical-site infections and wound dehiscence. The purpose of this study was to treat wound dehiscence in elderly patients who were subjected to orthopaedic surgical interventions. The dehisced wounds were treated with autologous micro-grafts obtained using a promising CE-certified medical device called Rigeneracons. This instrument is a biological disruptor of human tissues able to specifically select progenitor cells that, as already reported in previous studies, maintain high cell viabi

[Using vacuum in the treatment of surgical wounds complications].

BACKGROUND/AIM: Using vacuum in medicine has been known from long ago, however, it has not been used for the treatment of wounds. The first experiments in this field were performed by Wagner Fleischmann, University of Ulm, Ulm, West Germany, in 1993. The aim of this study was to present our clinical experience with the treatment of surgical wounds complications in vascular patients by the use of controled vacuum. METHOD: In a period October 2006 - December 2009 a total of 18 patients with infection and surgical wound dehiscence were treated by the use of vacuum. Vacuum was applied to wounds b

[Surgical strategy for wounds of the cervical part of the esophagus].

An analysis of results of treatment of 425 patients aged from 15 to 85 years with penetrating wounds of the neck was made. In 40 (8.1%) patients there were wounds of the cervical part of the esophagus, 29 of them had associated injuries. The proposed active surgical strategy consisted in a necessary thorough revision of the penetrating wounds of the neck using general anesthesia and then directly in the operation room using subsidiary methods of examination such as urgent x-ray analysis with a water-soluble contrast, esophagoscopy, larengotracheoscopy. This approach allowed detection of injuri

[Gunshot wounds--ballistics, physiopathology, surgical treatment].

The form and size of gunshot wounds depend on the projectile type (bird shot, deforming projectiles, non-deforming projectiles), its velocity at impact (high or low velocity projectiles) and the type of tissue affected by the projectile (soft tissues, bones). Gunshot wounds are always contaminated. While low velocity projectiles usually cause minimal wounds, high velocity projectiles can cause large, devastating wounds. All projectiles fired from firearms have a high velocity on impact and may cause death by traumatic shock instantaneously. Ballistics, pathophysiology and clinical findings of

Management of post-surgical wounds in general practice.

BACKGROUND: The availability of complex wound dressings following progressive innovations, increasing demand for hospital beds and the early discharge of post-operative patients have shifted the sharing of care of such patients from hospital specialists to general practitioners (GPs). Although several published guidelines on the management of chronic wounds exist, there is a lack of similar material addressing the assessment and management of post-surgical wounds. OBJECTIVE: The aim of this article is to provide a practical guideline for identifying early complications of post-surgical wounds
