Synovial fluid white blood cell counts are considered to be useful in diagnosing infectious arthritis, however, considerable overlap exists between infectious and noninfectious types of inflammatory arthritis. We undertook this review of synovial fluid studies at our institution to better define this degree of overlap and characterize the features of infectious arthritis in relationship to synovial fluid white cell counts. The records of 202 consecutive patients with synovial fluid white blood cell counts >2000/mm were reviewed. Infectious arthritis was diagnosed in 77% (10/13) of patients wit

BACKGROUND: The diagnosis of gout in the intercritical phase can be difficult.
OBJECTIVE: To determine whether synovial fluid analysis allows the diagnosis of intercritical gout.
DESIGN: Cross-sectional study.
SETTING: Outpatient rheumatology clinics.
PATIENTS: 101 patients with gout.
INTERVENTION: Arthrocentesis of 80 knees and 21 first metatarsophalangeal joints (each joint from a different patient) that had been inflamed but were currently asymptomatic.
MEASUREMENTS: Frequency with which arthrocentesis yielded synovial fluid; presence of monosodium urate crystals in the synovial fluid

High performance liquid chromatography with TSK 5000 PW or TSK 6000 PW size exclusion columns combined with a 125I labelled hyaluronic acid binding protein assay was used to study the effects of oxygen derived free radicals on synovial fluid hyaluronate. A continuous flux of free radicals was generated by the xanthine oxidase/hypoxanthine system. When the free radical flux was generated with xanthine oxidase/hypoxanthine in the presence of the iron chelator desferrioxamine and the hydroxyl radical scavenger mannitol a 30-50% decrease in hyaluronate peak was detected, but the molecular weight o

A unique case of an inflammatory monoarthritis associated with intracellular lipid inclusions in synovial fluid leukocytes is described. In contrast to previous reports, these lipid inclusions were uniformly intracellar, not associated with articular destruction, and not visible under ordinary light but readily seen under polarized light microscopy.

Synovial fluid (SF) is the complex biofluid that facilitates the exceptional lubrication of articular cartilage in joints. Its primary lubricating macromolecules, the linear polysaccharide hyaluronic acid (HA) and the mucin-like glycoprotein proteoglycan 4 (PRG4 or lubricin), interact synergistically to reduce boundary friction. However, the precise manner in which these molecules influence the rheological properties of SF remains unclear. This study aimed to elucidate this by employing confocal microscopy and multiscale rheometry to examine the microstructure and rheology of solutions contain

Synovial fluid analysis can provide a prompt and definite diagnosis of crystal-induced arthritis, the most common acute inflammatory arthritis and a cause of chronic arthritis that may mimic rheumatoid, psoriatic, or peripheral spondyloarthritis. In many patients the diagnosis of gout or calcium pyrophosphate arthritis cannot be made with certainty without synovial fluid analysis. Additional information from fluid analysis can assist the clinician in honing the differential diagnosis of non-crystalline arthritis.

Synovial fluid analysis is one of the most useful laboratory test in the diagnosis of joint diseases. It allows to determine the degree of synovial inflammation, the presence of pathogenic crystals and microorganisms, and to evaluate the effect of pharmacological treatments as well as the progression of the disease. Synovial fluid therefore represents a precious substrate able to give valuable information from both the clinical and the research points of view.In this educational review we present and discuss some unusual findings, at times associated with rare rheumatic conditions, observed wh

OBJECTIVE: The purpose of this study was to evaluate cytokine level characteristics in the hip joint fluid, including rapidly destructive coxopathy (RDC), OA, osteonecrosis (ON) of the femoral head and RA.
METHODS: Thirty-three hips with RDC, 57 with OA, 36 with ON and 10 with RA were included in the study. OA hips were divided into two groups: 20 hips with early OA without joint space narrowing and 37 hips with terminal OA. ON hips were divided into three groups: 13 hips with <3 mm collapse, 14 hips with >3 mm collapse and 9 hips with terminal ON. Joint fluid was collected during surgery. Cy

Joint aspiration/injection and synovial fluid (SF) analysis are both invaluable procedures for the diagnosis and treatment of joint disease. This chapter addresses (1) the indications, technical principles, expected benefits and risks of aspiration and injection of intra-articular corticosteroid and (2) practical aspects relating to SF analysis, especially in relation to crystal identification. Intra-articular injection of long-acting insoluble corticosteroids is a well-established procedure that produces rapid pain relief and resolution of inflammation in most injected joints. The knee is the

Presence of muramic acid, a bacterial cell wall component, was analysed by gas chromatography-mass spectrometry in synovial fluid (SF) of 40 patients with acute inflammatory arthritis. SF muramic acid was observed in 4/14 patients with acute, culture negative inflammatory arthritis of unclear origin. Each of these four patients had a history of a recent bacterial disease (pansinuitis, purulent leg ulcer, erysipelas, unexplained fever with suspicion of cholecystitis and urinary tract infection). In the bacterial arthritis, SF muramic acid was detected in 6/12 patients (in 2/6 culture negative c

The aim of the present work was to investigate the presence of tetrahydrocannabinole in the human synovial fluid. The samples were obtained post mortem at obduction. The tetrahydrocannabinole concentrations were determined by radioimmunoassay. The presence of tetrahydrocannabinole in the synovial fluid obtained from hashish smokers was demonstrated. In addition, the concentrations measured were compared with these found in blood and urine.

Detection of proteoglycans in biological fluids is a perspective method for the evaluation of the degree of catabolic processes in articular cartilage. The demand of accuracy and specificity of detection of substructures of the degradation products of the cartilaginous matrix, with the perspective of routine large scale examinations, restricts available possible methods practically only to the use of immunochemical methods. In the present investigation in the inhibitory ELISA test polyclonal antibodies with a double specificity in relation to basic structures of the cartilage--proteoglycans--w

The limulus assay for the detection of bacterial endotoxin has been applied to the study of synovial fluid. Three of 5 patients (60%) with culture-positive gonococcal arthritis had positive SF limulus assay results; as did 2 of 11 (18%) with presumptive evidence of gonococcal arthritis, 3 of 6 (50%) with nongonococcal infectious arthritis, and none of 47 patients with noninfectious arthritis. Endotoxin levels ranged from 0.25 to 128.0 ng/ml. As at present applied to synovial fluid the commercial limulus assay appears to be specific for the infectious process but apparently lacks sensitivity.

Synovial fluid (SF) of 16 adult patients with rheumatoid arthritis (RA) and 9 children with juvenile rheumatoid arthritis (JRA) was investigated for the presence of hidden 19S IgM rheumatoid factor (RF). SF was incubated with 500 IU hyaluronidase/g of SF, dialyzed against acetate buffers at decreasing pH, centrifuged and subjected to chromatography on a Sephadex G-200 column. The resulting IgM-containing fraction of SF from 5 seropositive patients (3 adult RA, 2 JRA patients) demonstrated RF activity by the complement-dependent hemolytic assay. Hidden 19S IgM RF was not found in the IgM-contai

In five patients with acute arthritis in whom gout was eventually documented, an initial synovial fluid analysis failed to reveal urate crystals. Four of the patients were seen in one hospital during a period of 30 months in which 103 cases of gout were documented on initial aspiration. While this is an uncommon event, the importance of being able to make a definitive diagnosis of gout is such that re-aspiration of the same or other joints may be justified under certain circumstances.

Aseptic loosening due to periprosthetic osteolysis has been accepted as one of the leading causes of revision procedures in patients with previous joint arthroplasty. Recently, several strategies for suppression of osteolysis were proposed, mostly based on biological treatment such as mitigation of chronic inflammatory reactions. However, these biological treatments do not stop the debris migration but only reduce the inflammatory reaction. To address this shortcoming, we propose the concept of ultrahigh molecular weighted polyethylene particles filtration storage by electrospun membranes. Fir
