The prevalence of common skin infections in four districts in Timor-Leste: a cross sectional survey.

BACKGROUND: Skin infections are a common public health problem in developing countries; however, they are rarely managed using a population based approach. Recent data on the burden of skin infections in Timor-Leste are limited. Our survey appears to be the only widespread survey conducted in more than 30 years and was designed to determine the baseline prevalence of some common skin infections in Timor-Leste. METHODS: We conducted a cross sectional survey in 14 sites including community health clinics, schools and hospitals within four different geographical regions. Participants were examin

Seroprevalence and prevention of hepatitis B, measles and rubella among healthcare workers in Dili, Timor-Leste.

INTRODUCTION: The World Health Organisation recommends that healthcare workers (HCWs) are immune to measles and rubella, and those at risk of exposure are offered the hepatitis B vaccine. No formal programme for occupational assessment and provision of vaccinations to HCWs currently exists in Timor-Leste. METHODS: A cross-sectional study was undertaken to determine the seroprevalence of hepatitis B, measles and rubella among HCWs in Dili, Timor-Leste. All patient-facing employees at three healthcare institutions during April-June 2021 were invited to participate. Epidemiological data were col

Changing the malaria treatment protocol policy in Timor-Leste: an examination of context, process, and actors' involvement.

In 2007 Timor-Leste, a malaria endemic country, changed its Malaria Treatment Protocol for uncomplicated falciparum malaria from sulphadoxine-pyrimethamine to artemether-lumefantrine. The change in treatment policy was based on the rise in morbidity due to malaria and perception of increasing drug resistance. Despite a lack of nationally available evidence on drug resistance, the Ministry of Health decided to change the protocol. The policy process leading to this change was examined through a qualitative study on how the country developed its revised treatment protocol for malaria. This proce

Social inclusion and exclusion of people with mental illness in Timor-Leste: a qualitative investigation with multiple stakeholders.

BACKGROUND: Social inclusion is a human right for all people, including people with mental illness. It is also an important part of recovery from mental illness. In Timor-Leste, no research has investigated the social experiences of people with mental illness and their families. To fill this knowledge gap and inform ongoing mental health system strengthening, we investigated the experiences of social inclusion and exclusion of people with mental illness and their families in Timor-Leste. METHODS: Eighty-five participants from the following stakeholder groups across multiple locations in Timor

An assessment of knowledge, attitudes, and practices concerning rabies among communities with a high disease risk in Timor-Leste.

Timor-Leste reported its first rabies outbreak in March 2024, making it an emerging zoonotic health threat in the country. However, there was a lack of information about community knowledge, attitudes, and practices (KAP) regarding rabies which is crucial for informing disease response plans. This study investigated the KAP regarding rabies among communities in high-risk areas for rabies incursion and spread. Between July to September 2023, 660 heads of households were interviewed using structured questionnaires in municipalities bordering Indonesian West Timor, and Dili municipality. Dog owne

Cross-sectional survey of resistant priority pathogen carriage in the hospital setting in Timor-Leste 2020-2021.

BACKGROUND: Antimicrobial resistance (AMR) is a major global threat. The World Health Organization (WHO) has identified key organisms that carry the potential for both pathogenicity and AMR, leading to reduced treatment options and worsened outcomes, through the Global Antimicrobial Resistance and Use Surveillance System (GLASS). In Timor-Leste, surveillance is now possible due to significant gains in diagnostic capacity. METHODS: In 2020-2021 a cross-sectional surveillance study was performed on inpatients and outpatients across all hospitals in Timor-Leste. Nose/axilla/groin swabs were sele

Giardia duodenalis infection in the context of a community-based deworming and water, sanitation and hygiene trial in Timor-Leste.

BACKGROUND: Giardiasis is a common diarrhoeal disease caused by the protozoan Giardia duodenalis. It is prevalent in low-income countries in the context of inadequate access to water, sanitation and hygiene (WASH), and is frequently co-endemic with neglected tropical diseases such as soil-transmitted helminth (STH) infections. Large-scale periodic deworming programmes are often implemented in these settings; however, there is limited evidence for the impact of regular anthelminthic treatment on G. duodenalis infection. Additionally, few studies have examined the impact of WASH interventions on

An environmental assessment and risk map of Ascaris lumbricoides and Necator americanus distributions in Manufahi District, Timor-Leste.

BACKGROUND: In Timor-Leste there have been intermittent and ineffective soil-transmitted helminth (STH) deworming programs since 2004. In a resource-constrained setting, having information on the geographic distribution of STH can aid in prioritising high risk communities for intervention. This study aimed to quantify the environmental risk factors for STH infection and to produce a risk map of STH in Manufahi district, Timor-Leste. METHODOLOGY/PRINCIPAL FINDINGS: Georeferenced cross-sectional data and stool samples were obtained from 2,194 participants in 606 households in 24 villages in the

Demographic and spatial predictors of anemia in women of reproductive age in Timor-Leste: implications for health program prioritization.

Anemia is a significant risk factor for poor health outcomes for both the mother and neonate; however, the determinants of anemia in many epidemiological settings are poorly understood. Using a subset of a nationally representative cluster survey (2010 Demographic and Health Survey) in combination with other non-contemporaneous survey data, the epidemiology of anemia among women of reproductive age in Timor-Leste has been explored. Logistic regression was used to identify risk factors, population-level impacts were estimated as population attributable fractions and spatial analytics were used

Early life exposures of enteric pathogens and nutritional outcomes in Timor-Leste: a longitudinal birth cohort study protocol.

INTRODUCTION: Child undernutrition remains a major public health problem in low- and middle-income countries. Timor-Leste has one of the highest childhood stunting rates worldwide, affecting an estimated 47% of children under 5 years. Repeated exposure to enteric pathogens is increasingly recognised as a contributor to malnutrition, yet the interplay between enteric infection, environmental exposure and socioeconomic factors remains poorly understood in this setting. This protocol describes a longitudinal birth cohort study, conducted within a One Health framework, examining early-life enteric

Case-control study of risk factors for abscess development following hepatitis B vaccination in children in Timor-Leste, 2024.

BACKGROUND: Hepatitis B vaccination is essential for preventing chronic hepatitis B infection and its long-term complications. In 2024, a high proportion of reported adverse events following immunization (AEFI) in Timor-Leste involved injection-site abscesses among infants who received the hepatitis B birth dose. This study aimed to explore potential factors associated with abscess formation to inform improvements in immunization practices. METHODS: We conducted a retrospective matched case-control study in public health facilities across 10 municipalities in Timor-Leste. Cases were infants a

Service user and family participation in mental health policy making in Timor-Leste: a qualitative study with multiple stakeholders.

BACKGROUND: Participation in mental health system strengthening by people with mental health problems and their families is a cornerstone of people-centred mental health care, yet there is a dearth of research about participation from low- and middle-income countries (LMICs), particularly from the Asia Pacific region. Hence, this study aimed to assess the current situation, challenges, enabling factors and future actions for service user and family participation in mental health policy making in Timor-Leste. METHODS: In-depth interviews were conducted with 85 adults (≥18 years) who were: (1)
