Tinnitus in Children.

This perspective reviews the current state of the art and literature on tinnitus in children, prevalence and risk factors, clinical management, and future priorities for healthcare provision and research. Most research in the field to date appears to be prevalence studies, which have reached dramatically different estimates; this reflects the lack of a standard language when asking about the presence of tinnitus, or how bothersome, distressing, or negatively impacting it is for the child. Estimates are also likely affected by a lack of awareness of tinnitus amongst children and parents. Childr

Astrocytes and Tinnitus.

Tinnitus is correlated with anomalies of neural plasticity and has been found to be affected by inflammatory status. The current theories on tinnitus, although still somewhat incomplete, are based on maladaptive plasticity mechanisms. Astrocytes play a major role in both neural responses to inflammation and plasticity regulation; moreover, they have recently been discovered to encode "context" for neuronal circuits, which is similar to the "expectation" of Bayesian brain models. Therefore, this narrative review explores the possible and likely roles of astrocytes in the neural mechanisms leadi

Definition of Tinnitus.

Tinnitus is generally defined as the perception of sound in the absence of vibration of an external elastic body. If this definition appears useful to differentiate tinnitus from somatosounds, it is not suitable for distinguishing it from psychiatric hallucinations. Nor does this solution define a temporal limit of duration of the perception, which is important for distinguishing pathological tinnitus from those occasional noises that we all perceive from time to time. A complete definition appears necessary not only to achieve homogeneity in epidemiological studies but also to set up correct

Neuroinflammation and Tinnitus.

Neuroinflammation is the central nervous system's response to: injury, infection, and abnormal neural activity. Inflammatory processes are known to mediate many diseases, and recently evidence indicates that neuroinflammation underlies hearing disorders such as presbyacusis, middle-ear disease, ototoxicity, noise-induced hearing loss, and tinnitus. This chapter provides a review of the role of neuroinflammation in the etiology and treatment of tinnitus. Specifically, our research team has demonstrated that both tumor necrosis factor alpha (TNF-α) and calpain signaling pathways are involved in

Tinnitus in Canada.

BACKGROUND: Tinnitus, which has no cure, can be a temporary irritant or a life-altering condition. Many factors can precipitate tinnitus, including hearing loss, exposure to loud noise and other otologic causes, neurological injuries or disease, dental disorders, some medications, and certain infectious diseases. This study summarizes new tinnitus data from the Canadian Health Measures Survey (CHMS). DATA AND METHODS: Data were collected for individuals aged 19 to 79 years (n=6,571) from 2012 through 2015 as part of the CHMS. Tinnitus is described as "the presence of hissing, buzzing, ringing

A review of tinnitus.

BACKGROUND: Tinnitus is one of the most common otological symptoms. In recent times our understanding of tinnitus has significantly progressed. Tinnitus may be defined as conscious awareness of a sound in the absence of an external auditory stimulus. People with tinnitus almost always seek the attention of their general practitioner, who is best placed to assess, investigate and provide appropriate counselling. OBJECTIVE: The aim of this article is to define and provide guidelines on causes, assessment, appropriate investigation and management of tinnitus. DISCUSSION: Tinnitus may be categor

Betahistine for tinnitus.

BACKGROUND: Tinnitus is a symptom defined as the perception of sound in the absence of an external source. In England alone there are an estimated ¾ million general practice consultations every year where the primary complaint is tinnitus, equating to a major burden on healthcare services. Clinical management strategies include education and advice, relaxation therapy, tinnitus retraining therapy, cognitive behavioural therapy, sound enrichment using ear-level sound generators or hearing aids, and drug therapies to manage co-morbid symptoms such as sleep difficulties, anxiety or depression. As

Imaging of Tinnitus.

Although not all patients with tinnitus require imaging, patients with tinnitus and asymmetric hearing loss, additional neurologic findings, or pulsatile tinnitus should be evaluated with an appropriately tailored imaging study. Choice of imaging study should be guided by type of hearing loss and additional physical examination findings, such as middle ear lesion, presence of carotid bruit, or pulsatile tinnitus extinguished by jugular compression.

Neuroscience of Tinnitus.

Tinnitus is a consequence of changes in auditory and nonauditory neural networks following damage to the cochlea. Homeostatic compensatory mechanisms occur after hearing loss and these mechanisms alter the balance of excitatory and inhibitory neurotransmitters. In many individuals with hearing loss, chronic tinnitus and related phenomena emerge. Some people with tinnitus are disturbed by this subjective sensation. When auditory network dysfunction is coupled with limbic-gating dysfunction, an otherwise meaningless auditory percept such as tinnitus may acquire negative emotional features. The d

Coping with tinnitus.

This narrative article on coping with tinnitus has four primary objectives. First, theoretical perspectives about defining and categorizing coping are discussed. Second, an overview of the empirical research on coping with tinnitus is provided, focusing on how coping with tinnitus has been measured and on the trends that were found in research on coping with tinnitus. Third, the problems related to the current state of research on coping with tinnitus are highlighted. Fourth, suggestions are provided on ways that researchers can improve research on coping with tinnitus.

Treatment of tinnitus.

PURPOSE OF REVIEW: Tinnitus, the perception of sound in the absence of a corresponding external acoustic stimulus, is a highly prevalent and frequently severely impairing disorder with worldwide impact. In this article after a short overview about epidemiology and pathophysiology the currently available treatment options will be discussed with specific consideration of the available evidence, their mechanisms of action and their limitations. RECENT FINDINGS: During the last decades, advances in neuroimaging methods and the development of animal models have contributed to an increasing underst

Tinnitus and Headache.

BACKGROUND: Tinnitus and headache are frequent disorders. Here, we aimed to investigate whether the occurrence of headache among tinnitus patients is purely coincidental or whether tinnitus and headache are pathophysiologically linked. We investigated a large sample of patients with tinnitus and headache to estimate prevalence rates of different headache forms, to determine the relationship between tinnitus laterality and headache laterality, and to explore the relationship between tinnitus and headache over time. METHOD: Patients who presented at a tertiary referral center because of tinnitu

Is tinnitus an acoasm?

Tinnitus and unspecific auditory hallucinations generally known as acoasms arise from identical or at least similar cerebral structures. Both phenomena can be interpreted as signs of an over activation of neuronal networks. Several pieces of evidence to underline this hypothesis as well as its implications are discussed. It is even speculated that both clinical entities might profit from treatment strategies that are normally employed for treatment of the other.

Tinnitus and pain.

Tinnitus has many similarities with the symptoms of neurological disorders such as paresthesia and central neuropathic pain. There is considerable evidence that the symptoms and signs of some forms of tinnitus and central neuropathic pain are caused by functional changes in specific parts of the central nervous system and that these changes are caused by expression of neural plasticity. The changes in the auditory nervous system that cause tinnitus and the changes in the somatosensory systems that cause central neuropathic pain may have been initiated from the periphery, i.e. the ear or the au

[Tinnitus and diabetes].

Tinnitus is the perception of sound in the absence of corresponding external sound(s). Tinnitus can be perceived in one or both ears or in the head. It is usually described as an unpleasant noise, but in some patients it takes the form of a ringing, buzzing, hissing, humming, or whistling sound, or as ticking, clicking, roaring, tunes, songs, or beeping. It is estimated that 1 out of every 5 people experience some degree of tinnitus. Tinnitus is not itself a disease but an unwelcome symptom. It can be accompanied by audiometric evidence of deafness which occurs in association with both conduct

Mechanisms of tinnitus.

The generation of tinnitus is a topic of much scientific enquiry. This chapter reviews possible mechanisms of tinnitus, whilst noting that the heterogeneity observed within the human population with distressing tinnitus means that there may be many different mechanisms by which tinnitus can occur. Indeed, multiple mechanisms may be at work within one individual. The role of the cochlea in tinnitus is considered, and in particular the concept of discordant damage between inner and outer hair cells is described. Biochemical models of tinnitus pertaining to the cochlea and the central auditory pa

Tinnitus and otosclerosis.

Fifty patients with otosclerosis and tinnitus were studied for the course of the tinnitus after stapes surgery. Tinnitus disappeared in 64% of the cases, improved in 16%, was unchanged in 14%, and worsened in 6%. The reduction in tinnitus was more favorable after a small fenestra stapedotomy than after a partial stapedectomy with removal of the posterior half of the footplate. An unfavorable postoperative course of tinnitus did not appear to be linked to the postoperative audiometric result. In our study the subjective evaluation of pitch and loudness of the tinnitus did not provide significan

Update on tinnitus.

The study of a disorder such as tinnitus is fraught with difficulties. Tinnitus, like pain, is a subjective symptom. The problem is compounded because several different mechanisms must operate to cause the persistent sensation of tinnitus. Therefore, it is difficult to measure objectively any improvements in the condition. For example, it has been reported previously that sectioning the eighth cranial nerve does not abolish tinnitus in a majority of patients; therefore, central mechanisms must act to preserve the tinnitus. Finally, we know that tinnitus can occur in a host of conditions other

Tinnitus and ECT.

We report the case of a 69-year-old man with major depressive disorder and occupational tinnitus; both problems were refractory to several different treatment modalities, but were successfully treated with electroconvulsive therapy (ECT). The tinnitus was present for 15-20 years, but had worsened during the 3 years before treatment, causing him significant distress. Extensive organic workup did not identify a treatable etiology for his symptoms. To date, he has been hospitalized four times due to depressive symptoms associated with tinnitus. He has received ECT each time, and has responded wel
