Human Infections by Multiple Spotted Fever Group Rickettsiae in Tennessee.

Rocky Mountain spotted fever is the most common tick-borne disease in Tennessee. However, Rickettsia rickettsii has rarely been isolated from endemic ticks, suggesting rickettsioses may be caused by other species. A total of 56 human serum samples that were serologically positive for exposure to Rickettsia were obtained from commercial laboratories in 2010 and 2011. In addition, 20 paired sera from patients with encephalitis and positive Rickettsia serology were obtained from the Tennessee Unexplained Encephalitis Surveillance (TUES) study. Using an immunofluorescence assay, reactivity of the

Opioid use disorder during pregnancy in Tennessee: expediency vs. science.

Methadone and buprenorphine are highly effective and commonly prescribed for the treatment of opioid use disorder. Both medications are also efficacious for the treatment of pregnant women with this disorder. In one third of states, however, Medicaid reimbursement will cover the cost of buprenorphine, but not methadone, to treat opioid use disorder in pregnant women. This commentary will explore the clinical and policy rational and consequences of this policy, with the opinion that this approach is guided by political expediency rather than sound clinical research. The commentary will focus on

Powassan virus infection presenting as acute disseminated encephalomyelitis in Tennessee.

Powassan virus is a rarely diagnosed cause of encephalitis, and is associated with significant neurologic sequelae. Although symptomatic infections with Powassan virus occur primarily in adults, we report a case of confirmed Powassan neuroinvasive disease in a child presenting to a Tennessee hospital, with symptoms and imaging studies suggestive of acute disseminated encephalomyelitis.

Trichinellosis associated with bear meat--New York and Tennessee, 2003.

Trichinellosis is a parasitic infection caused by tissue-dwelling Trichinella roundworms and is associated traditionally with ingestion of pork from infected domestic swine. As a result of improvements in swine production, trichinellosis has declined steadily in the United States. However, infection also can result from eating the meat of wild animals. During 1997-2001, a total of 72 cases of trichinellosis (median: 12 cases annually; range: 11-23 cases) were reported to CDC; the majority of these infections were associated with eating wild game, predominantly bear. This report describes three

The Impact on Environmental Health from Cemetery Waste in Middle Tennessee.

The burial of caskets with arsenic-treated wood and formaldehyde-based embalming fluids can harm the environment and health. Arsenic (As) can leach into water, affecting aquatic life and the food chain. Formaldehyde can contaminate groundwater, risking drinking water and causing health problems. The purpose of this study was to investigate the prevalence of As and formaldehyde in cemetery plots of different ages. For this, we evaluated whether there is a potential for formaldehyde and As from cemetery caskets to contaminate waterways, which could impact livestock and allow transmission to indi

Revisiting the Fetal Assault Law in Tennessee: Implications and the Way Forward.

The rising prevalence of opioid use disorder (OUD) among those living in the United States has demanded a collaborative response from health care and policy spheres. Addressing OUD among pregnant women is especially difficult, given the controversies surrounding the medical and ethical balance between meeting maternal versus fetal/newborn needs. Most medical organizations discourage the criminalization of drug use in pregnancy due to the adverse public health outcomes of such an approach. Despite this recommendation, many states continue to use punitive law to address drug use in pregnancy. In

A Data-Driven Mathematical Model of the Heroin and Fentanyl Epidemic in Tennessee.

Opioid addiction represents a major national health issue spanning decades. In recent years, prescription opioid use disorder has increasingly led to heroin and fentanyl use, with subsequent increases in mortality rates due to overdose. In this paper, we present a mechanistic, epidemic model for prescription opioid addiction and illicit heroin or fentanyl addiction which aims to better understand and predict the dynamics between these two stages of opioid use disorder. Our model aims to be both parsimonious and robust: as a system of five differential equations it is appropriate for use in the

Environmental investigation following the first human case of babesiosis in Tennessee.

Babesiosis is an emerging tick-borne zoonotic disease in the United States caused by Babesia parasites. In 2009, the first case of babesiosis was documented in Tennessee. Environmental investigation at the reported site of tick exposure included collection of ticks and specimens from eastern cottontail rabbits (Sylvilagus floridanus) and white-tailed deer ( Odocoileus virginianus ) that were tested for piroplasms by molecular and serologic methods. One hundred and sixty-six Ixodes scapularis ticks and biological samples from 8 rabbits and 5 deer were collected. Ixodes scapularis were PCR posit

A case-control study of sporadic canine blastomycosis in Tennessee, USA.

This study investigated environmental and host risk factors of canine blastomycosis in Knox County, Tennessee, USA. Data on 78 cases and 146 randomly selected controls were extracted from the medical database at the University of Tennessee Veterinary Teaching Hospital for the period 1977-1999. Home addresses of cases and controls were geocoded and linked to environmental risk factor data using a Geographic Information System (GIS) software. Multiple logistic regression analysis was performed to identify environmental and host factors associated with risk of canine blastomycosis. Important risk

Vaccinations for Expecting Mothers to Improve Pregnancy Care in Middle Tennessee.

Background: During pregnancy, mothers and their infants are at increased risk for complications due to COVID-19 infection, influenza, and pertussis. At the time of writing, the previous advisory committee on immunization practices (ACIP) recommended that pregnant women receive the COVID-19 vaccine, influenza, tetanus-toxoid, reduced diphtheria toxoid, and acellular pertussis (Tdap) vaccine, as well as respiratory syncytial virus vaccinations during pregnancy. The COVID-19 pandemic greatly impacted routine vaccinations especially among medically underserved women in the South. The barriers to r

Prevalence and Pathology Associated with Sarcocystis spp. in Raptors in Tennessee, USA.

Raptors may serve as both intermediate and definitive hosts for Sarcocystis spp. Past research has documented fatal encephalitis in raptors caused by various Sarcocystis spp., whereas other surveys have found a high prevalence of tissue cysts without evidence of disease. Little is known about the prevalence of Sarcocystis in raptors in the eastern USA. The aim of this study was to determine the occurrence of tissue cysts and histopathologic changes associated with Sarcocystis spp. infection in raptors in eastern Tennessee. Tissues of 33 raptors from Tennessee, USA, were assessed with histopath

First report of Toxocara vitulorum infection in a dairy calf in Tennessee.

The ascarid nematode, Toxocara vitulorum, is a destructive parasite of ruminants with a world-wide distribution that causes mortality in calves, especially those with poor nutrition. Reports of T. vitulorum are scarce in the continental U.S. and Canada. A single gravid female T. vitulorum was discovered on the necropsy of a 2.5-month-old miniature Jersey calf from a dairy farm in Sparta, Tennessee. Genetic analysis of the 18S and COI genes revealed a 100 % match to T. vitulorum samples isolated from beef calves in Iowa and a 96.73 % match to T. vitulorum isolated from a yak in Austria. Althoug

Who Is Not Linking to HIV Care in Tennessee - the Benefits of an Intersectional Approach.

INTRODUCTION: Guided by an intersectional approach, we assessed the association between social categories (individual and combined) on time to linkage to HIV care in Tennessee. METHODS: Tennessee residents diagnosed with HIV from 2012-2016 were included in the analysis (n=3750). Linkage was defined by the first CD4 or HIV RNA test date after HIV diagnosis. We used Cox proportional hazards models to assess the association of time to linkage with individual-level variables. We modeled interactions between race, age, gender, and HIV acquisition risk factor (RF), to understand how these variables

Rats! Hantavirus: A Case Report of a Suspected Case in Eastern Tennessee.

INTRODUCTION: Hantavirus is an RNA virus that is transmitted to humans by inhalation of aerosolized excrement from infected rodents. This case report demonstrates the value of taking a thorough social history and highlights the challenges associated with early diagnosis of this viral infection. CASE PRESENTATION: We highlight a case of suspected hantavirus infection with subtle gastrointestinal and pulmonary symptoms that challenged the initial diagnosis. DISCUSSION: Efforts are needed to improve clinical recognition and rapid detection of hantavirus infections, to reduce associated mortalit

A successful decade of regionalized perinatal care in Tennessee: the neonatal experience.

We hypothesized that successful implementation of regionalized perinatal care would result in early identification and antenatal referral of high-risk neonates, and also improved stabilization before and during transport of those transferred postnatally. We conducted a retrospective study of demographic characteristics and transport outcome in two defined groups of neonates transported to regional perinatal centers in Tennessee, one group (n = 218) from the first year of regionalization (1975), and a second group (n = 261) from the 12th year (1986). The percentage of outborn infants decreased,

Clinical and epidemiologic features of strongyloidiasis. A prospective study in rural Tennessee.

Hospitalized and domiciliary patients were studied to determine the incidence of the endemic nematode Strongyloides stercoralls in stool samples. Strongyloides was found in 14 (6.1%) of 229 hospitalized patients and in nine (2.6%) of 346 domiciliary patients. Clinical symptoms, laboratory data, and underlying diseases were compared for stool-positive and stool-negative patients. Infected patients were more likely to complain of abdominal bloating. They had a higher incidence of eosinophilia and guaiac-positive stools. They were more likely to have been treated with corticosteroids, cimetidine,

Improving Delayed Cord Clamping Across Tennessee Through a Statewide Quality Collaborative.

OBJECTIVE: The objective of this initiative was to increase the proportion of infants born in participating hospitals receiving the benefits of delayed cord clamping (DCC) for at least 60 seconds to a minimum of 90% for each facility. METHODS: In January 2022, a quality improvement (QI) initiative was launched across 5 pilot hospitals, later expanding to 22 additional hospitals in May 2022. The goal of the initiative was to ensure that all newborns at each facility experienced a delay of at least 60 seconds before umbilical cord clamping. Monthly data collection continued through June 2023, t
