Anterograde block in accessory pathways with retrograde conduction in reciprocating tachycardia.

3 patients with reciprocating tachycardia are described. In all 3, tachycardia was due to a reentrant mechanism. Anterograde conduction was via the AV node while retrograde conduction was over an accessory (Kent) pathway. In none of the 3 patients did this pathway sustain anterograde conduction despite pacing the atrium close to its location. With the advent of surgical modes of therapy, it is important to recognize such 'silent' bypasses. The observations which allow this diagnosis of unidirectional anterograde block in an accessory pathway and the exclusion of AV nodal reentry, the main diff

A new criterion to differentiate atrioventricular nodal reentrant tachycardia from atrioventricular reciprocating tachycardia: Combined AVR criterion.

AIM: A combined aVR criterion is described as the presence of a pseudo r' wave in aVR during tachycardia in patients without r' wave in aVR in sinus rhythm and/or a ≥50% increase in r' wave amplitude compared to sinus rhythm in patients with r' wave in the basal aVR lead. We aimed to investigate the use of combined aVR criterion in differential diagnosis of atrioventricular nodal reentrant tachycardia (AVNRT) and atrioventricular reciprocating tachycardia (AVRT). METHODS: In this prospective study, 480 patients with inducible narrow QRS supraventricular tachycardia (SVT) were included. Twelve

[Paroxysmal tachycardia in a patient with a permanent form of junctional reciprocating tachycardia. A case report].

Junctional reciprocating tachycardia is an atrioventricular reentrant tachycardia whose anterograde conduction occurs via the His Purkinje and the retrograde conduction via an accessory pathway with slow conduction. The most common form is incessant tachycardia but a paroxysmal form also exists. We report a 35 years old female with recurrent paroxysmal tachycardia, that underwent electrophysiological evaluation. A left posterolateral accessory pathway was documented. Reciprocating paroxysmal tachycardia was induced by electrical stimulation and a successful pathway ablation was performed.

Electrophysiologic comparison between incessant and paroxysmal tachycardia in patients with permanent form of junctional reciprocating tachycardia.

To clarify the differences between the incessant and paroxysmal types of the permanent form of junctional reciprocating tachycardia, we performed electrophysiologic studies in 11 patients with long RP' tachycardia using a slowly conducting accessory pathway as the retrograde conduction, and concluded that the short AH intervals during sinus rhythm and tachycardia are very important factors in the development of incessant tachycardia.

[Diagnosis and treatment of paroxysmal reciprocal atrioventricular tachycardia].

The differential diagnostic criteria of paroxysmal reciprocating atrioventricular tachycardias were evaluated on the basis of a study of two clinical cases. Noninvasive (transoesophageal) electrophysiological methods were used. Evaluation of the trigger mechanism of arrhythmia, its form in comparison with data of Holter monitoring and vegetative testing allows to institute adequate antiarrhythmic therapy.

[Paroxysmal junctional reciprocal tachycardia and fetoplacental anasarca].

Foeto-placental anasarca was diagnosed at 34 weeks gestation in a patient with acute hydramnios. Foetal tachycardia at 300 bpm was recorded. This obstetrical problem led to the birth of a premature baby with generalised oedema, for which the only apparent cause was the tachycardia. This was identified as a paroxysmal junctional reciprocating tachycardia, initiating on atrial extrasystolic echos, terminating on R waves, with lengthening of the PR interval at the onset of tachycardia, without acceleration of the sinus rate and P'R = RP'. Paroxysmal junctional reciprocating tachycardia in utero w

Intermittent ventricular pre-excitation in a dog with orthodromic reciprocating atrioventricular tachycardia.

A four-year-old female neutered Labrador retriever was referred for investigation of paroxysmal tachycardia. On physical examination, a heart rate of 220 beats/min with no heart murmurs was noticed. Cardiac troponin I was significantly increased. A six-lead electrocardiography revealed phases of narrow QRS complexes tachycardia with regular RR intervals and a QP'/P'Q of 0.80, compatible with orthodromic atrioventricular reciprocating tachycardia (OAVRT), alternating with sinus rhythm. On ambulatory electrocardiography, initial phases of OAVRT were followed by sinus rhythm. The sinus beats were

A Case of Persistent Junctional Reciprocating Tachycardia Coincident With Cor Triatriatum Sinister.

Persistent junctional reciprocating tachycardia is a rare form of refractory atrioventricular reentrant tachycardia that accounts for <1% of supraventricular tachycardia in pediatrics. The accessory pathways are generally isolated with few reported underlying structural heart defects. We present a case of a five-month-old male with refractory tachyarrhythmia found to have cor triatriatum sinister, which to our knowledge, is the first reported case of these two rare anomalies coexisting.

Negative lead I P waves during anteroseptal accessory pathway orthodromic reciprocating tachycardia.

Although a negative retrograde P wave in lead I during orthodromic reciprocating tachycardia is thought to indicate a left free wall accessory pathway, we describe similar negative P waves in lead I during orthodromic reciprocating tachycardia through anteroseptally situated accessory pathways.

Results of operative therapy in the permanent form of junctional reciprocating tachycardia.

To assess the results of operative therapy for permanent junctional reciprocating tachycardia, a type of incessant tachycardia, the clinical and electrophysiologic data of 8 such patients referred for management of tachycardia were reviewed. The duration of incessant tachycardia was 14 +/- 10 years (range 2 to 30). The heart rate at rest during tachycardia ranged from 120 to 150 beats/min. Four of 8 patients had cardiomegaly or depressed ejection fraction (16 +/- 10%, range 5 to 27) at presentation and, of these, 2 had symptoms of congestive heart failure. Exertional dyspnea despite normal lef

Conduction time oscillations precede the spontaneous termination of human atrioventricular reciprocating tachycardia.

Prior clinical research indicates that conduction slowing is the primary mechanism leading to the spontaneous termination of reentrant tachycardia in humans. Yet, some experimental models indicate that cycle length oscillations and enhanced conduction are important prerequisites. The role of oscillations in conduction times and enhanced conduction in the spontaneous termination of human reentrant tachycardia has not been adequately investigated. The electrophysiologic features preceding the spontaneous termination of orthodromic atrioventricular (AV) reciprocating tachycardia (RT) were evaluat

[Permanent junctional reciprocating tachycardia in children and adolescents. Efficacy of medical treatment].

Between 1969 and 1991, 11 patients were followed up for permanent junctional reciprocating tachycardia. The average age at diagnosis was 2 years and 4 months (1 day to 14 years). The tachycardia was diagnosed at routine examination in 5 cases and following an episode of cardiac failure in the other 6. Digitalis was prescribed in all patients with 4 good results, 5 average and 2 poor results. One patient, who remained in mild cardiac failure with digitalis therapy, died suddenly at the age of 9 years. In more recent cases, amiodarone was used from the onset or secondarily with good results in a

Reciprocating tachycardia due to a right-sided unidirectional retrograde anomalous pathway (URAP).

An 8 year-old boy had extensive electrophysiological evaluation of his recurrent supraventricular tachycardias. His ECG never showed delta waves but intracardiac stimulation and recording disclosed the following (1) eccentric retrograde atrial activation; (2) increased cycle length and retrograde conduction time following the development of right bundle-branch block; (3) constant retrograde conduction time for increasingly premature ventricular stimuli; (4) atrial captures by ventricular stimuli when the atrioventricular-His pathways were refractory; and (5) no delta waves upon stimulation of

Novel Diagnostic Observations of Nodoventricular/Nodofascicular Pathway-Related Orthodromic Reciprocating Tachycardia Differentiating From Atrioventricular Nodal Re-Entrant Tachycardia.

OBJECTIVES: This study sought to assess the performance of current diagnostic criteria and identify additional electrophysiological features differentiating orthodromic reciprocating tachycardia (ORT) with a concealed nodoventricular/nodofascicular (NV/NF) pathway from atrioventricular nodal re-entrant tachycardia (AVNRT). BACKGROUND: Diagnosing sustained supraventricular tachycardia (SVT) despite the occurrence of ventriculoatrial block (VAB) is challenging. METHODS: We analyzed electrograms of 25 sustained SVTs (9 NV/NF-ORTs [n = 7/2] and 16 AVNRTs) with VAB and 91 AVNRTs without VAB (for
