Arthrofibrosis of the knee following a fracture of the tibial plateau.

The aim of this study was to report the incidence of arthrofibrosis of the knee and identify risk factors for its development following a fracture of the tibial plateau. We carried out a retrospective review of 186 patients (114 male, 72 female) with a fracture of the tibial plateau who underwent open reduction and internal fixation. Their mean age was 46.4 years (19 to 83) and the mean follow-up was16.0 months (6 to 80). A total of 27 patients (14.5%) developed arthrofibrosis requiring a further intervention. Using multivariate regression analysis, the use of a provisional external fixator (o

[Bone disease with pain. Tibial plateau fracture].

Fractures that involve the proximal tibia affect knee kinematics and stability. Especially tibial prateau fractures are intra-articular injuries, impair not only metaphyseal bone but articular cartilage, meniscus and ligament. Basically the ideal treatment for tibial plateau fractures should be an anatomic reduction with stable fixation allowing weight bearing training and an early range of motion exercise, it is the reason that the indication of conservative treatment is limited. Open rudection and bone grafting followed by a buttress plate fixation is a general treatment for tibial plateau f

Bilateral Medial Tibial Plateau Stress Fractures: A Case Report.

Medial tibial plateau stress fractures are rare and often misdiagnosed due to nonspecific symptoms and initially normal radiographs. This case report describes a rare instance of bilateral medial tibial plateau stress fractures in a 43-year-old overweight male police officer with comorbidities including diabetes. The patient developed progressive knee pain after abruptly increasing his running activity. Initial radiographs were inconclusive, but magnetic resonance imaging (MRI) revealed bone marrow edema and non-displaced fractures, highlighting the importance of advanced imaging for early dia

Management of aseptic nonunions of bicondylar tibial plateau fractures.

PURPOSE: Nonunion of bicondylar tibial plateau (BTP) fractures following open reduction internal fixation (ORIF) is rare but challenging. We report a case series of aseptic BTP nonunions, approaches to treatment, and long-term outcomes. METHODS: Retrospective case series of aseptic nonunion in operatively treated BTP fractures. Cases with deep infection prior to a revision were excluded. Demographic, injury, and initial fixation characteristics were collected. Clinical course following diagnosis of nonunion was reviewed. Revision operation characteristics, timing, and outcomes were recorded.

Arthroscopically Assisted Percutaneous Screw Fixation of Tibial Plateau Fractures.

UNLABELLED: Tibial plateau fractures account for approximately 1% to 2% of fractures in adults1. These fractures exhibit a bimodal distribution as high-energy fractures in young patients and low-energy fragility fractures in elderly patients. The goal of operative treatment is restoration of joint stability, limb alignment, and articular surface congruity while minimizing complications such as stiffness, infection, and posttraumatic osteoarthritis. Open reduction and internal fixation with direct visualization of the articular reduction or indirect evaluation with fluoroscopy has traditionally

Age-Related Variances in Patients with Tibial Plateau Fractures.

Tibial plateau fractures account for approximately 8% of fractures in the elderly population. Treatment strategies in the elderly are similar to those for younger patients; however, practitioners must account for the elevated comorbidity burden in this population. To date, few studies have analyzed age-based outcomes in patients with tibial plateau fractures. Therefore, the purpose of this study was to determine age-related variances in demographics, fracture characteristics, mechanism of injury, and complications. A 10-year retrospective review was conducted to identify patients who received

Extended Anterolateral Approach for Complex Lateral Tibial Plateau Fractures.

Complex fractures of the lateral tibial plateau may extend to the posterior rim of the knee and to the tibial spines. Displaced fractures of the posterolateral corner of the tibial plateau may result in joint incongruity and instability, especially with the knee in flexion. Anatomical reduction of the joint surface and containment of the tibial rim are the primary goals of the treatment in such cases. Dedicated surgical approaches including dissection of the peroneal nerve, sometimes in association with an osteotomy of the fibular head are typically used to address these injuries. Some techniq

Treatment of complex tibial plateau fractures using Ilizarov technique.

Complex tibial plateau fractures are most commonly caused by high-energy trauma and they are often associated with severe soft tissue injuries that can frequently result in severe complications. Ilizarov external circular fixation is an ideal method of treatment for high- energy fractures of the tibial plateau when extensive soft tissue dissection and internal fixation are contraindicated. Our research included 50 consecutive patients and clinical features of the patients were evaluated during the follow up period using the following instruments: Gustillo-Anderson, Sc- hatzker and Arbeitsgemei

Treatment of tibial plateau fractures by limited internal fixation.

Seventy-five adults who sustained 76 tibial plateau fractures were treated according to a prospective protocol using instability in extension as the principal indication for operative fixation. Patients showing instability underwent closed manipulative reduction under fluoroscopic guidance. If significant joint depression persisted after reduction, elevation of the fracture was performed either from below using bone punches through a cortical window or via limited arthrotomy. Iliac crest bone graft was used to buttress depressed fractures. Fixation was then secured using 7-mm cannulated screws
