[Neonatal arterial ischemic stroke: Which thrombotic biological risk factors to investigate and which practical consequences?].

All biological risk factors that have been previously identified to increase the risk of thrombosis in adults, have also been studied in neonates with arterial Ischemic Stroke (NAIS), but most studies were retrospective and included relatively low numbers of affected children. We therefore could not suggest recommendations with a strong level of evidence and only expert proposals potentially useful for clinical practice will be presented in this text. Despite these limitations, the extensive analysis of published data supported that factor V Leiden (FVL) and increased levels of Lp(a) could be

Occipital Stroke Due to Non-bacterial Thrombotic Endocarditis as the Initial Manifestation of Endometrioid Ovarian Cancer.

Ischemic stroke is a rare initial manifestation of malignancy. We report the case of a 46-year-old female who presented with an embolic stroke, subsequently found to be due to endometrioid ovarian carcinoma. She presented to our clinic with sudden-onset bilateral visual blurring and syncope. Neurological examination was unremarkable. Investigations revealed iron-deficiency anemia and elevated C-reactive protein. Brain MRI showed multiple cardioembolic infarcts in the bilateral frontoparietal lobes and cerebellum. transesophageal echocardiogram (TEE) revealed a left atrial mass attached to the

Impact of a modified anti-thrombotic guideline on stroke in children supported with a pediatric ventricular assist device.

BACKGROUND: Stroke is the most feared complication associated with the Berlin Heart EXCOR pediatric ventricular assist device (VAD), the most commonly used VAD in children, and affects 1 in 3 children. We sought to determine whether a modified anti-thrombotic guideline, involving more intense platelet inhibition and less reliance on platelet function testing, is associated with a lower incidence of stroke. METHODS: All children supported with the EXCOR at Stanford from 2009 to 2014 were divided into 2 cohorts based on the primary anti-thrombotic guideline used to prevent pump thrombosis: (1)

Thrombotic molecular markers after intravenous thrombolysis are associated with early neurological deterioration in acute ischemic stroke.

Early neurological deterioration (END) is a frequent and serious complication after intravenous thrombolysis in acute ischemic stroke (AIS), yet the clinical relevance of thrombotic molecular markers related to coagulation, fibrinolysis, and endothelial dysfunction remains incompletely understood. In this retrospective cohort study, 91 patients with AIS treated with intravenous thrombolysis were included between November 2022 and February 2025. Plasma levels of thrombin-antithrombin complex (TAT), plasmin inhibitor-plasmin complex (PIC), thrombomodulin (TM), and tissue-type plasminogen activat

Acute ischemic stroke and vaccine-induced immune thrombotic thrombocytopenia post COVID-19 vaccination; a systematic review.

INTRODUCTION: One of the rare but potentially serious side effects of COVID-19 vaccination is arterial and venous thrombosis. Acute ischemic stroke (AIS) cases have been reported post COVID-19 vaccination. Herein, we systematically reviewed the reported cases of AIS after COVID-19 vaccination. METHOD: This systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline. We searched PubMed and Scopus until April 14, 2022 to find studies that reported AIS post COVID-19 vaccination. RESULTS: We found 447 articles. From those

Venous Thrombotic and Thromboembolic Considerations in Stroke: An Expert Meeting Report on Future Directions for Clinical Trials.

BACKGROUND: Venous thrombosis and venous thromboembolism (VTE) are important causes and complications of stroke. They may complicate acute stroke as deep venous thrombosis or pulmonary embolism (DVT/PE), contribute causally to ischemic stroke through paradoxical embolism from a patent foramen ovale (PFO), or present directly as cerebral venous thrombosis (CVT). Despite shared pathophysiological features and overlapping antithrombotic strategies, these conditions are typically considered separately, fragmenting evidence and limiting the exchange of concepts. PURPOSE: We therefore convened expe

Suppressing the Inflammatory Prostaglandin Signaling after Thrombotic Stroke Ameliorates Ischemic Brain Injury and Facilitates Poststroke Recovery.

Acute cerebral ischemia is a leading cause of death and disability, particularly among old adults. The narrow therapeutic window and risk of hemorrhagic transformation largely limit patient eligibility for the current treatment. The neuroinflammatory signaling pathway involving the prostaglandin E2 (PGE2) receptor subtype EP2 has now been clarified to contribute to the secondary neurotoxicity following ischemic stroke. We previously demonstrated the feasibility of pharmacologically targeting EP2 for ischemic stroke using an EP2 antagonist in a mouse model of transient middle cerebral artery oc

Nonbacterial thrombotic endocarditis in antiphospholipid syndrome, presenting with severe mitral stenosis, heart failure, and stroke: case report.

BACKGROUND: Nonbacterial thrombotic endocarditis (NBTE) is a rare cardiac manifestation of antiphospholipid syndrome (APS) and may also occur in patients with malignancy. It can mimic infective endocarditis or rheumatic mitral valve disease and often results in valvular disease and embolic complications. CASE SUMMARY: A 39-year-old man with a 10-year history of primary APS presented with acute decompensated heart failure and left-sided weakness. Clinical examination revealed signs of biventricular failure, mid-diastolic and systolic murmurs, and neurological deficits. Transthoracic echocardio

Nonbacterial Thrombotic Endocarditis in a 78-Year-Old Female With Recurrent Acute Ischemic Stroke: A Case Report.

Nonbacterial thrombotic endocarditis (NBTE) is a rare but serious complication, particularly in patients with malignancies like acute myeloid leukemia (AML), where a hypercoagulable state increases the risk of embolic events. This case report describes a rare and complex presentation of marantic endocarditis in a 78-year-old female with relapsed AML. The uniqueness of this case lies in the intersection of a hypercoagulable state induced by AML and the resultant NBTE, leading to recurrent embolic strokes, despite oral anticoagulation. This case contributes to the scientific literature by highli

Congenital Thrombotic Thrombocytopenic Purpura Presenting as Recurrent Ischemic Stroke in a Young Adult: A Genetically Confirmed Case.

Thrombotic thrombocytopenic purpura (TTP) is a rare thrombotic microangiopathy that can present with neurological and renal involvement and may rarely manifest as acute ischemic stroke in young adults. We report the case of a 23-year-old male who presented with acute-onset right-sided hemiplegia and expressive aphasia, with a history of recurrent transient ischemic attacks and renal dysfunction. Laboratory evaluation revealed severe thrombocytopenia, microangiopathic hemolytic anemia with schistocytes, and markedly elevated renal parameters. Neuroimaging demonstrated an acute infarct predomina

A case of progressive hypertension preceding gemcitabine-associated thrombotic microangiopathy complicated by acute kidney injury and stroke.

Gemcitabine-associated thrombotic microangiopathy is being increasingly recognized as a serious complication of treatment. We report a normotensive patient who developed progressive hypertension after commencing gemcitabine therapy. She also developed subtle changes in her platelet count and serum creatinine months before her emergent presentation. Clinicians should be aware of new onset or worsening hypertension and 'mild' biochemical changes in gemcitabine-treated patients.

Stroke and Thrombotic Events Associated With Concomitant Use of Oral Anticoagulants and Antiepileptic Drugs in the United States and Japan.

INTRODUCTION: Direct oral anticoagulants (DOACs) are substrates of efflux P-glycoprotein (P-gp) transporters, and the hepatic enzyme cytochrome P450 plays an important role. Antiepileptic drugs (AEDs) may reduce absorption or increase the metabolism of DOACs, thereby reducing their antithrombotic efficacy. However, there is no evidence that concurrent use of metabolic inducers in patients taking DOACs is associated with an increased risk of stroke or thrombotic events. METHODS: We analyzed adverse event cases submitted to the Food and Drug Administration Adverse Event Reporting System (FAERS)

Inherited Thrombotic Thrombocytopenic Purpura Revealed by Recurrent Strokes in a Male Adult: Case Report and Literature Review.

Thrombotic thrombocytopenic purpura (TTP) is a thrombotic microangiopathy related to a severe deficiency of ADAMTS13 (a disintegrin and metalloprotease with thrombospondin type 1 repeats, member 13). In this article, we describe the first case of a young male adult suffering from a hereditary TTP revealed by recurrent strokes, relapsing despite antiplatelet and anticoagulant therapy. Because of the persistent moderate thrombocytopenia, plasmatic ADAMTS13 activity was investigated and was found lower than 5% in the absence of anti-ADAMTS13 IgG. Direct sequencing of ADAMTS13 gene led to the diag

Comparison of Prasugrel and Clopidogrel in Thrombotic Stroke Patients with Risk Factors for Ischemic Stroke Recurrence: An Integrated Analysis of PRASTRO-I, PRASTRO-II, and PRASTRO-III.

INTRODUCTION: Patients with stroke are at a high risk of recurrence, and although they receive antiplatelet therapies such as clopidogrel for secondary prevention of non-cardioembolic stroke, the recurrence rate remains high. Three phase 3 trials (PRASTRO-I/II/III) were conducted to determine the efficacy of prasugrel in preventing recurrent stroke. Here, we performed an integrated analysis of these studies to confirm the generalizability of the PRASTRO-III findings and to supplement the small sample size of the study. METHODS: Patients from PRASTRO-I, PRASTRO-II, and PRASTRO-III with ischemi

Systemic Lupus Erythematosus (SLE) and Thrombotic Thrombocytopenic Purpura (TTP) Presenting as Recurrent Strokes: A Case Report.

Thrombotic thrombocytopenic purpura (TTP) is a rare but potentially life-threatening condition characterized by fever, acute hemolysis, thrombocytopenia, renal dysfunction, and CNS dysfunction. A peripheral smear shows schistocytes because of microangiopathy. It is extremely rare for TTP and systemic lupus erythematosus (SLE) to coexist. This report details an Indian female patient's uncommon clinical presentation of TTP linked to SLE. A 52-year-old woman presented to the emergency department with complaints of altered sensorium and weakness of the right side of the body. She was initially eva
