[Skin-soft tissue replacement on the lower leg--therapy and prophylaxis of osteomyelitis].

The adequate and early replacement of soft tissue in the lower leg is equivalent to the management of bone and prevention of osteomyelitis. The acute or delayed primary management of soft tissue loss with early flap coverage prevents infections. In the treatment of osteomyelitis, flap coverage provides the necessary blood supply for healing. In the first stage we remove bone until bleeding indicates viable tissue, gentamycin-PMMA-chains are introduced before flap coverage. In the second stage, if necessary, we substitute the gentamycin-PMMA-chains with bone graft.

The Role of Hyperbaric Oxygen Therapy in Management of Necrotizing Soft Tissue Infection.

Necrotizing soft tissue infection (NSTI) is a life-threatening, high morbidity pathology that requires aggressive, multidisciplinary management. Surgery and antibiotic administration are core components of treatment. Adjunctive incorporation of hyperbaric oxygen therapy (HBOT) can further enhance treatment and recovery. Benefit is achieved through multiple effects brought about by increase of local and systemic oxygen tension. Direct effects include bacteriostasis, disruption of bacterial toxin production, and attenuation of inflammation. Indirect benefits include demarcation of viable tissue

The effect of hyperbaric oxygen therapy on the bout of treatment for soft tissue infections.

OBJECTIVES: Hyperbaric oxygen (HBO) therapy is often combined with antibiotic therapy for infections such as gas gangrene and osteomyelitis. Although numerous investigations have been undertaken to assess the effect of adjunctive HBO therapy on the treatment of infections, the bout of treatment has not been referred in the previous investigations. The purpose of this retrospective study was to evaluate the efficacy of HBO therapy on the bout of treatment for soft tissue infections. PATIENTS AND METHODS: In the period between 1994 and 2001, we treated 23 patients with soft tissue infections. N

Recurrent Soft Tissue Infections Associated With Burosumab Therapy in X-Linked Hypophosphatemic Rickets.

X-linked hypophosphatemic rickets (XLH) is a genetic disorder characterized by elevated fibroblast growth factor 23 (FGF23), resulting in renal phosphate wasting and inadequate bone mineralization. Burosumab, a monoclonal antibody that inhibits FGF23 activity, has shown promise in improving renal phosphate reabsorption and clinical outcomes in XLH patients. However, the potential side effects of burosumab, particularly its impact on immune function and susceptibility to infections, remain a subject of concern. In this case report, we describe a 57-year-old male individual with XLH who experien

Preclinical evaluation of oncolytic vaccinia virus for therapy of canine soft tissue sarcoma.

Virotherapy using oncolytic vaccinia virus (VACV) strains is one promising new strategy for canine cancer therapy. In this study we describe the establishment of an in vivo model of canine soft tissue sarcoma (CSTS) using the new isolated cell line STSA-1 and the analysis of the virus-mediated oncolytic and immunological effects of two different Lister VACV LIVP1.1.1 and GLV-1h68 strains against CSTS. Cell culture data demonstrated that both tested VACV strains efficiently infected and destroyed cells of the canine soft tissue sarcoma line STSA-1. In addition, in our new canine sarcoma tumor x

Applying negative pressure therapy to deep pressure ulcers covered by soft necrotic tissue.

Negative pressure therapy was applied to 39 deep pressure ulcers covered by soft necrotic tissue. All of the wounds were so deep that there were bones or ligaments just beneath the soft necrotic tissue. They had already received several types of conservative therapy including a necrotomy for periods ranging from 1 to 72 months. The wounds were minimally debrided and put in an adequately wet environment with negative pressure. This environment was established by the application of a suction-dressing. All of these wounds showed clear wound beds within only 1 month. All of the wounds were success

Negative Pressure Wound Therapy With and Without Instillation in Necrotizing Soft Tissue Infections.

Background: Necrotizing soft tissue infections (NSTIs) are rare but deadly infections that require early and often extensive surgical debridement. After debridement, patients frequently have substantial morbidity because of large, open wounds. Hypothesis: Negative pressure wound therapy with instillation (NPWTi) results in higher wound closure rates compared with traditional negative pressure wound therapy (NPWT) or wet to dry dressings (moist wound care dressing). Patients and Methods: A prospectively maintained Acute and Critical Care Surgery database spanning 2008-2018 was queried for patie

Late Soft Tissue Complications of Head and Neck Cancer Therapy: Lymphedema and Fibrosis.

Head and neck cancer and its treatment result in soft tissue damage secondary to lymphedema and fibrosis. Lymphedema is the result of pathological accumulation of interstitial fluid in tissues. It is caused by the inability of the lymphatic system to transport lymph fluid from the tissues to the central circulatory system and is manifested clinically by tissue swelling. Fibrosis is defined as an overaccumulation of fibrotic tissues within the skin and soft tissues after a single or repetitive injury and is characterized by hardening of the soft tissues with associated loss of elasticity. Lymph

Seeing the Light--the truth about soft tissue lasers and nonsurgical periodontal therapy.

Although laser treatment has generated considerable interest among dentists and the public, there is no evidence that any laser system adds clinical value over and above scaling and root planing and conventional surgical treatment for periodontitis. Following a brief explanation of the mechanism behind soft tissue lasers, the evidence on the use of laser therapy in addition to traditional nonsurgical periodontal treatment in the management of periodontal diseases is reviewed.
