Arthroscopic shoulder anchorless biceps tenodesis: a novel intracapsular technique.

Pathology of the long head of the biceps tendon (LHBT) is a common source of anterior shoulder pain and is frequently encountered during arthroscopic shoulder surgery. Although biceps tenodesis is widely performed, many existing techniques rely on implants that increase cost and may be associated with bone- or fixation-related complications. This paper describes a novel arthroscopic intracapsular soft-tissue biceps tenodesis technique emphasizing efficiency, reproducibility, and implant minimization. The LHBT is pierced percutaneously using a spinal needle approximately 1 cm proximal and later

Arthroscopic Intra-articular Biceps Tenodesis With All-Suture Anchor.

BACKGROUND: Biceps tenodesis can be used to treat both superior labral anterior and posterior (SLAP) tears and degenerative biceps pathologies in the setting of rotator cuff pathology or as stand-alone techniques. Research has shown that arthroscopic biceps tenodesis has comparable outcomes to open tenodesis with the benefit of being less invasive. INDICATIONS: Surgical indications for intra-articular tenodesis of the long head of the biceps (LHB) include SLAP tear, failed SLAP repair, partial-thickness LHB tear, biceps instability, tenosynovitis, and clinical examination consistent with LHB

Near-anatomic allograft tenodesis of chronic lateral ankle instability.

BACKGROUND: Current operative treatment options for chronic lateral ankle instability include anatomic repairs utilizing existing local tissue and nonanatomic reconstructions sacrificing the peroneus brevis tendon to mechanically stabilize the ankle. Recent studies have modified these techniques to create an anatomic reconstruction utilizing allograft tendons. The purpose of this study was to retrospectively examine the clinical outcomes of a near-anatomic ligament reconstruction utilizing an allograft tendon for recurrent or complex lateral ankle instability. METHODS: Twenty-eight patients u

Rebalancing the tetraplegic wrist using extensor carpi ulnaris-tenodesis.

Patients with cervical spinal cord injury and tetraplegia often present with a radial deviation deformity of the wrist owing to impaired active wrist flexion and extension. Tenodesis of the extensor carpi ulnaris can help optimize grip strength. The purpose of the study was to compare reconstruction of the grip with and without extensor carpi ulnaris-tenodesis, as well as evaluating the outcome of the procedure. The grip strength of the group with tenodesis of the extensor carpi ulnaris was twice as strong as of the group without the tenodesis and with similar wrist joint flexion-extension ran

Flexor tenodesis procedure in the treatment of lesser toe deformities.

In this technical report study, we describe the use of a flexor tenodesis procedure in the treatment of lesser toe deformities (LTD). Using a specific implant, both the flexor digitorum longus and brevis tendons are attached to the plantar aspect of the proximal phalanx, allowing dynamic correction of flexible deformities of metatarsophalangeal and interphalangeal joints. Good clinical results and absence of complications were observed in a series of 3 patients, with considerable correction of the LTD, and absence of substantial residual floating toe or metatarsophalangeal joint stiffness. Subpectoral Biceps Tenodesis of the Shoulder: Indications and Technique Options.

Subpectoral biceps tenodesis of the shoulder may be a useful tool that can address a wide range of disorders in the setting of pathology of the long head of the biceps tendon. Primary indications include (1) zone 2 or zone 3 tendon pathology and (2) failed previous proximal tendon tenodesis. Secondary indications include (1) an overhead athlete or thrower, (2) chronic tendinopathy, and (3) surgeon preference. A subpectoral technique allows tendon fixation directly posterior (deep) to the pectoralis tendon high in the bicipital fossa or in the mid fossa or fixation low in the fossa inferior to

The Effects of Anterolateral Tenodesis on Tibiofemoral Contact Pressures and Kinematics.

BACKGROUND: Anterolateral tenodeses are increasingly popular in combination with intra-articular anterior cruciate ligament reconstructions. Despite the perception of risk of overconstraint and lateral osteoarthritis, evidence is lacking regarding the effect of graft tensioning on knee kinematics and intra-articular compartmental joint pressures. PURPOSE: To investigate tibiofemoral joint contact pressures and kinematics related to an anterolateral lesion and the effectiveness of a MacIntosh tenodesis in restoring these when varying (1) graft tension and (2) tibial rotation during graft fixat

Biomechanics of a novel technique for suprapectoral intraosseous biceps tenodesis.

BACKGROUND: The Caspari-Weber (C.W.) tenodesis is a standard miniopen intraosseous technique to fix the long head of the biceps tendon. The suprapectoral intraosseous biceps tenodesis (SPIBiT) is a novel arthroscopic, intraosseous, tendon-sparing alternative using a cortical button. No biomechanical data exist comparing the time-zero performance of the SPIBiT and C.W. constructs. METHODS: Nine pairs of human cadaver shoulders were tested. The SPIBiT used a finger-trap suture pattern holding the tendon inside a humeral tunnel above the pectoralis tendon, anchored with a cortical button on the

Outcomes of dorsal capsulodesis and tenodesis for treatment of scapholunate instability.

PURPOSE: The ideal treatment for scapholunate (SL) instability has not yet been established. This study examined the results of 2 procedures for the primary treatment for chronic SL dissociation of dorsal capsulodesis and flexor carpi radialis tenodesis. METHODS: A retrospective analysis was conducted that examined dorsal capsulodesis and tenodesis procedures performed for chronic SL instability between January 1995 and February of 2003. Twenty-nine patients were identified with isolated chronic SL instability. Of the 29 patients, 14 had a dorsal capsulodesis procedure and 15 had a tenodesis

Semitendinosus tenodesis for repair of recurrent dislocation of the patella in children.

Recurrent dislocation of the patella is more common in girls than in boys. Although several predisposing factors may exist, patellar dislocation is most commonly associated with familial ligamentous laxity. Many surgical repairs have been described to stabilize the patella. We have found the semitendinosus transfer to the patella to result in a predictable, stable patellofemoral joint without risk of injury to the proximal tibial physis. Between January 1990 and December 1997, 29 children have been treated at the Children's Hospital of Eastern Ontario with a semitendinosus transfer for recurre

Lateral Extra-Articular Tenodesis as an Adjunct to ACL Reconstruction.

BACKGROUND: Lateral extra-articular tenodesis (LET) is a surgical technique used to decrease the risk of anterior cruciate ligament reconstruction (ACLR) failure by supplementing internal rotational stability and reducing pivot shift. INDICATIONS: A growing body of literature indicates that LET should be performed for patients under 25 years, those with increased posterior tibial slope and ligamentous laxity, and elite athletes in cutting and/or pivoting sports. Additional indications include patients with grade 2 pivot shift or greater and those with a history of anterior cruciate ligament g

Disorders of the long head of the biceps: tenotomy versus tenodesis.

Disorders of the long head of biceps tendon are common in clinical practice. Their causes could be degenerative, inflammatory, instability (subluxation or luxation) or traumatic. They are generally associated to other diseases of the shoulder, mainly rotator cuff injuries. Currently, there is controversy in the literature regarding the indications for surgical treatment and the choice of the best technique for each case, due to the possibility of esthetic deformity, loss of muscle strength, and residual pain. The objective of this study was to identify the indications for surgical treatment, t

Management of failed biceps tenodesis or tenotomy: causation and treatment.

The purpose of this paper is to review the management of long head biceps tendon pathology, with a particular emphasis on a prior failed biceps tenotomy or tenodesis. Failed biceps tenotomy generally results from a lack of thorough preoperative discussion of potential outcomes rather than from technical problems. Patients with unsatisfactory results can be treated with conversion to a biceps tenodesis. Failed biceps tenodesis is usually recognized with persistent pain in the area of the bicipital groove, often caused by either the mechanical failure of the tenodesis or associated shoulder path

Bicipetal tenodesis for anterior subluxation of the superior tibiofibular joint.

A technique for stabilising the superior tibiofibular joint using an autogenous biceps graft passed through a tibial tunnel is described. The common peroneal nerve should be decompressed and the lateral inferior genicular artery protected. The technique proved to be safe and effective in two patients who were followed for at least two years.

Biomechanical assessment of a new tenodesis for correction of hallux varus.

Each of six below-the-knee amputation specimens were transfixed to a wooden block and mounted to a jig on an amputee testing device preloaded with 5 N applied to the proximal phalanx and displaced at a constant rate of 2 mm/min. Load displacement curves were generated for the intact joint and after sequential incisions of the lateral capsule, the adductor hallucis, and the lateral slip of the flexor hallucis brevis tendon, which caused varus dislocation of the hallux. An extensor hallucis brevis tenodesis was performed after the varus dislocation. Division of the lateral capsule, the adductor,

Voice-Controlled Dual-Site Functional Electrical Stimulation Enables Tenodesis Grasping.

Patients with high-level spinal cord injuries (SCIs) that maintain wrist movement but lack the ability to close the hand volitionally may preserve some degree of hand function by using the tenodesis grasp. The tenodesis grasp consists of extending the wrist to passively shorten the finger flexors, resulting in the fingers closing. However, the tenodesis grasp is unavailable to patients who lack wrist function. To address this, we developed a voice-controlled, dual-site, functional electrical stimulation system that evokes the tenodesis grasp by stimulating the wrist extensors and finger flexor
