[Are There any Differences in Quality or Complication Rate in Total Knee Arthroplasty after Establishing a Centre for Endoprosthesis?].

BACKGROUND: The aim of the study is to show whether there are any changes in quality or incidence of complications in total knee arthroplasty (TKA) after establishing a centre for endoprothesis (EPZ). MATERIAL AND METHODS: We conducted a retrospective study comparing 100 TKAs one year before establishing an EPZ (Group I) with 100 TKAs one year after establishing an EPZ (group II). Data were collected by analysing our electronic documentation system, and the report of the rehabilitation hospital. The following parameters were documented which are necessary to establish an EPZ: existence of X-r

Laparoscopic incisional hernia repair in an ambulatory surgery-extended recovery centre: a review of 259 consecutive cases.

PURPOSE: The high prevalence of incisional hernias and an average stay of 3-10 days for open procedures have made this pathology both a health problem and an economic issue. A protocol was developed for performing this procedure in an Ambulatory Surgery Center (ASC) with extended recovery. METHODS: From January 2000 to December 2011, data about all laparoscopic incisional hernia repairs were gathered prospectively. The patients' clinical features, hernia type, intraoperative and postoperative complications and reasons for hospital admission are studied. RESULTS: A total of 259 patients have

Strategic response by providers to specialty hospitals, ambulatory surgery centers, and retail clinics.

Radical innovation and disruptive technologies are frequently heralded as a solution to delivering higher quality, lower cost health care. According to the literature on disruption, local hospitals and physicians (incumbent providers) may be unable to competitively respond to such "creative destruction" and alter their business models for a host of reasons, thus threatening their future survival. However, strategic management theory and research suggest that, under certain conditions, incumbent providers may be able to weather the discontinuities posed by the disrupters. This article analyzes

Outpatient anterior cervical discectomy and fusion on the rise: changing economics and utilization patterns in U.S. hospital-owned ambulatory surgical centers.

BACKGROUND CONTEXT: The migration of spine surgery to ambulatory surgical centers (ASCs) continues to expand, but national trends in the utilization and cost of anterior cervical discectomy and fusion (ACDF) in these settings remain understudied. This study evaluates trends in volume, payer mix, and inflation-adjusted charges for single-level ACDF procedures performed in hospital-owned ASCs across the United States. PURPOSE: To characterize national trends in the utilization and inflation-adjusted cost of single-level ACDF procedures performed in hospital-owned ASCs from 2016 to 2022. DESIGN

[Some aspects of work at the city laser surgery center of Ashgabad].

During 7-year work 2,500 operations with a laser were performed for 52 diseases at the center. The authors studied the medico-economical data on the use of the laser techniques in the treatment of persisting wounds and trophic ulcers, cutaneous leishmaniasis, chronic anal fissure, purulent diseases of the soft tissues, epithelial coccygeal passage, ingrowing nail, and localized hyperkeratosis. A comparison group consisted of patients who received traditional treatment. With the use of the laser methods the clinical results of treatment were appreciably improved--the number of poor results redu

Controversies in cardiovascular medicine. Benefits of surgery in obstructive hypertrophic cardiomyopathy: bring septal myectomy back for European patients.

Hypertrophic cardiomyopathy (HCM), a heterogeneous genetic heart disease with global distribution, is an important cause of heart failure disability at any age. For 50 years, surgical septal myectomy has been the preferred and primary treatment strategy for most HCM patients with progressive, drug refractory functional limitation due to left ventricular (LV) outflow tract obstruction. With very low surgical mortality at experienced centres, septal myectomy reliably abolishes impedance to LV outflow and heart failure-related symptoms, restores quality of life, and importantly is associated with

Impact of anastomotic leakage on long-term survival after total gastrectomy for carcinoma of the stomach.

BACKGROUND: Recent studies suggest that anastomotic leak may adversely affect long-term survival in patients undergoing surgery for gastrointestinal malignancies. Data relating to total gastrectomy for gastric cancer are scarce. METHODS: An electronic database of all patients with resectable gastric cancer treated between January 1999 and December 2004 at seven university surgical centres cooperating in the Polish Gastric Cancer Study Group was reviewed. RESULTS: Anastomotic leakage was diagnosed in 41 (5.9 per cent) of 690 patients who underwent total gastrectomy. The prevalence of surgical

Outpatient Mastectomy with Reconstruction at Ambulatory Surgery Centers.

ABSTARCT: BACKGROUND: While ambulatory surgery centers (ASCs) are increasingly utilized for outpatient surgery for cost savings and resource efficiency, data comparing outcomes and costs of same-day mastectomy (SDM) with reconstruction at ASCs versus hospital outpatient departments (HOPDs) remain limited. We aimed to identify predictors of overnight admission following SDM with immediate implant/tissue expander reconstruction, and evaluate its safety, feasibility, and cost-efficacy at ASCs versus HOPDs. PATIENTS AND METHODS: We performed a retrospective review of women undergoing mastectomy w

Surgical Care Through a Community Free Clinic-Ambulatory Surgical Center Partnership.

BACKGROUND: Disparities secondary to underinsurance present throughout the surgical care continuum. Community free clinics are uniquely capable to provide health care services to the medically underserved, but surgery often falls outside their scope of care. METHODS: Retrospective chart review was conducted on consecutive community free clinic patients receiving free surgical services via referral to a partnering ambulatory surgery center between March 2016 and September 2021. Those with documented contact information were recruited 1-3 years post-procedure for long-term quality-of-life (LTQO

Our Surgical Experience: Open Versus Endoscopic Carpal Tunnel Surgery.

Carpal tunnel release is one of the most common hand operations in the United States and every year approximately 500,000 patients undergo surgical release. In this article, we examine the argument for endoscopic carpal tunnel release versus open carpal tunnel release, as well as some of the literature on anatomical variants in the median nerve at the wrist. We further describe the experience of several surgeons in a large academic practice. The goals of this article are to describe key anatomic findings and to present several cases that have persuaded us to favor offering patients open carpal

Outpatient laparoscopic cholecystectomy: what predicts the need for admission?

Laparoscopic cholecystectomy (LC) is commonly performed as an outpatient (OP) procedure in selected patients, either in ambulatory surgery units associated with a hospital or in freestanding facilities. To identify factors that may preclude OPLC, a retrospective analysis of all patients who underwent LC by two surgeons from August 1996 through June 1998 was performed. A total of 126 patients were divided into three groups. Group I comprised 102 patients who underwent attempted elective OPLC. Group II comprised 20 patients who underwent LC on an emergent basis. Group III comprised 5 patients wh

A hybrid model of ureteral stent change - ambulatory cystoscopy suite under local anesthesia versus operating room under general or monitored anesthesia.

PURPOSE: Patients with long-term ureteral stents need regular change, traditionally under General Anesthesia (GA). We present a hybrid model where patients are risk-stratified for anticipated technical difficulty: (1) local anesthesia (LA) in the outpatient cystoscopy suite as day procedures, or (2) GA/ Monitored Anesthesia Care (MAC) in operating room (OR). METHODS: A retrospective review was conducted of ureteral stent changes between 2021 and 2025, using a risk-stratified model: lower-risk as outpatient LA in ambulatory cystoscopy suite, and higher-risk under GA/MAC in the OR. RESULTS: A

Outpatient Total Joint Arthroplasty at an Ambulatory Surgical Center: An Analysis of Failure to Launch.

BACKGROUND: There has been a tremendous increase in same-day discharge (SDD) following primary total joint arthroplasty (TJA). Although the concept of failure to launch (FTL) has been recently investigated in hospital settings, there is a paucity of data in the ambulatory surgical center (ASC) context. This study aimed to examine the incidence and underlying causes of FTL within an ASC at a major academic medical center. METHODS: A retrospective review from 2021 to 2024 was performed on all patients who underwent same-day surgery at our ASC after intentional selection and medical optimization
