[Surgical treatment of penetrating chest wounds with injuries to the internal thoracic and intercostal arteries].

Injuries of the internal thoracic and intercostal arteries are observed rather frequently in penetrating wounds of the thorax. In intrapleural hemorrhage the only correct tactics is the urgent surgical intervention. Its efficiency is dependent on the terms of admittance of the patient to a specialized clinic, preparedness of different services (reanimation, x-ray analysis, operation room) for urgent help, on qualification of surgeons and careful management during the postoperative period.

Unique collateralisation between the left internal thoracic artery and the coeliac artery.

Collateralisation around stenotic or occlusive arterial disease is a commonly observed adaptation for maintenance of arterial perfusion. It occurs through pathways of well-understood embryological origin. Examples include Winslow's pathway (internal thoracic artery to iliac artery via the epigastric arteries) and the arc of Riolan (between superior and inferior mesenteric arteries). In this article, we describe a pathway of collateralisation around stenotic disease of the coeliac and superior mesenteric arteries in a 76-year-old man with an abdominal aortic aneurysm. The collateral pathway was

Bilateral internal thoracic artery grafting for peripheral arterial disease patients.

BACKGROUND: Peripheral arterial disease (PAD) is known to have a poor prognosis. Some reports have also described poor late results after coronary artery bypass grafting (CABG). However, there have been few reports about whether bilateral internal thoracic artery grafting improves the long-term survival of PAD patients after CABG. We performed this study to clarify whether or not this is the case. METHODS: One hundred and thirty-six PAD patients who underwent elective CABG with two or more grafts were enrolled in this study. Patients were divided into two groups, 71 patients in the bilateral

Is internal thoracic artery grafting suitable for a moderately stenotic coronary artery?

Grafting an internal thoracic artery to a coronary artery with moderate stenosis remains controversial. Competitive flow from the native coronary artery has been proposed as the cause of distal narrowing and ultimate failure of the internal thoracic artery graft. We investigated intraoperative phasic blood flow in internal thoracic arteries grafted to coronary arteries with various degrees of stenosis and the influence of stenosis on postoperative angiographic findings. One hundred patients who underwent coronary artery bypass grafting of an internal thoracic artery to the left anterior descen…

Longitudinal histomechanical heterogeneity of the internal thoracic artery.

The internal thoracic artery (ITA) is the principal choice for coronary artery bypass grafting (CABG) due to its mechanical compatibility, histological composition, anti-thrombogenic lumen, and single anastomotic junction. Originating at the subclavian artery, traversing the thoracic cavity, and terminating at the superior epigastric and musculophrenic bifurcation, bilateral ITAs follow a protracted circuitous pathway. The physiological hemodynamics, anatomical configuration, and perivascular changes that occur throughout this length influence the tissue's microstructure and gross mechanical p

Contemporary Management of Arterial Thoracic Outlet Syndrome.

BACKGROUND: Arterial thoracic outlet syndrome (aTOS) is characterized by compression of the subclavian artery as it exits the thoracic girdle. Chronic and repetitive compression leads to several degrees of arterial wall damage. The treatment is varied and depends on the presentation severity. This study aimed to describe the contemporary experience in managing arterial thoracic outlet syndrome at a large tertiary hospital. METHODS: We conducted a single-institution retrospective review of aTOS cases from January 2009 to January 2020. Demographic data, clinical presentation, medical images, op

True Aneurysm of the Left Internal Thoracic Artery.

Aneurysms of the internal thoracic artery (ITA) are rare and can have many etiologies. Hyperflow is an exceptional etiology. We report the case of a 56-year-old woman who presented with a stress-induced ischemia of the left arm. Computed tomography angiography showed occlusion of the subclavian artery and a true aneurysm of the ITA. The ITA aneurysm was resected without restoration of the ITA continuity and a carotid-subclavian bypass was performed. Pathological examination of the aneurysm sac was not specific.

Vertebral artery variations in thoracic aortic patients.

OBJECTIVES: Arterial variation is common in the vertebral artery, and simple occlusion of the left subclavian artery may result in brain infarction, especially when it terminates in the posterior inferior cerebellar artery (PICA). We report the results of preoperative vertebral artery evaluation by magnetic resonance angiography (MRA) and its impact upon operative strategy. METHODS: Among the 214 patients who underwent thoracic aortic surgery from 2009 through 2012, 159 patients with preoperative MRA were retrospectively analysed. Patients' age ranged from 35 to 88 (median 72), 122 were male

[Aneurysms of the subclavian artery in thoracic outlet syndrome].

The true subclavian artery aneurysm is a rare but dangerous complication of TOS. During a 20 years time period we observed 18 patients with subclavian artery aneurysms. 12 of these patients showed cervical ribs. Half of the patients (n = 9) presented with a critical ischemia of an upper extremity, nine patients were operated on electively. The acute ischemia makes the desobliteration of the outflow tract necessary first. The next step ist the transaxillary exarticulation of the first rib and the resection of the aneurysm. The reconstruction of the subclavian artery is performed by using autolo

[Changes in luminal diameter of internal thoracic arteries five years after coronary artery bypass grafting].

Five years after coronary artery bypass grafting, 23 internal thoracic arteries (ITAs) were reexamined angiographically, and their luminal diameters were compared with those of one month after the operation. Proximal diameter, just below the subclavian artery, and distal diameter, just above the anstomosis, were measured. One month after surgery, these were 2.38 +/- 0.48 mm and 1.69 +/- 0.39 mm, respectively. After five years, the proximal diameter had not changed significantly (2.48 +/- 0.47 mm), but the distal diameter had increased to 1.96 +/- 0.58 mm (p < 0.05). Especially when there was n…

Evaluation and Management of Arterial Thoracic Outlet Syndrome.

Arterial thoracic outlet syndrome is rare and may be associated with a bony anomaly. Patient presentation can range from mild arm discoloration and claudication to severe limb-threatening ischemia. For patients with subclavian artery dilation without secondary complications, thoracic outlet decompression and arterial surveillance is sufficient. Patients with subclavian artery aneurysms or distal embolization require decompression with reconstruction or thromboembolectomy and distal bypass respectively.

MRI measurements of the thoracic aorta and pulmonary artery.

INTRODUCTION: The purpose of this study was to obtain a range of normal measurements of the adult thoracic aorta and main pulmonary artery using cardiac MRI, and to assess agreement between measurements made on ECG-gated two-dimensional (2D) breath held steady-state-free precession (SSFP), and three-dimensional (3D) breath held SSFP image acquisitions. METHODS: Forty-nine normal volunteers underwent cardiac MRI using a 1.5T system. Two independent examiners measured the ascending aorta, aortic arch, descending thoracic aorta and main pulmonary artery in pre-defined locations. RESULTS: Overal

Bypass surgery in arterial thoracic outlet syndrome.

Arterial thoracic outlet syndrome (TOS) causes ischemic symptoms; it is the rarest type, occurring in 5% of all TOS cases. This paper is a case report of a 38-year-old male patient diagnosed with arterial TOS, displaying symptoms of acute critical limb ischemia caused by thromboembolism. Brachial artery of the patient has been diffusely damaged by repeated occurrence of thromboembolism. It was thought to be not enough only decompression of subclavian artery to relieve the symptoms of hand ischemia; therefore, bypass surgery using reversed great saphenous vein was performed.

Lateral thoracic artery embolization in cystic fibrosis.

We report the embolization of an aberrant origin of haemoptysis - from the internal branch of the lateral thoracic artery - in a patient affected by cystic fibrosis. The technical implications for embolotheraphy in case of an aberrant origin of a haemorrhage are emphasized. Many different systemic arteries may contribute to the blood supply of the lung and many connections may exist between the systemic, bronchial and pulmonary circulations. The presence of non-bronchial systemic arteries supply should be investigated when inconclusive findings are seen during bronchial artery embolization for

Diagnosis and management of arterial compression at the thoracic outlet.

Neurovascular compression syndromes at the thoracic outlet generally present with predominantly arterial, venous, or neurogenic symptoms. The arterial abnormalities produce unique problems in diagnosis and management, and usually affect young, otherwise healthy, active individuals. Between 1984 and 1995 23 patients presented to our facility, with acute symptoms of arterial occlusion or embolization, found to be originating from the axillosubclavian arterial segment. The group comprised 14 females and nine males, ranging from 15 to 74 years, with an average age of 37 years. There were seven com

[Arterial thoracic outlet syndrome and diagnostic angioresonance].

INTRODUCTION: The anatomy of the upper cervical region, in the presence of structural anomalies favours the compression of the brachial plexus and/or subclavian vessels, giving rise to the thoracic inlet syndrome (SATS). Neurological involvement is more common than vascular involvement (95 and 5% respectively); the latter is known as the arterial SATS. The forms of clinical presentation of arterial SATS are very variable and have different prognoses: it may present as acute distal ischaemia with a variable course, peripheral embolism or as a non-serious condition such as Raynaud's phenomenon.

[The arterial supply of the thoracic and lumbar spine in newborns].

The major arteries supplying the vertebral column are definite. The branching arteries of these trunks are variable. The posterior intercostal arteries and the lumbar arteries have an ascending, descending or horizontal course. In accordance with the literature, we found large, arched anastomoses between the rami ascendentes and the rami descendentes arising from the rami canalis vertebralis anteriores on the dorsal plane of the vertebral bodies. There are also nearly horizontal junctions between the rr. ascendentes of both sides. We found junctions on the front of the thoracolumbar vertebral
