Five- and 10-Year Follow-Up of Nonvascularized Toe Phalanx Transfers.

PURPOSE: The purpose of this study was to evaluate long-term outcomes of nonvascularized toe phalanx transfer. METHODS: We retrospectively reviewed 54 nonvascularized toe phalanx transfers in 29 children with symbrachydactyly. Forty-seven transfers in 24 children were evaluated at 5-year follow-up and 27 transfers in 14 children were evaluated at 10-year follow-up. We recorded the incidence of the early physeal closure and the length of the transferred toe phalanx on plain radiographs at 5- and 10-year follow-up. Growth rate in the first 5 years and the following 5 years were calculated. Func

Primary On-top Plasty for Treatment of Short-type Postaxial Polydactyly of the Foot.

BACKGROUND: Postaxial polydactyly of the foot is typically treated with medial or lateral toe ray resection. However, simple ray resection does not always give a natural postoperative appearance, especially for short-type polydactyly. The purpose of this article was to describe our primary operation with on-top plasty for lengthening of short-type postaxial polydactyly. METHODS: Four patients (mean age, 11.3 months) underwent this procedure. Ray transfer was performed at the proximal phalangeal level. Lateral distal and middle phalanges were resected and the medial ray was transferred on top

Duplicated distal phalanx of thumb or hallux in trisomy 13: A recurrent feature in a series of 42 fetuses.

Trisomy 13 or Patau syndrome (PS) is a well-known aneuploidy characterized by a polymalformative syndrome. We described a large series of fetuses with PS and compared them with cases described in the literature, most of which were live-born. In all, 42 fetuses, aged from 14 to 41 gestational weeks (GW), were examined. The main defects observed were similar to those described in live-born patients: congenital heart defects (76%), holoprosencephaly spectrum anomalies including arhinencephaly and hypotelorism (74%), urinary tract anomalies (71%), ear anomalies (69%), postaxial polydactyly (67%),

Shoe adaptation after amputation of the II - V phalangeal bones of the foot.

In The Netherlands, about 50% of all amputations of the lower limb are toes and forefoot amputations. Traumata of toes and mid-foot are rare. Preservation of the foot is the primary goal for treatment. Crush injuries of the foot may be associated with prolonged morbidity. This case study presents an insole solution for the solitary first phalangeal bone after amputation of the phalangeal bones II - V. The normal adaptation for forefoot amputations is stiffening of the sole of the shoe and a rocker bar to improve the toe off phase with load reduction of the forefoot. Because the patient had to

The Summitt syndrome: observations on a third case.

A 6 1/2 year old male presented with acrocephaly, brachydactyly, clinodactyly, mild syndactyly of the hands and feet, genu valgum, and marked obesity. Roentgenograms of the hands revealed hypoplasia or aplasia of the middle phalanges. Roentgenograms of the feet revealed hypoplasia of the middle phalanges and deformity of the proximal phalangeal epiphyses of the great toes. Chromosomes studies revealed a normal 46,XY karyotype, and psychological testing revealed low normal intelligence. Current data support autosomal recessive inheritance, although X-linkage cannot be excluded.

Sarcoidosis-associated acro-osteolysis.

Sarcoidosis is characterised by the formation of noncaseating granulomas classically affecting lungs, lymph nodes and skin. Osteoarticular involvement affects up to 15% of patients; however, acro-osteolysis, destruction involving distal phalanges of fingers and toes, associated with sarcoidosis, is extremely rare. A 44-year-old woman with a history of biopsy-proven sarcoidosis managed with prednisone and methotrexate presented with swelling and pain in the distal fingers of her right hand without skin manifestations. Radiographic imaging showed erosion of distal phalanges on second, third and

Osteochondroma of the proximal phalanx of toe presenting as widening of third web space- a rare case report.

Osteochondromas are the most common benign bone tumors, which usually affect the metaphyseal region of long bones. Foot is an unusual location for these tumors with phalanges being a much rare site. Only a few case reports have been made of phalangeal osteochondromas. A patient with proximal phalangeal osteochondroma of third toe, presenting clinically as widening of the third web space, is described. A simple extraperiosteal excision of the tumor was done which resolved his forefoot deformity. At five years followup the patient had no recurrence or symptoms.

Macrodactyly of the feet and hands.

We reviewed the records of 16 patients with true macrodactyly and analyzed the typical clinical features and methods of treatment. Fourteen feet were involved in 13 patients (one was bilaterally affected). Three hands were involved in three patients. Clinically, all lesions in the hands and lesions in 11 of 14 feet involved the preaxial side. There was multiple digit involvement in two hands and 11 feet. Progressive macrodactyly (10 feet and two hands) was more common than the static type (four feet and one hand). Proximal involvement of the sole or palm occurred in seven feet and one hand; al

Long-term postoperative outcomes of post-axial polydactyly of the foot with an emphasis on collateral ligament reconstruction.

BACKGROUND: Several studies have reported surgical techniques and favorable postoperative outcomes of post-axial polydactyly of the foot; however, few studies have investigated the long-term postoperative outcomes. The aim of this study was to evaluate long-term results of our surgical treatment, which emphasized the alignment between the distal and proximal phalanges, and the ligamentous stability. METHODS: This is a retrospective chart review of patients who underwent surgery for post-axial polydactyly and were followed up until epiphyseal closure of the foot. The study included 13 feet fro

Morphologic Changes in Postaxial Polydactyly of the Foot: A Standardized Quantitative Analysis Using the Watanabe-Fujita Classification.

BACKGROUND: Postaxial polydactyly of the foot is one of the most common congenital abnormalities. A wide forefoot, short toe, and lateral joint deviation are associated with aesthetic and functional outcomes. This study used the Watanabe-Fujita classification to characterize the preoperative and postoperative skeletal morphology of postaxial polydactyly of the foot. METHODS: This retrospective study included 42 patients (51 feet) with postaxial polydactyly treated at age 1 year. Radiographs taken at ages 0 and 3 to 4 years were used for morphologic analysis. The length of the reconstructed to

Variations in the insertion of the extensor hallucis longus muscle.

Reports that describe abnormalities of the muscles and tendons of the leg and foot are important with regard to understanding the function of the lower limb and correlating these abnormalities with the clinical aspects. Variations in the insertion of the extensor hallucis longus muscle were studied in 60 adult human cadavers. Three different patterns of insertion were noticed. Pattern I (65%): the extensor hallucis longus muscle had a single tendinous insertion on the dorsal aspect of the base of the distal phalanx of the big toe. Pattern II (26.67%): the muscle terminated in two tendons. Patt

New Notations for Better Morphological Distinction of Postaxial Polydactyly of the Foot.

UNLABELLED: There have been many reports on the classification and treatment of postaxial polydactyly of the foot. However, despite its being a common congenital anomaly, there is no universal notation about its morphology. METHODS: We performed an analysis of 65 postaxial polydactyly cases from 2004 to 2021. Judgment criteria for deciding the surgical procedure were selected, and the points required for notation were decided. Based on them, we devised a new notation. RESULTS: The necessary points required for notation were decided based on the following criteria: (1) the presence and level

Pseudoclubbing in a patient with sarcoidosis of the phalangeal bones.

We treated a patient who had sarcoidosis with pseudoclubbing. The involvement was asymmetric and involved some fingertips more than others. It did not involve the toes. The pseudoclubbing totally disappeared with effective prednisone therapy. Roentgenograms of the hands showed bone cysts of the distal and middle phalanges, and a gallium citrate Ga 67 scan showed uptake in two of the phalanges. We believe the pseudoclubbing in this patient is a manifestation of phalangeal bone involvement by sarcoidosis with associated dactylitis. A literature review indicates that true clubbing can exist in pa

Treatment of preaxial polydactyly of the foot.

Fourteen patients with preaxial polydactyly are classified into four types according to their morphologic configuration: type 1--ray duplication; type 2--completely duplicated phalanges; type 3--incompletely duplicated metatarsals; and type 4--incompletely duplicated phalanges. The surgical treatment and timing thereof appropriate for each group are discussed. Three cases of congenital hallux varus deformity are analyzed, focusing on the pathomechanism of this disorder. Based on our experience, two main factors are noticed in the etiology of congenital hallux varus deformity: inadequacy of the

An unusual cause of ulceration: ainhum (dactylolysis spontanea).

We describe a case of ainhum, a mutilating condition, usually seen in peoples of West African origin. A patient presented at the Diabetic Foot Clinic, Kings College Hospital, with a painful infected ulcer between the fourth and fifth toe of his right foot. He was a 43-year-old Nigerian and did not have diabetes. He reported that his left fifth toe had autoamputated some months before. Radiograph of the right fifth toe showed absorption of the proximal and intermediate phalanges, and a diagnosis of ainhum was made. Ainhum or dactylolysis spontanea is a rare condition of unknown aetiology in whi

Late Onset Osteonecrosis of the Proximal Phalanx of the Great Toe in a Child.

Osteonecrosis, also known as avascular necrosis or osteochondrosis, refers to the death of bone tissue resulting from a to lack of blood supply. Osteonecrosis of the foot is most commonly seen in the talus and metatarsals and not as frequently in other bones. We report an extremely rare case of late-onset osteonecrosis of the proximal phalangeal head of the great toe in a 9-year-old male. Plain radiographs and computed tomography did not show any findings of osteonecrosis initially. However, because of persistent pain at the interphalangeal joint of the first toe and a decrease in the range of

MODED: microcephaly-oculo-digito-esophageal-duodenal syndrome.

We report on 4 families with microcephaly, type A brachydactyly, variable learning disabilities, short stature, duodenal atresia, patent ductus arteriosus (PDA), hallux valgus, restricted elbow and finger movements, and amesophalangy and syndactyly of toes. Three previous families manifested all signs of the syndrome, and in another four reports the phenotype was incomplete. These cases are reviewed. Penetrance of digital and toe anomalies is almost complete and microcephaly is present in 78% of known cases. Short stature was present in 70% of our cases, but this is probably an overestimation.
