Medial temporal lobe dysgenesis in hypochondroplasia.

We describe two patients who have hypochondroplasia with medial temporal lobe dysgenesis. This association has only been reported once before. Both patients had an FGFR3 mutation: 1620C --> A, resulting in Asn540Lys. FGFR3 is expressed in the brain during development and plays a role in hippocampal formation. We suggest FGFR3 mutations might cause cerebral malformations in hypochondroplasia as well as in thanatophoric dysplasia. Further neuroimaging studies of patients with hypochondroplasia and epilepsy or developmental delay may clarify the proportion of patients with hypochondroplasia who h

MRI of temporal lobe pleomorphic xanthoastrocytoma.

Pleomorphic xanthoastrocytomas (PXA) are classically supratentorial, peripherally located, well-circumscribed, partially cystic neoplasms, which can enhance on CT following the administration of intravenous contrast agents. Focal calcification may also be seen. Although the CT appearance has been described, we report the MR findings in two cases of histologically documented temporal lobe PXA. The two well-circumscribed lesions were predominantly cystic and both contained a Gd-DTPA enhancing mural nodule. The latter was isointense with gray matter on T1-weighted images and hyperintense on T2-we

Ictal tachycardia during temporal lobe seizures.

A seldom-recognized accompaniment of temporal lobe seizures is tachycardia. This phenomenon was observed in 12 consecutive patients in whom spontaneous seizures were recorded with simultaneous electroencephalographic, electrocardiographic, and videotape monitoring; this finding indicates with a 99% confidence level that this phenomenon may occur in at least 64% of temporal lobe seizures. The autonomic influences responsible for ictal tachycardia during temporal lobe seizures may be inconsequential in patients without cardiac disease but can have serious consequences in patients with cardiac di

Genetic factors in temporal lobe epilepsy.

Genetic investigation of 74 patients with temporal lobe epilepsy, resistant to medication, who underwent unilateral temporal lobectomy 1960-1969. Thirty per cent had a positive family history of epilepsy; 49 per cent of other neurological diseases; and 46 per cent of major psychiatric disorders. Neither of the genetic factors per se influenced the therapeutic result or the psychiatric status at follow-up. A multiple epileptic or neurological predisposition affecting first degree relatives unfavourably influenced the prognosis regarding relief from seizures. A multiple psychiatric predispositio

Neuropeptide systems in temporal lobe epilepsy.

Neuropeptides commonly function as co-transmitters alongside classical neurotransmitters within the same neuron. In temporal lobe epilepsy (TLE), the expression of numerous neuropeptides in the brain is profoundly altered. These changes may reflect activity-dependent regulation of the respective neuronal populations-often associated with enhanced neuronal firing and co-release of neuropeptides with classical transmitters-but may also exert direct functional effects through activation of peptide-specific receptors. This review summarizes alterations in neuropeptide systems in experimental model

[Memory impairments in temporal lobe epilepsy].

Temporal lobe epilepsy is known to present with various cognitive impairments, among which memory deficits are frequently reported by patients. Memory deficits can be classified into two types: classical hippocampal amnesia, which is characterized by abnormalities detected in neuropsychological assessments, and atypical memory deficits, such as accelerated long-term amnesia and autobiographical memory impairment, which cannot be identified using standard testing methods. These deficits are believed to arise from a complex interplay among structural brain abnormalities, interictal epileptic dis

Surgery procedures in temporal lobe epilepsies.

Temporal lobe epilepsy (TLE) is the most common cause of refractory epilepsy amenable for surgical treatment and seizure control. Surgery for TLE is a safe and effective strategy. The seizure-free rate after surgical resection in patients with mesial or neocortical TLE is about 70%. Resective surgery has an advantage over stereotactic radiosurgery in terms of seizure outcomes for mesial TLE patients. Both techniques have similar results for safety, cognitive outcomes, and associated costs. Stereotactic radiosurgery should therefore be seen as an alternative to open surgery for patients with co

Temporal Lobe Epilepsy and Psychiatric Comorbidity.

Most focal seizures originate in the temporal lobe and are commonly divided into mesial and lateral temporal epilepsy, depending upon the neuronal circuitry involved. The hallmark features of the mesial temporal epilepsy are aura, unconsciousness, and automatisms. Symptoms often overlap with the lateral temporal epilepsy. However, the latter present a less evident psychomotor arrest, frequent clones and dystonic postures, and common focal to bilateral tonic-clonic seizures. Sclerosis of the hippocampus is the most frequent cause of temporal lobe epilepsy (TLE). TLE is among all epilepsies the

Behavioral phenotypes of temporal lobe epilepsy.

OBJECTIVE: To identity phenotypes of self-reported symptoms of psychopathology and their correlates in patients with temporal lobe epilepsy (TLE). METHOD: 96 patients with TLE and 82 controls were administered the Symptom Checklist 90-Revised (SCL-90-R) to characterize emotional-behavioral status. The nine symptom scales of the SCL-90-R were analyzed by unsupervised machine learning techniques to identify latent TLE groups. Identified clusters were contrasted to controls to characterize their association with sociodemographic, clinical epilepsy, neuropsychological, psychiatric, and neuroimagi

Microsurgical treatment of temporal lobe cavernomas.

BACKGROUND: Cavernomas of the temporal lobe occur in 10-20% of patients with cerebral cavernomas. They frequently cause epileptic seizures, some of which tend to become refractory to medical therapy. Surgical removal of safely achievable symptomatic lesions has been frequently consistent with good long-term outcome. In the present study, a postoperative outcome is assessed. METHODS: Of our 360 consecutive patients with cerebral cavernomas, 53 (15%) had a single cavernoma in the temporal lobe. Forty-nine patients were treated surgically and were included in the study. All data were analyzed re

Pathophysiology of posttraumatic temporal lobe lesions.

BACKGROUND: Posttraumatic parenchymal lesions in the temporal lobe may cause neurologic deterioration. An analysis was made of the natural evolution of this type of lesion, with emphasis on its 2 components: hemorrhage (hyperdense on computed tomography [CT]), and edema and necrosis (hypodense on CT). The clinical repercussions were studied, and the factors that might influence such evolution were investigated. METHODS: Forty head-injured patients with temporal lobe lesions admitted within 12 hours after the injury were selected in a prospective manner. Computed tomography scans were systemat

Temporal-lobe abnormalities in thanatophoric dysplasia.

We identified distinctive and characteristic abnormalities in the hippocampal formations of 4 individuals with thanatophoric dysplasia (TD), one of whom was a fetus of 19 weeks gestational age. Primitive medial fissures in this subject could be identified, but development of the dentate gyrus and organization of the pyramidal layer in the hippocampal formation were abnormal. We infer that temporal-lobe dysmorphogenesis in TD begins between 11.5 weeks gestational age, when hippocampal and fimbriodentate fissure formation takes place, and 13.5 weeks gestational age, when differential proliferati

Temporal lobe lesions in head injury.

In cases of closed head injury temporal lobe lesions e.g. contusion, laceration, pulping, or intracerebral haematoma frequently result in an expanding process. These are frequently associated with an overlying subdural haematoma. Eighty five cases of such lesions from a consecutive series of 1,000 cases of head injury have been analyzed. The lesions are caused by severe injury resulting in loss of consciousness and skull fracture. Most of them are contre-coup lesions. Clinically they manifest themselves like any other acute or subacute intracranial hematoma. The majority of the patients have c

Amygdalotomy for bilateral temporal lobe seizures.

A 37-year-old woman had bilateral anygdalotomy for psychomotor and minor motor seizures and long periods of very pronounced mental depression. The patient has been seizure-free for four years postoperatively. A preoperative left temporal spike focus disappeared after extensive left anygdalotomy. Of particular interest in this case is the recovery of olfactory and memory functions which were partially impaired by the bilateral lesions. Bilateral anygdalotomy is effective in relieving temporal lobe seizures without the complications and deficits that usually occur with resections of the bilatera

Medial Temporal Lobe Damage Impairs Temporal Integration in Episodic Memory.

Although the role of the medial temporal lobe (MTL) and the hippocampus in episodic memory is well established, there is emerging evidence that these regions play a broader role in cognition, specifically in temporal processing. However, despite strong evidence that the hippocampus plays a critical role in sequential processing, the involvement of the MTL in timing per se is poorly understood. In the present study, we investigated whether patients with MTL damage exhibit differential performance on a temporal distance memory task. Critically, we manipulated context shifts, or boundaries, which

OBJECTIVE: To assess the clinical implications and the pathophysiologic determinants of interictal bitemporal hypometabolism (BTH) in temporal lobe epilepsy (TLE) not associated with bilateral MRI abnormalities or intracranial space-occupying lesions. METHODS: The authors compared the clinical, interictal, and ictal EEG, Wada test, and neuropsychology data of 15 patients with intractable complex partial seizures of temporal lobe origin and BTH with those of 13 consecutive patients with unilateral TLE associated with unilateral temporal hypometabolism (UTH) who remained seizure free for more t
