Human cortical responses to water in the mouth, and the effects of thirst.

In an event-related functional magnetic resonance imaging (fMRI) study in humans it was shown, first, that water produces activations in cortical taste areas (in particular the frontal operculum/anterior insula which is the primate primary taste cortex, and the caudal orbitofrontal/secondary taste cortex) comparable to those produced by the prototypical tastants salt and glucose. Second, the activations in the frontal operculum/anterior insula produced by water when thirsty were still as large after the subjects had consumed water to satiety. Third, in contrast, the responses to water in the c…

Determinants of the sensation of thirst in terminally ill cancer patients.

While a sensation of thirst causes severe distress for a certain proportion of cancer patients in the terminal stage, the factors contributing to this symptom have not been established. To clarify the association between sensation of thirst and medical factors, especially dehydration, a cross-sectional observational study was performed on terminally ill cancer patients receiving inpatient hospice care. On admission to a palliative care unit, 88 consecutive patients underwent blood sampling and were requested to rate the intensity of thirst on a visual analogue scale (VAS). Physicians prospecti…

Clinical validation of the nursing diagnostic proposition perioperative thirst.

OBJECTIVE: to verify the clinical validity of the proposition of a new nursing diagnosis called perioperative thirst, based on the diagnostic accuracy of its clinical indicators, including the magnitude of effect of its etiological factors. METHOD: clinical diagnostic validation study with a total of 150 surgical patients at a university hospital. Sociodemographic variables and clinical indicators related to thirst were collected. The latent class analysis technique was used. RESULTS: two models of latent classes were proposed for the defining characteristics. The model adjusted preoperative…

In utero extracellular dehydration modifies thirst in neonatal rats.

The main aim of this study was to investigate the effect of maternal extracellular dehydration during pregnancy in rats on the development of thirst mechanisms in the offspring. Pregnant rats underwent three episodes of extracellular dehydration induced by injecting s.c. 15ml/kg b.w. of a 20% wt/vol solution of polyethylene glycol (PEG) in saline. The treatment given on days 14, 17 and 20 postconception is thought to induce endocrine and natriophilic responses similar to those elicited by vomiting. The offspring were tested for their responses to three different thirst stimuli at 2, 4 and 6 da…

Thirst and vasopressin secretion counteract dehydration in newborn infants.

OBJECTIVE AND STUDY DESIGN: Our goal was to study the water balance in healthy breast-fed infants (n = 139) during their first 5 days, by cr … ponse to fluid hypertonicity. This state also affects feeding behavior, perhaps as an expression of thirst. It is likely that hormone release is also stimulated in parallel with a weight reduction of less than 10%, because it is also accompanied by a hyperosmotic state.

Thirst in diabetes insipidus: clinical relevance of quantitative assessment.

Patients with cranial diabetes insipidus are unable to concentrate urine, and depend on thirst and water intake to prevent hypertonic dehydration. Using a visual analogue scale (0-10 cm) we studied osmotically stimulated thirst induced by hypertonic saline infusion in 15 patients with diabetes insipidus and 15 healthy controls. Plasma osmolality in the patients rose from 292 +/- 1 to 316 +/- 1 mOsm/kg (p less than 0.001), and 13 patients showed a progressive rise in thirst ratings (1.4 +/- 0.4 to 8.1 +/- 0.3 cm, p less than 0.001) with abolition of thirst by drinking, in a similar fashion to c…

Implementing a Safety Protocol for Thirst Management to Improve Postoperative Thirst Using the Iowa Model of Evidence-Based Practice: A Propensity Score-Matched Evaluation.

AIM: This study aimed to (1) implement a Safety Protocol of Thirst Management (SPTM) as an evidence-based practice for quenching postoperative thirst and (2) evaluate its effectiveness using a comparative pre-and-post induction design. DESIGN: A quasi-experimental study using propensity scored matching. METHOD: Guided by the Iowa Model, the SPTM was implemented at a tertiary medical centre in Taiwan in 2023. Outcomes were compared between adult surgical patients admitted in 2023 (post-induction) and those admitted prior (pre-induction). Data on thirst and pain intensity, body temperature, an…

Thirst in Patients With Heart Failure in Sweden, the Netherlands, and Japan.

BACKGROUND: Thirst is a distressing symptom and influences quality of life of patients with heart failure (HF). Knowledge about thirst in HF is insufficient; therefore, the aim of this study was to describe factors related to thirst, self-reported reasons for thirst, and interventions to relieve thirst in 3 different countries. METHODS: A cross-sectional study was conducted in Sweden, the Netherlands, and Japan. Patients were recruited at the HF clinic or during HF hospitalization. Thirst was assessed by a visual analog scale (0-100); reasons for thirst and interventions to relieve thirst wer…

Thirst and sodium appetite in rats with experimental nephrotic syndrome.

Nephrotic syndrome is a renal disease accompanied by abnormal body fluid balance. The present experiments investigated the role of behavioral mechanisms in contributing to disordered fluid homeostasis in rats with experimentally-induced nephrotic syndrome. The studies examined water and sodium ingestion under ad libitum conditions and in response to dehydration-related challenges in rats made nephrotic by treatment with the antibiotic, adriamycin. Rats with nephrotic syndrome had greater ad libitum water intakes beginning 3 weeks after treatment, but daily sodium (0.3M NaCl) intakes were not a

In utero development of fetal thirst and appetite: potential for programming.

Thirst and appetite-mediated ingestive behavior develop and are likely programmed in utero, thus preparing for newborn and adult ingestive behavior. Fetal swallowing activity is markedly different from that of the adult, as spontaneous fetal swallowing occurs at a markedly (six-fold) higher rate compared with spontaneous adult drinking activity. This high rate of fetal swallowing is critical for the regulation of amniotic fluid volume and the development of the fetal gastrointestinal tract. Disordered fetal swallowing has been associated with both a decrease (oligohydramnios) and increase (pol…

Vasopressin and thirst in patients with posterior pituitary ectopia and hypopituitarism.

OBJECTIVE: Partial diabetes insipidus has been documented in patients with congenital hypopituitarism and posterior pituitary ectopia, some cases being clinically silent except for enuresis. The objective of our study was to evaluate vasopressin (AVP) secretion and thirst appreciation in hypopituitary patients with posterior pituitary ectopia. PATIENTS: Twelve males and three females, aged between 13 and 38 years (median 19 years). Eleven had multiple pituitary deficiencies, adequately replaced at the time of the study, and four were only growth hormone deficient. None of the patients suffere…

[Marked hypernatremia in suprasellar germinoma lacking a sense of thirst].

We here report a 17-year-old high school boy having suprasellar germinoma who presented marked hypernatremia probably due to damages of both the osmoregulation and thirst centers. He was in good health until July, 1996, when he noticed slight general malaise and complained of dryness of the mouth, but without polyuria. He was found to have hypernatremia of mild degree (serum Na 151 mEq/l), but dropped out from the follow-up. In April, 1997, he was admitted to our hospital with complaints of general malaise and weakness of the upper and lower extremities. Serum Na was high at 202 mEq/l with a p…

Intracerebroventricular glycerol: a potent inhibitor of ADH-release and thirst.

Infusions of isotonic or hypertonic (0.3 or 0.5 M) glycerol into the lateral cerebral ventricle (60 min, 0.02 ml/min) of non-hydrated goats invariably induced a conspicuous and sustained water diuresis. Corresponding infusions of 0.3 M glycerol/0.16 M NaCl were almost equally efficient in this respect. A more short-lasting and less pronounced water diuresis was obtained in response to equivalent infusions of pure d-glucose, and the response to 0.3 M glucose/0.16 M NaCl was variable. Intravenous injections of vasopressin blocked the glucose-induced diuresis, but only postponed the glycerol-indu
