Cervical thymic cyst as a cause of acute suppurative thyroiditis.

A case of acute suppurative thyroiditis following a perithyroidal abscess, which was thought to have resulted from infection of a cervical thymic cyst, is reported herein. The patient was an 8 year old asthmatic Japanese boy who originally presented with tender swelling of the left anterior neck in July, 1986. Although pharyngography could not clearly demonstrate the pyriform sinus fistula, a hypoechoic area around the left lobe of the thyroid gland was noted on ultrasonography. Incisional drainage revealed Streptococcus milleri. A diagnosis of acute suppurative thyroiditis was established, an

Acute Suppurative Thyroiditis Extending to Retropharyngeal Space: Report of 2 Cases.

Acute suppurative thyroiditis (AST) is a rare endocrine emergency, typically of bacterial origin, that can lead to significant morbidity and life-threatening complications. It is far less common than other inflammatory thyroid conditions, such as subacute thyroiditis, and may mimic noninfectious inflammatory disorders. Case 1 describes a 40-year-old man with a 5-day history of anterior neck swelling and odynophagia. Physical examination and imaging confirmed AST with retropharyngeal space involvement. He was treated with broad-spectrum intravenous antibiotics and thyroid abscess aspiration. Ca

A case of acute suppurative thyroiditis caused by Streptococcus suis infection.

INTRODUCTION: Acute suppurative thyroiditis (AST) is an uncommon but potentially life-threatening condition. The majority of AST cases are caused by bacterial infection. Streptococcus suis is a swine pathogen that mostly causes meningitis and septicemia. To date, AST caused by Streptococcus suis has not yet been reported. CASE REPORT: A 64-year-old Thai female presented with rapid enlargement of a preexisting large left thyroid nodule without compressive symptoms for one week. She had a fever and purulent discharge spontaneously drained from her fluctuated mass. Her thyroid function workup re

Acute spontaneous suppurative thyroiditis caused by Eikenella corrodens presented with thyrotoxicosis.

Acute suppurative thyroiditis is a very rare and life-threatening endocrine emergency. Thyrotoxicosis is a rare condition accompanying acute suppurative thyroiditis. While the majority of the cases in the literature are caused by different reasons, spontaneous development is very rare. We present a patient with acute suppurative thyroiditis who presented to our clinic with thyrotoxic findings, and we compared the case to the literature. A 31-year-old male patient was admitted to our clinic with a complaint of progressive neck pain, swelling and redness on midline neck, fever, and palpitations.

[Congenital pyriform fistula with acute suppurative thyroiditis as the initial presentation].

Objective: To summarize the clinical characteristics of congenital pyriform fistula with acute suppurative thyroiditis as the initial presentation. Method: A total of 71 patients with congenital pyriform sinus fistula were treated, of which 33 cases had acute suppurative thyroiditis as the first symptom. For the patients with congenital pyriform sinus fistula who have acute suppurative thyroiditis as the first symptom, infection should be controlled first. Full drainage should be done when necessary, and the lesion should be completely excised during the stable period. Result: All the patients

Acute suppurative thyroiditis caused by Streptococcus agalactiae infection: a case report.

Acute suppurative thyroiditis is a serious disease; therefore, its diagnosis in the acute phase is important. Fine needle aspiration biopsy of the thyroid gland plays a pivotal role in the diagnosis of acute suppurative thyroiditis. Appropriate culture technique and optimal imaging modalities are also important for its diagnosis.

A pediatric patient with acute suppurative thyroiditis caused by Eikenella corrodens.

A previously healthy 6-year-old boy had continuous fever for 6 days before admission to our hospital. His general condition was good except for pyrexia. The left lobe of the thyroid gland was swollen, red, hot, and tender, and neck movement was limited. The provisional diagnosis was upper respiratory tract infection. We demonstrated the existence of an orifice of the left piriform sinus by esophagography in this case, and made a diagnosis of acute suppurative thyroiditis caused by a piriform sinus fistula in the hypopharynx. The causative organisms of acute suppurative thyroiditis include Pept

[Atypical form of acute suppurative thyroiditis in pediatric patient: case report].

Acute thyroiditis is an unusual disease. In pediatric patients it is usually associated with congenital abnormalities towards the unilateral envelopment of the left thyroid lobe. In general, in the absence of preexisting thyroid disease, the thyroid function is most often normal. It is here described a case of a six-year-old girl who had presented a mass with flogistic signs in the anterior neck region for fifteen days, together with leukocytosis and increased levels of VSH and TSH. There was no evidence of previous thyroid disease and the cervical ultrasonography suggested thyroid abscess. In

Acute suppurative thyroiditis caused by Pasteurella multocida and associated with thyrotoxicosis.

Acute suppurative thyroiditis (AST) is an uncommon condition of the thyroid gland. Organisms of the staphylococcal and streptococcal species are the most commonly reported causative agents. Rarely, AST has been associated with transient hyperthyroidism. We report a unique case of AST that was caused by Pasteurella multocida and was associated with thyrotoxicosis in a previously healthy 51-year-old woman.

Impaired vocal cord mobility in the setting of acute suppurative thyroiditis.

BACKGROUND: Acute suppurative thyroiditis is an increasingly rare entity, accounting for less than 0.1% of thyroid surgery. This is the first report in the literature of acute thyroiditis causing a unilateral reversible vocal cord paralysis. METHODS: A 41-year-old woman was initially seen with a unilateral vocal cord paralysis resulting from acute suppurative thyroiditis. RESULTS: The vocal cord paralysis resolved following surgical drainage of the thyroid abscess and treatment with intravenous antibiotics. CONCLUSIONS: We present this case to emphasize the need for a complete and thorough

Acute suppurative thyroiditis: a ten-year review in a Taiwanese hospital.

12 patients with acute suppurative thyroiditis were treated over a 10-year period. Their ages ranged from 2-81 years. Eight of them had a pre-existing thyroid mass. A piriform sinus fistula was demonstrated in one of the 2 patients with a history of recurrent infections. Cultures of the pus taken from the infected glands grew Gram-positive streptococci in 4 patients, Gram-negative bacteria in 4, and anaerobic bacteria in 1; the other 3 were sterile. Both patients who grew Klebsiella pneumonia were diabetic. Most of the patients received drainage in addition to systemic antibiotics. Only 2 pati

[Congenital piriformo-thyroid fistula as a cause of recurrent suppurative thyroiditis].

A previously healthy five year old boy presented with suppurative, febrile thyroiditis of the left lobe with pronounced general and local signs of inflammation, normal thyroid function and lack of thyroid antibodies. Intravenous antibiotic therapy improved the condition quickly. Six and nine months later however recurrent left lobe thyroiditis occurred. Endoscopic examination showed an internal fistula from the left piriform sinus to the left thyroidal lobe which was cut out. The case report confirms previous communications, that recurrent suppurative thyroiditis is nearly always caused by a p

Piriform sinus fistula. A route of infection in acute suppurative thyroiditis.

Fifteen patients had acute suppurative thyroiditis resulting from infection through the left piriform sinus fistula. Characteristic clinical features included (1) onset in infancy or childhood, (2) left lobe involvement, and (3) frequent recurrence unless the fistula was extirpated completely. Fistulectomy in eight patients demonstrated three distinct courses of the fistula: lateral to, medial to, and penetrating the left thyroid lobe. Anatomical and histological findings suggest that the fistula is a remnant of the third pharyngeal pouch. We believe that the fistula is a common route of infec

Internal fistula as a route of infection in acute suppurative thyroiditis.

Seven cases of acute suppurative thyroiditis are described. Six patients had a recurrent painful swelling of the left anterior neck and one was seen at her first episode of the disease. A barium meal revealed a fistula originating from the apex of the left pyriform sinus in all cases. The fistula, a remnant of the fourth pharyngeal pouch, thus seems to be a common route of infection in acute suppurative thyroiditis, allowing bacterial infection to begin in the perithyroidal space and spread to the thyroid gland. Complete extirpation of the fistula is required for a permanent cure.

A case report of acute suppurative thyroiditis caused by Clostridium perfringens.

A 42-year-old woman inadvertently discovered a neck mass, which caused pain. Initially, she was treated with antibiotics at a local clinic; however, this treatment did not alleviate the symptoms. She visited the authors' outpatient clinic for further treatment and underwent thyroid ultrasonography, which revealed a mixed echo nodule. On day 4 after admission, surgery was performed to remove the diseased thyroid tissue and levofloxacin (0.4 g/day) was infused. Bacterial culture confirmed infection with Clostridium perfringens. Subsequently, the treatment was switched to ceftriaxone sodium (2 g/

Acute suppurative thyroiditis seeded from infective endocarditis and intravenous drug use.

Acute suppurative thyroiditis (AST) is a rare infection of the thyroid gland, and most patients are euthyroid upon presentation. We present an interesting case of a 42-year-old man with a history of intravenous drug use (IVDU) and poorly controlled type 2 diabetes mellitus who was admitted for sepsis and thyrotoxicosis from infective endocarditis (IE), AST, prostate abscess, and pyelonephritis. He suffered from a cerebral vascular accident (CVA) from septic embolic showering. Thyroid-stimulating hormone (TSH) was <0.10 mIU/L, and free thyroxine (T4) levels were>90 pmol/L. Methicillin-resistant

Fourth branchial arch fistula and suppurative thyroiditis: a life-threatening infection.

A potentially life-threatening case of recurrent left-sided thyroid abscess formation secondary to a fourth branchial arch sinus fistula is presented. The patient developed a reversible left vocal fold palsy during an acute episode of suppurative thyroiditis requiring a temporary tracheostomy due to a compromised airway. Investigations commonly used to demonstrate this anomaly may fail to confirm the diagnosis as in the case presented and exploratory surgery with excision of the fistulous tract should still be considered. We describe a method of repairing the pharyngeal opening to reduce the r
