Transcutaneous Tibial Nerve Stimulation as Therapy for Functional Constipation.

BACKGROUND: Functional constipation is a common disorder that is difficult to treat on occasion. Symptoms of this condition can per- sist despite dietary modification, exercise, and medication. Results of neuromodulation with nerve stimulation have been promising in terms of efficiency for treatment-resistant patients. This study aimed to investigate the efficacy of bilateral transcutaneous tibial nerve stimulation as a noninvasive treatment method for functional constipation. METHODS: We evaluated 105 patients with functional constipation diagnosed using the Rome IV criteria. Bilateral trans

Posterior Tibial Nerve Schwannoma Presenting as Tarsal Tunnel Syndrome.

Schwannomas are rare, benign tumors originating in the Schwann cells of the peripheral nervous system. They are most commonly found in the head, neck, and upper extremities, which involve the spinal nerves of the brachial plexus. However, schwannomas of the lower extremities are extremely uncommon, and few studies have reported a schwannoma originating from the posterior tibial nerve. We report on a case of a 71-year old male who presented to our clinic because of left foot and ankle neuritic pain. A nerve tumor was found; subsequently, the tumor was surgically excised along with the release o

Percutaneous tibial nerve stimulation for patients with faecal incontinence.

Percutaneous tibial nerve stimulation (PTNS) is a new, non-invasive treatment for faecal incontinence. It is given in an outpatient setting and has few side effects. An increasing number of studies confirm its efficacy. This article discusses the procedure, reports early experiences of using PTNS at one centre in the UK and considers cost effectiveness.

Regional nerve block for femoropopliteal and tibial arterial reconstructions.

Thirteen high-risk patients underwent lower extremity revascularization anesthetized with a regional nerve block technique. The sciatic, femoral, and obturator nerves were infiltrated with 1% lidocaine and 0.25% bupivacaine. Eight femoropopliteal and five femorotibial bypasses were performed for limb salvage (11 patients), disabling claudication (one patient), and popliteal artery aneurysm (one patient). Analgesia was adequate with only one patient who needed supplemental nitrous oxide. One patient died on the sixth postoperative day of a myocardial infarction. Regional nerve block is an effec

Tibial nerve entrapment by a Baker cyst: case report.

We report a rare case of degenerative joint disease of both knees, complicated by a Baker cyst. Our emphasis is on the role of electromyography and electrodiagnosis in the localization of this nerve entrapment syndrome. The patient presented with pain and swelling; venography revealed deep venous thrombosis of the right calf, including the popliteal and proximal superficial femoral vessels. The patient responded well to bed rest, analgesics, intravenous heparin and subsequent Coumadin anticoagulation, and was discharged two weeks later. Five weeks after onset of these acute problems, nerve con

Emerging Implantable Tibial Nerve Stimulation Devices for Overactive Bladder.

Overactive bladder (OAB) substantially reduces quality of life (QoL), and novel implantable percutaneous tibial nerve stimulation (iPTNS) devices have emerged as promising alternatives to conventional management. This review provides a comprehensive comparative analysis of iPTNS devices for OAB, evaluating clinical outcomes, technical specifications, regulatory status, innovative features, and limitations-key aspects insufficiently addressed in prior studies. A narrative synthesis was conducted by reviewing clinical trials, technical reports, and regulatory documents related to 7 iPTNS devices

Posterior Tibial Tendon Transfer for Common Peroneal Nerve Injury.

Common peroneal nerve dysfunction after a multiligament knee injury can be devastating. In patients with persistent foot drop, posterior tibial tendon transfer to the dorsum of the foot is a reliable and safe procedure to restore dorsiflexion. These authors favor passing the posterior tibial tendon through the interosseous membrane and docking it into the lateral/middle cuneiforms. A Strayer procedure or tendo-Achilles lengthening must be performed in patients unable to achieve at least 10° of passive dorsiflexion. Despite the operative limb having 30% to 40% of ankle dorsiflexion strength of

Earthquake generated proximal tibial nerve compression treated by surgery.

PURPOSE: This article reports on nine cases of proximal tibial nerve compression by the soleal tendinous arch caused by unsuitable treatment of acute compartment syndrome (ACS). Also, we report the clinical results of neurolysis and analyse the cause of this special type of neurological compression. METHODS: There were nine extremities in nine patients included in the study. All patients were among the victims of the Wenchuan earthquake in 2008. All patients had a previous lower extremity ACS. Pain level, numbness in the sole, muscle strength of the flexor hallucis longus and Tinel's sign wer

Posterior tibial nerve stimulation and faecal incontinence: a review.

PURPOSE: Faecal incontinence is a common and important multifactorial disorder with a range of treatment options. Over the last two decades, neuromodulation via sacral nerve stimulators has been shown to be effective for both faecal and urinary incontinence, although associated with complications. Peripheral neuromodulation, via the posterior tibial nerve, is widely used in urinary incontinence; however, its use in faecal incontinence, whilst evolving is limited to eight small heterogeneous studies. REVIEW: These eight studies are discussed in the context of the methodology and underlying neu

Tibial and peroneal nerve conduction studies in ankle sprain.

BACKGROUND: Lower limb injuries and ankle sprain occur frequently among football players, due to the high incidence of physical contact. As a result, lower limb nerves are subject to injury. This project aims to evaluate the lower limb nerve conduction among students playing football to investigate the probable relationship between ankle sprain in football and nerve conductivity. MATERIALS AND METHODS: Fifty volunteer students aged between 19 and 25 were studied. They fell into three groups; one comprising of 20 healthy football players whose football experience exceeded three years, the seco

Ground reaction forces in Greyhounds with tibial nerve injury.

OBJECTIVES: To measure ground reaction force variables during lameness resulting from impaired tibial nerve function and to determine whether these variables changed significantly as recovery progressed. ANIMALS: 11 healthy Greyhounds of either sex, weighing between 22 and 39 kg. PROCEDURE: On 3 consecutive days before surgery, ground reaction forces were measured by force platform gait analysis at the trot. In dogs under general anesthesia, the left tibial nerve was crushed proximal to innervation of the gastrocnemius muscle. Gait analyses were repeated on days 8 to 10, 28 to 30, 43 to 45,

Tibial nerve somatosensory evoked potentials in spinal cord hemisection.

Somatosensory evoked potentials (SEPs) have been studied in many disease states since they were first described by Dawson in 1947. However, there have been very few reports of SEP findings in patients with spinal cord hemisection. On the basis of clinical correlation, Giblin first postulated that the activity giving rise to the initial cortical components of the SEP travels through the dorsal column-lemniscal system. However, the pathway through which these evoked potentials are transmitted has recently been questioned. This paper examines the results of tibial nerve SEPS performed on four pat

Gait abnormalities in tibial nerve paralysis: a biomechanical study.

Gastrocnemius-soleus dysfunction is a frequent result of cauda equina lesions and peripheral neurophathies and of stroke and brain injury. Temporary tibial nerve paralysis constitutes a comparable laboratory condition which allows the controlled examination of aspects of these disabilities. The biomechanical effects of temporary tibial nerve paralysis in six normal young adult volunteers were examined to quantitatively define the gait abnormalities resulting from gastrocnemius-soleus paralysis and to provide a basis for the assessment of the effectiveness of different orthotic designs in resto

Fascicular Involvement of the Posterior Tibial Nerve as a Result of Perineural Ganglion Cyst at the Posterior Tibial Nerve in the Calf: A Case Report and Review of Literature.

We report a 19-year-old woman with a 6-month history of nontraumatic left foot numbness associated with intermittent weakness. Nerve conduction studies and electromyography localized the lesion to the posterior tibial nerve, below the innervation to the soleus and medial gastrocnemius muscles. MRI of the left leg revealed a multiloculated cystic collection near the proximal tibiofibular joint. Surgical excision and pathology confirmed the diagnosis of a ganglion cyst, in an atypical location distal to the popliteal fossa. We believe this is the first reported case of fascicular posterior tibia

Axonal elongation through acellular nerve segments of the cat tibial nerve: importance of the near-nerve environment.

Peripheral nerve regeneration is considered to be influenced by structural, cellular and humoral factors in the distal nerve stump. Axonal elongation was, however, not affected by the presence of a 20 mm acellular nerve segment (ANS) distal to a crush lesion in a cat tibial nerve which was shielded from the environment by a silicone cuff [K. Fugleholm, H. Schmalbruch, C. Krarup, Early peripheral nerve regeneration after crushing, sectioning, and freeze studied by implanted electrodes in the cat, J. Neurosci., 14 (1994) 2659-2673]. In the present study axons were challenged to regenerate throug

Redefining the Topography of the Tibial Nerve: Updated Treatment Guide for Tibial Nerve Entrapments: An Addendum to Surgical Treatment Protocol for Peripheral Nerve Dysfunction of the Lower Extremity: A Systematic Approach.

Dysfunction of the tibial nerve can progress to painful and potentially disabling conditions. There are multiple sites of entrapment along the course of the tibial nerve. Detailed knowledge of the anatomy and anatomic variations is critical for a surgeon to be able to properly diagnose and treat patients with tibial nerve injuries. Repair of tibial nerve injuries involves a thorough history, physical examination, diagnostic studies, and microsurgical techniques. This article discusses sites of tibial nerve entrapment and use of a surgical algorithm that provides a systematic approach that has
