[The functional significance of the suspending ligaments of the ear ossicle chain].

The contraction of the tensor tympani muscle has not enough force to stabilise the drum membrane-malleus complex against a negative pressure in the external ear nor against a hyperpressure in the middle-ear, as in sneezing, Valsalva's manoeuvre, etc. The ossicular ligaments must therefore perform this task. It was shown in experiments with fresh temporal bone preparations that the superior malleolar ligament diminishes the pressure-induced displacements of the malleus. This proves that this often neglected ligament has true functional significance. The most important ligament, which retains th

Characterization of middle ear ossicular ligaments using photon counting detector CT.

BACKGROUND AND PURPOSE: The middle ear suspensory ossicular ligaments have been previously described on traditional energy integrating detector CT scanners. With recent developments in photon counting detector CT, these ligaments can be better visualized due to greater spatial and contrast resolution. MATERIALS AND METHODS: 50 patients (100 temporalbones) without hearing loss or middle ear cavity pathology were retrospectively reviewed to characterize the middle ear suspensory ossicular ligaments including the anterior, superior, lateral mallear, and posterior incudal ligaments, and stapes an

Endoscopic Anatomy of the Chorda Tympani: Systematic Dissection, Novel Anatomic Classification, and Surgical Implications.

HYPOTHESIS: A transcanal endoscopic approach enables visualization of the variable course of the chorda tympani inside the middle ear. BACKGROUND: The chorda tympani is the longest intrapetrous branch of the facial nerve. Despite having been investigated in several studies, a description of its tympanic tract from an endoscopic point of view is lacking in the literature. METHODS: We performed transcanal endoscopic dissections of 44 human cadaveric head and ear specimens. The entry point of the chorda tympani into the middle ear was classified into four categories according to its location, a

Diagnosis of Menière's disease.

Stapedius-reflex measurement and the glycerol-test in combination with pure tone audiogram and speech-audiogram are reliable methods for an exact diagnosis of Menière's disease. For differential diagnosis one should know the so called "tensor tympani syndrome" which is discussed in this paper.

Tympanic muscle reflex elicited by electric stimulation of the tongue in normal and pathological subjects.

A bilateral reflex contraction of the tensor tympani muscle has been obtained in man by electric stimulation of the tongue (1-2 mA). The stimulus is well tolerated and always effective. The advantage is stressed of eliciting a contraction of this muscle without involvement of the stapedius, as occurs with other methods. An analysis has been subsequently conducted in normal subjects and in patients affected by pathology of the tympano-ossicular system: tympanosclerosis, otosclerosis, suprastapedial facial paralysis; in cases of interruption of the afferent arch: section of the homo-lateral ling

[Diagnosis and treatment of middle ear myoclonic tinnitus].

Middle ear myoclonus tinnitus is an abnormal sound perceived by unusual contraction of the stapes muscle or tensor tympani muscle. The current diagnostic methods include medical history inquiry, physical examination and audiological tests. The diagnosis was confirmed by tympanic exploration of myoclonus. Etiological treatment is considered to be the first treatment of choice, supplemented by behavior therapy and pharmacotherapy. Surgery will be performed when conservative treatment fails. This article reviews the diagnosis and treatment of middle ear myoclonus tinnitus.

Eustachian Tube Function.

The fibrocartilaginous eustachian tube is part of a system of contiguous organs including the nose, palate, rhinopharynx, and middle ear cleft. The middle ear cleft consists of the tympanic cavity, which includes the bony eustachian tube (protympanum) and the mastoid gas cells system. The tympanic cavity and mastoid gas cells are interconnected and allow gaseous exchange and pressure regulation. The fibrocartilaginous eustachian tube is a complex organ consisting of a dynamic conduit with its mucosa, cartilage, surrounding soft tissue, peritubal muscles (ie, tensor and levator veli palatine, s

Experimental observation of particulate flow along cranial nerves.

The purpose of this study was to investigate the communicative routes via the cranial nerves among the middle ear, the auditory tube, the inner ear, and the subarachnoid space under nearly physiological conditions. Various volumes of Indian ink were slowly injected into the facial nerve trunk of rabbits at the stylomastoid foramen, then serial sections of the heads were observed under the light microscope. This study yielded the following results: (i) Indian ink penetrated into the subarachnoid space through the internal auditory canal and/or the jugular foramen along the nerve sheaths of the

Dental occlusion as one cause of tinnitus.

There is large support in literature linking tinnitus to dental occlusion and temporomandibular joint disorders (TMD). However, there is no model to explain such a link. This hypothesis explains how the fusimotor system of the muscles innervated by the trigeminal motor nucleus is affected by inadequacies in the occlusion of the teeth that cause changes in posture and movement of the mandible. Reptile to mammal evolution shows that stomatognathic structures underwent changes related to mastication. Among several changes, there was the appearance of a new articulation between the mandible and sk

Treatment of objective tinnitus with transpalatal Botox® injection in a pediatric patient with middle ear myoclonus: A case report.

We describe a case of bilateral middle ear myoclonus (MEM) that was treated with trans-palatal Botox® injection after failing surgery to section the tensor tympani and stapedial tendons. MEM is a rare disorder resulting from rhythmic contraction of middle ear muscles. An 8-year old girl presented with audible clicking tinnitus, and resultant inability to focus in school. Her parents declined medical therapy, and she underwent surgeries for tendon lysis, with only temporary relief. She had successful response to trans-palatal Botox® injection to the tensor veli palatini aponeurosis. Trans-palat

Effects of stapedial arch fractures on conductive hearing loss and stapedial reflexes.

OBJECTIVE: It has been stated in the clinical literature that stapedial fractures could produce a significant conductive hearing loss while leaving the contralateral stapedial reflex intact. The objective of this study is to test this hypothesis in an animal model. DESIGN: Nine hooded rats underwent acoustic-stapedial reflex (ASR) and cochlear microphonic (CM) threshold determinations before and after middle ear surgery. An argon laser was used to divide the tensor tympani tendon, the anterior stapedial curs, and the posterior stapedial crus, sequentially. The initial functional measures were

[Anatomical factors of temporal bone residual cholesteatoma: our experience].

The article describes clinical cases of invasive cholesteatoma of the temporal bone pyramid, leading to a massive destruction of the surrounding structures. Paying attention to the importance of knowing the microsurgical anatomy of critical structures of temporal bone (the anterior bony plate (cog), the tensor tympani fold and the tendon, the tympanic orifice of the eustachian tube, the pretympanic recess, tympanic sinuses) bearing a significance and helping to improve the removal of the invasive cholesteatoma.

Endoscopic approach to tensor fold in patients with attic cholesteatoma.

CONCLUSION: The endoscopic approach to attic cholesteatoma allows clear observation of the tensor fold area and consequently, excision of the tensor fold, modifying the epitympanic diaphragm. This permits good removal of cholesteatoma and direct ventilation of the upper unit, preventing the development of a retraction pocket or attic cholesteatoma recurrence, with good functional results. OBJECTIVES: An isthmus block associated with a complete tensor fold is a necessary condition for creation and development of an attic cholesteatoma. During surgical treatment of attic cholesteatoma, tensor f

A clockwork ear.

Objective tinnitus refers to a tinnitus that can also be heard by the examiner. It is a relatively rare condition, and can be misdiagnosed or neglected. Some causes of objective tinnitus are head and neck vascular malformations, or muscular myoclonus of the tensor tympani, stapedial, or palatal muscles. The case of an 11-year-old girl with an objective tinnitus lasting from 1 year is herein presented, and the diagnostic workup performed in this unusual case is described.

Bilateral tinnitus due to middle-ear myoclonus.

Tinnitus is a common otological symptom. Usually it is subjective (perceived only by the patient); very rarely is it objective (heard by both the patient and the examiner) Objective tinnitus due to middle-ear myoclonus is extremely rare, with only a few case reports published in the literature. We present three cases of objective tinnitus caused by middle-ear myoclonus. All patients were cured by tympanotomy with stapedial and tensor tympani tendon section.

Suppressed sneezing as a cause of hearing loss and vertigo.

Two cases of inner ear injury caused by suppressed sneezing are described. One patient experienced vestibular symptoms in the form of reflexogenic vertigo that was relieved by surgical section of the tensor tympani tendon. The other patient had a sudden severe permanent sensorineural hearing loss. It is proposed that the aerodynamic pressure increase associated with suppressed sneezing is transmitted via the eustachian tube to cause an implosive fistula of either the round or oval window with injury to the membranous labyrinth.

Origin and distribution of facial nerve anatomy in van cats.

This study aims to reveal the morphological properties of facial nerve and the middle ear in Van cats. Study material was composed of 6 female Van cats. Dissections were performed under a Zoom Stereo Microscope. There was no plexus buccalis in Van cats. The chorda tympani was observed to pass through an opening in the tympanic cavity, emerge through a small opening just behind the retroarticular process, and join the lingual nerve. A rounded anatomical formation with a size of 2.75 ± 0.3 mm was found to be located within the mastoid process of the temporal bone between the facial nerve and the
