The practice of thoracic surgery in Canada.

The objective of the consensus conference of the Canadian Association of Thoracic Surgeons (CATS) was to define the scope of thoracic surgery practice in Canada, to develop standards of practice, to define training and resource requirements for the practice of thoracic surgery in Canada and to determine appropriate waiting times for thoracic surgery care. A meeting of the CATS membership was held in September 2001 to address issues facing thoracic surgeons practising in Canada. The discussion was facilitated by an expert panel of surgeons and supplemented by a survey. At the end of the meeting

[Laparoscopic surgery of a thoracic stomach].

We report the case of a 54-year-old patient with up-side-down-stomach with recurrent gastric bleeding. Reposition of the stomach, closure of the diaphragmatic opening with interrupted silk sutures, fundophrenicopexy and corpopexy to the anterior abdominal wall was done laparoscopically. Because the patient presented no reflux symptoms, but hypomotility of the oesophagus, no fundoplication was done. To prevent recurrence of the hernia a polypropylene mesh was fixed over the hiatus.

Reoperative surgery on the thoracic aorta.

OBJECTIVE: The objective of our study was to report our hospital and long-term results after reinterventions on the thoracic aorta. METHODS: Between 1986 and 2011, 224 reoperations on the proximal thoracic aorta after previous aortic surgery were performed in our institution. The number of reinterventions quadrupled during the course of the study period. Mean patient age was 58.1 years, and 174 patients (77.7%) were male. An urgent/emergency operation was performed in 39 patients (17.4%). Indications for surgery included degenerative and chronic postdissection aneurysm (n = 166), false aneury

American Association for Thoracic Surgery Quality Gateway Procedural Outcomes Models for General Thoracic Surgery Quality Assurance.

OBJECTIVE: To develop quality assurance models for adverse procedural events after lung resection and esophagectomy for cancer. METHODS: Using procedural outcomes after 16,430 lung resections and 3,068 esophagectomies for cancer at 9 hospitals in 7 hospital systems, we developed 5 individual and 2 composite outcome models from preoperative information. Random forest quantile classification for imbalanced data with model-free variable priority screening identified predictors. Out-of-bag prediction provided regularized estimates and internal validation. Hospital-holdout analysis assessed instit

Camera-based Respiratory Imaging for Thoracic Asymmetry in Thoracic Surgery Patients.

The current tool for assessing thoracic asymmetry of thoracic surgery patients is inappropriate for timely or frequent clinical routines due to its dependency on empirical physical examinations or specialized machines. This study investigates the camera-based respiratory imaging for screening thoracic asymmetry, in an intelligent and convenient way. The respiratory heatmaps are generated based on the respiratory magnitudes, phases and angles extracted from the chest video, and bilateral chest region of interest are compared statistically. Due to the variability of chest respiratory direction,

Digital Thoracic Drainage System: A New Tool For Pediatric Thoracic Surgery.

INTRODUCTION: Digital thoracic drainage systems usage in pediatrics is underreported, making current recommen- dations based on those for adults. We aim to review our experience on the use of this digital system and to evaluate the adequacy of those recommendations. METHODS: Retrospective analysis of patients in whom digital thoracic drainage system was used. All patients were submitted to thoracoscopic bleb/apical lung resection for primary spontaneous pneumothorax. Initially patients were man- aged using the few existing recommendations for children, but after 2 unsuccessful cases we change

Fast track pediatric thoracic surgery: Toward day-case surgery?

PURPOSE: Thoracoscopic lung resection for congenital pulmonary airway malformation (CPAM) is a safe technique for children. Our purpose was to evaluate the feasibility of a fast-track protocol in such cases. METHODS: From September 2007 to May 2016, 101 patients underwent a thoracoscopic pulmonary resection of which 83 for CPAM (lobectomy, wedge resection or sequestrectomy). We retrospectively reviewed the characteristics of surgical procedure, postoperative management and complications through three time periods (September 2007-December 2009: n=14, January 2010-March 2013: n=30, April 2013-M

[Thoracic lymphadenectomy in video-assisted mini-invasive thoracic surgery. First experience].

The Authors report their experience in the thoracoscopic surgical treatment of neurovascular abnormalities of the upper extremities. The experience acquired in the last two years with mini-invasive video-assisted thoracic surgery of the pleura and lung forced the Authors to perform thoracic sympathectomy following the same approach. In this way 4 thoracic sympathectomies were performed in 3 patients. Two of them presented Raynaud's phenomenon, and one was affected by bilateral idiophatic upper limb hyperhidrosis. The operation is performed thanks to 3 12mm Thoracoports that allow the introduct

Patient-Reported Outcomes in Thoracic Surgery.

Patient-reported outcomes (PROs) provide critical insight into recovery and survivorship after thoracic surgery. This article outlines a framework for implementing PROs for thoracic surgery patients, from preoperative baseline assessment through perioperative monitoring and long-term follow-up. Emphasis is placed on lung and esophageal surgery, where symptom burden and functional impairment significantly influence quality of life and overall outcomes. Integration of PROs into routine clinical workflows supports shared decision-making, guides perioperative management, and enables patient-center

Cardiac Rehabilitation After Thoracic Aortic Surgery.

Thoracic aortic diseases, including aortic aneurysm and aortic dissection, are disorders that frequently require cardiac surgical intervention. Current clinical practice guidelines recommend cardiac rehabilitation for patients after thoracic aortic surgery. However, the evidence to support these recommendations remains limited, and there is a notable absence of individualized approaches for resuming regular exercise. In this narrative review the literature on cardiovascular rehabilitation after thoracic aortic surgery for inherited and acquired thoracic aortic disease is examined, a clinical c

Robotic thoracic surgery for neurogenic tumors.

OBJECTIVE: Thoracic neurogenic tumors usually present as benign nerve sheath tumors that can be resected via transthoracic or posterior approaches, depending on the anatomical location. Robot-assisted thoracic surgery (RATS) is increasingly being used for the transthoracic approach, but evidence is very limited. The authors initiated the current study to evaluate the efficacy and safety of RATS for thoracic neurogenic tumors. METHODS: This retrospective study is based on a prospectively created database that includes all RATS surgeries between 2018 and 2023. All patients with histologically c

Modern fluid management in thoracic surgery.

PURPOSE OF REVIEW: To provide an approach to perioperative fluid management for lung resection patients that incorporates the entire patient pathway in the context of international guidelines on enhanced recovery after surgery (ERAS). RECENT FINDINGS: The concern with intraoperative fluid management is that giving too little or too much fluid is associated with worse outcomes after lung resection. However, it has not emerged as a key care element in thoracic ERAS programs probably due to the influence of other ERAS elements. Carbohydrate loading 2 h before surgery and the allowance of water u

Coagulopathy and Anticoagulation During Thoracic Surgery.

Coagulopathy and bleeding in thoracic surgery may be compounded by the chronic use of anticoagulants and antiplatelet agents. Timely preoperative cessation and postoperative resumption of these antithrombotic drugs are critical in reducing the risks of perioperative major bleeding and thromboembolism. This article describes the various strategies for the optimal perioperative management of antithrombotics based on individual assessment of each patient and the most recent multisociety guidelines.

Assessment of paediatric thoracic robotic surgery.

OBJECTIVES: Many studies have reported that robotic-assisted surgery is safe and feasible for paediatric cases. However, very few paediatric thoracic robotic cases have been described. The aim of this study was to share our preliminary experience with robot-assisted thoracic surgery (RATS). METHODS: We reviewed our first, consecutive thoracic robotic procedures between January 2008 and December 2013. Data describing the perioperative and intraoperative periods were prospectively collected in two surgical paediatric centres and then retrospectively analysed. Operation time, completion rate, le

Herpes myelitis after thoracic spine surgery.

Herpes simplex or herpes zoster reactivation after spinal surgery is rarely reported. This case report and review of the literature describes patients in whom this reactivation occurs to clarify the diagnosis and management. In addition to reporting their case, the authors reviewed case reports and series published between 1980 and 2012 found through a PubMed search. Herpes reactivation is generally confined to a vesicular rash that can be treated with acyclovir. However, occasional dissemination has occurred and has led to myelitis or encephalitis. Atypical presentations led to delays in diag

Video-assisted thoracic surgery for bronchiectasis.

BACKGROUND: Bronchiectasis is one of the common diseases diagnosed in the world. No major improvement for the treatment approaches and limited efficacy promote a big challenge for management of this disease. Video-assisted thoracoscopic surgery (VATS) offers a new choice for the treatment of bronchiectasis. The purpose of this study was to present our experience of VATS for bronchiectasis and to compare this with thoracotomy in our institution. METHODS: We reviewed the medical records of patients who underwent VATS lobectomy and general lobectomy for bronchiectasis between January 2005 and De
