Acute, chronic and differential effects of several anesthetic barbiturates on glutamate receptor activation in neuronal culture.

The acute and chronic effects of several anesthetic barbiturates, in therapeutic concentrations, on the excitatory amino acid (EAA)-induced elevation of intracellular calcium levels ([Ca2+]i) were examined in neuronal tissue culture. The ultrashort-acting barbiturate, thiamylal, was effective in blocking elevations of [Ca2+]i induced by kainate, N-methyl-D-aspartate (NMDA), and quisqualate or by membrane depolarization with 40 mM KCl. The structurally similar barbiturate, secobarbital which differs from thiamylal only by having an oxygen in place of a sulfur, was able to block elevations induc

A novel step-down infusion method of barbiturate therapy: Its safety and effectiveness for intracranial pressure control.

Intracranial pressure (ICP) has to be maintained quite constant, because increased ICP caused by cerebrovascular disease and head trauma is fatal. Although controlling ICP is clinically critical, only few therapeutic methods are currently available. Barbiturates, a group of sedative-hypnotic drugs, are recognized as secondary treatment for controlling ICP. We proposed a novel "step-down infusion" method, administrating barbiturate (thiamylal) after different time point from the start of treatment under normothermia, at doses of 3.0 (0-24 h), 2.0 (24-48 h), 1.5 (48-72 h), and 1.0 mg/kg/h (72-96

[Anesthetic management of a patient with dentato-rubro-pallido-luysian atrophy].

This report may be the first case of perioperative management in a patient with dentato-rubro-pallido-luysian atrophy (DRPLA). A 19-yr-old man with DRPLA was admitted for recurrent aspiration pneumonia. Medications included phenobarbital, carbamazepine and clonazepam. The planned surgical procedure was tracheostomy and tracheoesophageal separation. He recovered from these procedures uneventfully. Two months later, however, he was readmitted for plastic surgery of narrowing tracheostoma. All of these procedures were performed under general anesthesia, induced using propofol (or thiamylal) and v

Influence of barbiturate anesthetic induction on circulatory responses to morphine.

Circulatory responses to morphine (1 mg/kg, 10 mg/min) following a barbiturate anesthetic induction (thiamylal 4 mg/kg) were determined in 7 patients with coronary artery disease. Compared with awake measurements, the mean arterial pressure and cardiac index were decreased 16 and 42 percent, respectively (p less than 0.05) and systemic vascular resistance increased 35% (p less than 0.05) at the end of morphine infusion. The same dose of morphine, but without prior barbiturate, has been previously reported not to significantly alter these measurements. Therefore, the assumption that morphine is

Contractile responses of guinea pig trachea to oxybarbiturates and thiobarbiturates.

To determine what mechanisms are involved in barbiturate-induced tracheal constriction and whether a relationship exists between barbiturate structure and the ability of the barbiturate to induce constriction, we compared the effects of thiamylal, thiopental, methohexital, pentobarbital, and phenobarbital at increasing airway tone in an intact guinea pig tracheal preparation in the presence and absence of cyclooxygenase and thromboxane synthetase inhibition. Whole tracheas were suspended between two cannulas in 50-ml tissue baths and perfused at a constant flow rate with Krebs-Henseleit soluti

Cardiovascular and respiratory effects of three rapidly acting barbiturates in dogs.

The cardiovascular and respiratory effects of 3 rapidly acting barbiturates, thiopental sodium, thiamylal sodium, and methohexital sodium, were studied in dogs from completion of injection until 12.5 minutes after injection. The doses administered were 19.4 mg of thiopental/kg of body weight, 18.4 mg of thiamylal/kg, and 9.7 mg of methohexital/kg, which were chosen as equipotent doses necessary to inhibit the laryngoscopic reflex in 50% of the population. To determine the cardiovascular and respiratory effects for each drug, the values at each measurement time following injection were compared

Pronounced QT Prolongation During General Anesthesia in a Child with Left Ventricular Noncompaction Cardiomyopathy: A Case Report.

We report the case of an 8-year-old boy with left ventricular noncompaction cardiomyopathy (LVNC) and QT prolongation who experienced further prolongation of the QTc during general anesthesia for extraction of a maxillary mesiodens. Pronounced prolongation of the QTc was observed after induction of general anesthesia with thiamylal and during emergence. No notable fluctuations in blood pressure, heart rate, and estimated continuous cardiac output were observed. We considered it likely that the QT prolongation was triggered by thiamylal and increased sympathetic nervous system activity. During

[Barbiturate therapy in 16 cases with intracranial lesion with special reference to the indication and limitation].

The effects and indications of barbiturate therapy for brain protection, and prevention and reduction of the intracranial hypertension were investigated using an ultrashort acting barbiturate, thiamylal, in sixteen cases with intracranial lesions. Final outcome of the treatment revealed 8 good recoveries which were actively administered thiamylal during operation or immediately after. On the other hand, four cases, whose intracranial pressures (ICPS) of over 40 mmHg could not be controlled suffered brain death. Barbiturate therapy was not effective for brain protection of primary damaged lesio

The intravenous anesthetics barbiturates inhibit hypoxia-inducible factor 1 activation.

Hypoxia-inducible factor 1 (HIF-1) is a master transcription factor of hypoxia-induced gene expression. Anesthetics and perioperative drugs have been reported to affect HIF-1 activity. However, the effect of barbiturates on HIF-1 activity has not been reported. In this study, we investigated the effect of thiopental and thiamylal on HIF-1 activity using the neuronal SH-SY5Y cells, the non-neuronal HEK293 cells, and the macrophage-differentiated THP-1 cells. Cells were exposed to 20% or 1% O(2) conditions with or without thiopental or thiamylal treatment. The cell lysate were subjected to Weste

Short-acting barbiturate sedation: effect on arterial pH and PaCO2 in children.

One hundred and fifteen unpremedicated children, induced with thiamylal, a short-acting thiobarbiturate, administered rectally (25 mg.kg-1 body wt) or intravenously (6 mg.kg-1) had arterial pH of 7.36 +/- 0.03 and PaCO2 of 40 +/- 4 mmHg. In 22 children over two years of age, the use of fentanyl (1.2 +/- 0.7 microgram.kg-1) in addition to the thiamylal did not change blood gas data compared to those children over two years receiving only barbiturates (pH 7.36 vs 7.36, PaCO2 41 vs 40 mmHg). Children with cyanotic congenital heart disease showed similar pH and PaCO2 to acyanotic children followin

Postanesthetic hepatic injury in rats: influence of alterations in hepatic blood flow, surgery, and anesthesia time.

Hepatic injury was measured in enzyme-induced rats following ligation of the hepatic artery performed under anesthesia with thiamylal, halothane, enflurane, or isoflurane. The influence of surgical site was determined by performing upper abdominal (sham ligation), lower abdominal, and peripheral surgery on rats anesthetized with halothane. The effect of anesthesia time was evaluated by continuing halothane anesthesia for 1, 2, or 4 hours after upper abdominal (sham ligation) surgery. Centrilobular necrosis did not occur in enzyme-induced rats anesthetized with thiamylal, isoflurane, or enflura

Effects of intravenous anesthetics on the contraction and response of rat trachea to phosphatidylinositol.

Some intravenous anaesthetics have significant effects on the tonicity of airway smooth muscle. There are evidences for a direct relationship between airway smooth muscle contraction and phosphatidylinositol (PI) response. The aims of this study were to clarify 1) the effects of anaesthetics on the cholinergically induced contraction, and 2) the relationship between the effects on the PI response and the contraction. Rat tracheal rings or slices were incubated in Krebs-Henseleit solution containing 5.5 microM carbachol (CCh) in the presence of varying doses of anaesthetics. Thiamylal, fentanyl

Barbiturate Induction for the Prevention of Emergence Agitation after Pediatric Sevoflurane Anesthesia.

OBJECTIVES: Emergence agitation (EA) is a common and troublesome problem in pediatric patients recovering from general anesthesia. The incidence of EA is reportedly higher after general anesthesia maintained with sevoflurane, a popular inhalational anesthetic agent for pediatric patients. We conducted this prospective, randomized, double-blind study to test the effect of an intravenous ultra-short-acting barbiturate, thiamylal, administered during induction of general anesthesia on the incidence and severity of EA in pediatric patients recovering from Sevoflurane anesthesia. METHODS: Fifty-fo
