Unicoronal synostosis. A surgical intervention.

Computer-assisted medical imaging technology provides useful tools for the in vivo study of congenital craniofacial deformities. Using these tools, a standardized operation for infants with nonsyndromal unicoronal synostosis has been shown to consistently normalize orbital dimensions and decrease exocranial base angulation. Normalization of endocranial base angulation is less consistent. The plasticity of the infant's cranial base after cranial vault surgery suggests a locus of the primary pathology at a site other than the cranial base.

Functional anatomy of cranial synostosis.

An understanding of how sutural growth processes relate to the totality of cranial growth is necessary to cure the cause and not just the symptoms of cranial synostosis. There is no direct genetic determination for the origin, growth, size, shape or maintenance of bones. Rather, phenotypic expression of skeletal tissues is regulated via genetic information encoded in the cells of functional matrices. The primary morphogenetic event in neurocranial growth is the volumetric expansion of the neural mass, which causes the surrounding neurocranial capsule to expand. Calvarial bones arise at widely

Additional squamosal suture synostosis and segmented intracranial volume in patients with non-syndromic sagittal synostosis.

PURPOSE: To evaluate the incidence of squamosal suture synostosis (SQS) in children with non-syndromic sagittal synostosis and to evaluate whether the additional SQS affects the intracranial volume (ICV). METHODS: Thirty-four consecutive patients (23 boys) who had been operated by cranial vault remodelling because of sagittal synostosis were compared retrospectively from 3D-CT imaging data sets obtained from volumetric CT. The mean age of the patients at preoperative CT imaging was 0.48 (range 0.13-1.3) years. Mann-Whitney U test was used in the statistical analyses. RESULTS: Sagittal synost

The Significance of Squamosal Suture Synostosis.

The squamosal suture is one of the lateral minor skull sutures, separating the parietal and squamous temporal bones. While the phenotypic appearances and sequelae of synostosis of the major cranial vault sutures are well documented, little is reported concerning synostosis of the squamosal suture (SQS). The aim of this study was to determine the frequency of squamosal suture synostosis, and to document the significance of this entity.A retrospective review of the diagnostic imaging for all new pediatric patients (aged ≤16 years) referred to the Oxford Craniofacial Unit between January 2008 and

Synostosis of the lambdoid suture: a spectrum.

PURPOSE: Lambdoid synostosis (LS) is a rare condition, which is either isolated; associated with sagittal synostosis, the "Mercedes-Benz" syndrome (MBS); or with synostosis of the coronal sutures (oxycephalic form). In addition, LS is part of the phenotype of a growing number of genetic diseases. The nosology, pathophysiology, and management are controversial. We decided to review our experience with LS. METHODS: We reviewed retrospectively pediatric cases of LS proved on CT-scanner, isolated or associated with other conditions, followed in our craniofacial center during the last 15 years, re

Classification of trigonocephaly in metopic synostosis.

BACKGROUND: The orbitofrontal deformity in metopic synostosis is recognized clinically but has not been quantitatively defined in a large patient population. The authors' purpose was to document the dysmorphology in metopic synostosis and define subtype gradations. METHODS: Demographic and computed tomographic information was recorded. Three-dimensional computed tomographic renderings were created digitally. Craniometric analysis was conducted for endocranial bifrontal angle, interzygomaticofrontal suture and interdacryon distance, and angle of orbital aperture to the midline. RESULTS: Thirt

[Distal symphalangia with humeroradial synostosis, carpal synostosis and brachyphalangia of the thumb. A dominant syndrome (author's transl)].

In a family with genetically dominant joint aplasia, 12 of the 27 involved members in four generations had bilateral humeroradial synostoses; the rest, ventral luxation of the radius. In addition, various degrees of malformations and aplasia of the carpal, tarsal and interphalangeal joints as well as shortening of the proximal phalanx of the thumb were observed. All other similar cases described in the available literature differ significantly from those described here.

Isolated Squamosal Synostosis: Defining the Phenotype.

This study aims to characterize the clinical manifestations of isolated squamosal synostosis (ISS) and review the relevant published literature. Computed tomography imaging from 2008 to 2022 was reviewed to identify patients with ISS. Patients were reviewed for age at presentation, signs of intracranial pressure, and management. A systematic review was conducted of all studies on ISS from 1990 to 2023. Eighteen patients from our institution were included. Three unilateral cases were diagnosed at 1.4 ± 0.6 years. None had signs of intracranial hypertension nor required surgery. Of the 15 bilate

Delayed-Onset Familial Sagittal Suture Synostosis.

Sagittal craniosynostosis is the most common of all craniosynostoses. Patients with sagittal craniosynostosis exhibit a typical cranial appearance, including scaphocephaly, and an elongated head, with a fused and ridged sagittal suture. Moreover, some recent reports described atypical sagittal craniosynostosis accompanied by autism, speech delay, and hyperactivity. This condition, known as delayed-onset craniosynostosis, is rarely reported, given that it is difficult to determine if the condition is congenital or has a delayed-onset.This report describes the clinical course and treatment of 2

Headache in Postoperative Isolated Sagittal Synostosis.

BACKGROUND: This study investigates the relationship between headache and the occurrence of signs associated with intracranial hypertension such as ophthalmic signs, restricted skull growth, and a vertex bulge in children who were operated on for sagittal synostosis. METHODS: A total of 94 patients (aged 6 to 18 years) with sagittal synostosis were asked to indicate their headache frequency. Based on their age at referral, the patients had undergone either frontobiparietal remodeling or an extended strip craniotomy. Data on funduscopy, optical coherence tomography, occipitofrontal head circum

Endoscope-assisted repair of metopic synostosis.

PURPOSE: Endoscopic treatment of nonsyndromic craniosynostosis has been used in recent decades. The aim of this study is to present the results of endoscope-assisted surgery of the patients with metopic synostosis. METHODS: Nineteen patients with metopic synostosis underwent endoscope-assisted surgery between 2005 and 2012. The frontal angle was measured on CT scans pre- and postoperatively. A midline strip craniectomy was made, making sure that the removal extended inferiorly to the nasofrontal suture. Postoperatively, all patients wear a custom molding helmet. The helmet was then worn for 2

Bilateral orbital dysmorphology in unicoronal synostosis.

BACKGROUND: Orbital dysmorphology is believed to cause ipsilateral ocular abnormalities in unicoronal synostosis. Recently, there has been increasing evidence of visual problems in the contralateral eye. The purpose of this study was to explore morphology of both the ipsilateral and contralateral unicoronal synostosis orbit. METHODS: Demographic data and computed tomographic information were recorded. Three-dimensional computed tomographic renderings were created digitally and analyzed (SurgiCase). Craniometric analysis was conducted for orbital volume, horizontal and vertical orbital cone an

[Mandibular asymmetry in plagiocephaly without synostosis].

SUBJECT: The number of plagiocephalies without synostosis (PWS) strongly increased in the last decade. Its impact on the mandible remains badly understood, the more so as there are various forms of PSS. The purpose of this study was to analyze mandible deformation according to the deformation of PWS. MATERIAL AND METHODS: The study was carried out retrospectively starting from three-dimensional scans of children presenting with a PWS. They were classified in 2 groups according to the deformation of the cranium. There were 51 frontal plagiocephalies (PF) and 19 occipital plagiocephalies (PO) f

Antley-Bixler syndrome with radioulnar synostosis.

This report describes a newborn boy with Antley-Bixler (AB) syndrome. AB syndrome is a rare disorder characterized mainly by craniosynostosis and multiple joint contractures including radiohumeral synostosis. Our patient differs from the usual presentation by the presence of radioulnar rather than a radiohumeral synostosis. In addition, the child had a FGFR1 1300T mutation, which has not previously been associated with AB syndrome. Thus, our patient presents unique features and represents another argument in favour of an AB-like spectrum rather than a single syndrome.

Recurrent lambdoid synostosis within two families.

We report on 2 families with multiple members who have proven or suspected lambdoid craniosynostosis. In one family the lambdoid suture was unilaterally involved in one sib, and bilaterally in the other. In the second family the propositus had unilateral lambdoid synostosis and his twin sisters each had bilateral lambdoid synostosis. Both families had another distant relative, a maternal grandmother in the first and paternal uncle in the second, who were also reported to have posterior plagiocephaly. We report these families as evidence for genetic transmission of a craniosynostotic trait whic
