Reduced attentional capacity, but normal processing speed and shifting of attention in developmental dyslexia: evidence from a serial task.

We report the performance of a group of adult dyslexics and matched controls in an array-matching task where two strings of either consonants or symbols are presented side by side and have to be judged to be the same or different. The arrays may differ either in the order or identity of two adjacent characters. This task does not require naming - which has been argued to be the cause of dyslexics' difficulty in processing visual arrays - but, instead, has a strong serial component as demonstrated by the fact that, in both groups, Reaction times (RTs) increase monotonically with position of a m

Task shifting of point-of-care testing to lay health workers in rural primary health facilities in Nsanje, Malawi.

BACKGROUND: Point-of-care testing (POCT) can reduce diagnostic gaps, but because it frequently falls to overstretched frontline healthcare workers (HCW), its real-world effectiveness is diminished. Despite World Health Organization recommendations for task sharing (TS) with lay health workers (LHW), it remains underutilized. OBJECTIVE: To explore whether task-shifted POCT by LHWs can improve uptake compared to standard HCW-led care. METHODS: A retrospective study at 5 PHC facilities in Nsanje, Malawi, compared a pre-intervention (model 1) period where HCW conducted POCT (for rapid HIV, AHD,

AI in Point-of-Care Imaging for Clinical Decision Support: Systematic Review of Diagnostic Accuracy, Task-Shifting, and Explainability.

BACKGROUND: Artificial intelligence (AI) integrated with point-of-care imaging is a promising approach to expand access in settings with limited specialist availability. However, no systematic review has comprehensively evaluated AI-assisted clinical decision support across multiple point-of-care imaging modalities, assessed explainability implementation, or quantified clinical impact evidence gaps. OBJECTIVE: We aim to systematically evaluate and synthesize evidence on AI-based clinical decision support systems using point-of-care imaging. METHODS: We searched PubMed, Scopus, IEEE Xplore, a

Aberrant thalamocortical connectivity and shifts between the resting state and task state in patients with schizophrenia.

Prominent pathological hypotheses for schizophrenia include auditory processing deficits and dysconnectivity within cerebral networks. However, most neuroimaging studies have focused on impairments in either resting-state or task-related functional connectivity in patients with schizophrenia. The aims of our study were to examine (1) blood oxygen level-dependent (BOLD) signals during auditory steady-state response (ASSR) tasks, (2) functional connectivity during the resting-state and ASSR tasks and (3) state shifts between the resting-state and ASSR tasks in patients with schizophrenia. Dynamic shifts in brain network activation during supracapacity working memory task performance.

Despite significant advances in understanding how brain networks support working memory (WM) and cognitive control, relatively little is known about how these networks respond when cognitive capabilities are overtaxed. We used a fine-grained manipulation of memory load within a single trial to exceed WM capacity during functional magnetic resonance imaging to investigate how these networks respond to support task performance when WM capacity is exceeded. Analyzing correct trials only, we observed a nonmonotonic (inverted-U) response to WM load throughout the classic WM network (including bilat

Task-shifting for lactation support to enhance mother's own milk use in neonatal intensive care unit: a quality improvement initiative.

Mother's own milk improves outcomes for sick and preterm neonates, yet its use in many neonatal intensive care units (NICUs) remains suboptimal. This quality improvement study aimed to increase proportionate consumption of expressed breast milk (EBM) in the NICU and exclusive (EBF) and any breastfeeding (ABF) at discharge by ≥20% above baseline within 12 weeks using sequential Plan-Do-Study-Act (PDSA) cycles and sustain gains thereafter. Root-cause analysis identified poor role clarity in lactation support. In the first implementation phase, neonates receiving <50% EBM were identified during t

Obsessive-compulsive disorder and abstract sequence task contributions shift prefrontal cortical connectivity.

Obsessive-compulsive disorder (OCD) is characterized, in part, by repetitive, sequential behaviors, such as cleaning rituals, yet underlying neural circuitry related to abstract sequencing in OCD remains poorly understood. Prior work has implicated a set of cortical regions activated during abstract sequences, which are defined by rules rather than specific stimulus features (Desrochers et al., 2022). These regions include the rostrolateral prefrontal cortex (RLPFC) that is necessary for performance on abstract sequence tasks (Desrochers et al., 2015), as well as the anterior cingulate cortex/

Memory and perseveration on a win-stay, lose-shift task in rats exposed neonatally to alcohol.

OBJECTIVE: It is important to understand the relationship between perseverative responding resulting from perinatal exposure to alcohol and potential underlying causes, including attention, memory, or response-inhibition problems. The present study was designed to examine the relationship between perseveration and memory. METHOD: Rats exposed neonatally to 6 g/kg/day alcohol from postnatal day (PD) 4 through PD 9 using an artificial rearing technique (n = 8) were compared with an artificially reared gastrostomy control group (n = 8) and a suckle control group (n = 8). Task-Shifting to Nurses for Hypertension Control in Rural Sub-Saharan Africa: A Narrative Review of Effectiveness and Implementation Challenges.

BACKGROUND: Rising cardiovascular morbidity and mortality in rural sub-Saharan Africa are largely attributable to insufficient awareness, treatment and control of hypertension. The delivery of care is further constrained by health system limitations, including shortages of physicians, inadequate infrastructure, challenges in medication supply chains and geographical barriers to access. As a tactic to increase access, adherence and continuity of care, task-shifting hypertension management to nurses-often with the assistance of community health workers-has become popular. The evidence on the eff

From task-shifting to capacity building: A qualitative exploration of facilitators and barriers to general practice nurse education and role expansion.

AIM: This study aimed to explore facilitators and barriers to general practice nurse education that supports expanded practice, from the perspectives of general practitioners (GPs), GPNs and Advanced Nurse Practitioners (ANPs) in general practice. BACKGROUND: Demand on general practice is escalating due to demographic changes and policy initiatives that increasingly shift healthcare delivery to community settings. These developments have substantially expanded the workload and responsibilities of general practice teams. In response, the role of the general practice nurse (GPN) is evolving, wi

A global scoping review of task shifting and sharing interventions to improve the mental health of people living with HIV/AIDS.

People living with HIV/AIDS (PLWH) often experience co-morbid/co-occurring mental health conditions, e.g., depression, anxiety, and post-traumatic stress disorder (PTSD). In resource-limited settings, where provider shortages are common, task shifting and task sharing (i.e., service delivery by non-professionals) are recommended strategies to promote access to and utilization of mental health and psychosocial support (MHPSS) services among PLWH. We conducted a global scoping review of the literature on MHPSS task shifting and sharing intervention studies for PLWH. Data extracted and summarized

"And we just have to keep going": Task shifting and the production of burnout among overdose response workers with lived experience.

Overdose response programs in North America increasingly employ task shifting-shifting overdose response tasks to less specialized workers-to increase effectiveness and promote involvement of people with lived/living experience of drug use (PWLE). In Canada, task shifting has occurred through community-driven implementation of overdose response programs staffed primarily by PWLE. The implications of this task shifting on workers' well-being and service delivery has received little scholarly consideration, despite reports of widespread burnout among frontline responders. This study examines exp

Visuo-postural adaptation during the acquisition of a visually guided weight-shifting task: age-related differences in global and local dynamics.

The effects of aging on the acquisition of a novel visuo-postural coordination task were addressed at two levels: (a) changes in the intersegmental coordination (local dynamics) (b) changes in the coupling of postural sway to the visual driving stimulus (global dynamics). Twelve elderly (age: 71.2 +/- 6.4 years; height: 169.3 +/- 3.8 cm; mass: 72.4 +/- 6.1 kg) and 12 young women (age: 27.1 +/- 4.9 years; height: 178.3 +/- 2.9 cm; mass: 56.7 +/- 4.1 kg) practiced a visually guided Weight-Shifting (WS) task while standing on a dual force platform. Optimizing the delivery of contraceptives in low- and middle-income countries through task shifting: a systematic review of effectiveness and safety.

OBJECTIVE: To assess the effectiveness and safety of task shifting for the delivery of injectable contraceptives, contraceptive implants, intrauterine devices (IUDs), tubal ligation and vasectomy in low- and middle-income countries. METHODS: Multiple electronic databases were searched up to 25 May 2012 for studies which had assessed the delivery of contraceptives by health workers with lower levels of training, compared to delivery by health workers usually assigned this role, or compared to no organized provision of contraceptives. We included randomized controlled trials, non-randomized con

Integration and task shifting for TB/HIV care and treatment in highly resource-scarce settings: one size may not fit all.

BACKGROUND: A crucial question in managing HIV-infected patients with tuberculosis (TB) concerns when and how to initiate antiretroviral therapy (ART). The effectiveness of CD4-stratified ART initiation in a nurse-centered, integrated TB/HIV program at primary care in Kinshasa, Democratic Republic of Congo, was assessed. METHODS: Prospective cohort study was conducted to assess the effect of CD4-stratified ART initiation by primary care nurses (513 TB patients, August 2007 to November 2009). ART was to be initiated at 1 month of TB treatment if CD4 count is <100 cells per cubic millimeter, at

Motor related potential amplitude and latency shifts with repetitive task execution as an indication of motor adaptation.

Motor Related Potentials (MRPs) measure cortical activity relative to a motion task. We evaluated MRPs for a repetitive wrist flexion/extension task to identify changes in the signal characteristics that are indicative of task performance and motor adaptation. Our experiment consisted of a custom-built isometric joystick that controls a cursor on screen, while EEG and EMG data are recorded. The participants (6 healthy adults) performed more than 60 task repetitions, including flexion, extension, and alternating wrist movements while using the joystick. Task performance was tracked as successes

Scaling Out the SafeCare Parenting Program: Comparing Task-Shifted and Standard Implementation in a Refugee Community.

Scale-up efforts of parenting programs have not served diverse communities well due to inadequate curricular adaptations and the lack of a culturally and linguistically competent workforce. We compared delivery of the parent-child interaction module of SafeCare© (SC-PCI) in a refugee community using two implementation approaches: standard delivery via community-based agencies and a task-shifted delivery via community members trained as community health workers (CHWs). Two hundred and six mothers received SC-PCI from trained SafeCare providers. Implementation metrics included the number enrolle

Effects of histamine H(1) receptor antagonists on delayed spatial win-shift task in rats.

We investigated the effects of histamine H(1) receptor antagonists in 3 different stages of spatial working memory in comparison with scopolamine. The effects of drugs were measured using an eight-arm radial maze with a delayed spatial win-shift task in rats. Drugs were injected 10 min before the training phase (timing 1), immediately after the training phase (timing 2), or 10 min before the test phase (timing 3). Intrahippocampal injection of pyrilamine and diphenhydramine resulted in significant effects on the number of correct choices and across-phase errors in all 3 timings. Scopolamine im

Legal outcomes of task-shifting contingency management for youth substance use to rural probation officers: Secondary results from a randomized clinical trial.

OBJECTIVE: Youth substance use disorder is a significant problem in rural communities, yet treatment access is negligible. Task-shifting evidence-based treatments to juvenile probation officers (JPOs) may help, but could increase surveillance and thus be iatrogenic for legal outcomes. Task-shifting contingency management (CM) to JPOs was examined for increased youth legal system consequences. METHOD: Data were from a randomized clinical trial that randomized 32 JPOs in Idaho to probation as usual (PAU) or family-based CM integrated into probation. Then, 160 youth with substance use disorder o
