Toothache and Alzheimer's Disease: A Mendelian Randomization Study of the NAAG-CD33 Neuroimmune Axis.

BACKGROUND: Toothache, as an oral pain condition, may disrupt neuroimmune signaling and potentially influence neurodegenerative processes. This study investigated the relationship between toothache and Alzheimer's disease (AD) through a neuroimmune axis involving N-acetylaspartylglutamate (NAAG) and CD33+ myeloid monocytes. METHODS: We conducted a two-sample Mendelian randomization (MR) analysis using genome-wide association study (GWAS) summary statistics from the UK Biobank for toothache, a large-scale plasma metabolomics study for NAAG, and immune cell trait cohorts for CD33+ myeloid monoc

Losartan-associated toothache in a patient with diabetes and hypertension: a hypothesis-generating case report.

BACKGROUND: Angiotensin II receptor blockers (ARBs) are standard of care for hypertensive patients, particularly those with type 2 diabetes mellitus (T2D). Although generally well tolerated, rare adverse drug reactions can be overlooked. To our knowledge, no prior case report up to 2025 has described isolated toothache attributable to losartan. CASE PRESENTATION: A 45-year-old man with T2D and essential hypertension developed a reproducible, diffuse maxillary toothache ≈30 min after each 50 mg dose of losartan potassium. Oral examination and panoramic radiography found no odontogenic patholog

Screening preschool children with toothache: validation of the Brazilian version of the Dental Discomfort Questionnaire.

BACKGROUND: The Dental Discomfort Questionnaire (DDQ) is an observational instrument intended to measure dental discomfort and/or pain in children under 5 years of age. This study aimed to validate a previously cross-culturally adapted version of DDQ in a Brazilian children sample. METHODS: Participants included 263 children (58.6% boys, mean age 43.5 months) that underwent a dental examination to assess dental caries, and their parent that filled out the cross-culturally adapted DDQ on their behalf. Exploratory factor analysis (principal component analysis form) and psychometric tests were d

Toothache and temporomandibular disorder-like symptoms as red flags for maxillary sinus carcinoma.

A case of a patient with left maxillary molar pain unresponsive to standard dental treatment is presented. Advanced imaging revealed an aggressive lesion in the maxillary sinus with associated bone destruction, soft tissue extension, and diffusion restriction suggestive of malignancy. The lesion was subsequently confirmed as maxillary sinus carcinoma. This case highlights the diagnostic challenges associated with nonspecific temporomandibular disorder-like symptoms and emphasizes the importance of recognizing red flags as prompts for cross-sectional imaging and specialist referral, which are e

Does Toothache Before a Dental Visit Associate with the Risk of a First Myocardial Infarction?

Background/Objectives: Ischemic heart disease is typically characterized by chest pain that sometimes radiate to other areas, including the orofacial region. Atypical clinical presentation of cardiac disease risks leading to a delay in the diagnosis and treatment. Symptoms in the orofacial region may also lead to unnecessary dental interventions. The objective of this study was to assess occurrence of toothache, or other oral problems, that prompted a visit to a dental office prior to a first myocardial infarction. Methods: In 2010 until 2014, a total of 805 patients hospitalized for a first m

Case Report: A case of toothache of cardiac origin with a long-term clinical course.

BACKGROUND: Toothache of cardiac origin is a rare but significant form of referred pain originating from cardiac pathology such as angina pectoris. Although jaw and throat discomfort are known referred pain sites, toothache alone is an uncommon presentation. Misdiagnosis often leads to unnecessary dental interventions and delays in appropriate cardiac treatment, highlighting the need for greater awareness among both dentists and internists. CASE PRESENTATION: A 76-year-old woman presented with persistent pain in the gingiva around teeth #33 and #34, accompanied by sharp chest discomfort which

A Patient with Herpes Zoster of the Maxillary Division of Trigeminal Nerve Presents for Oral Evaluation and Toothache.

Herpes zoster (HZ) is an acute and painful neurocutaneous infection caused by the reactivation of a latent varicella-zoster virus in the dorsal root or cranial nerve ganglia. It is characterized by 3 stages: prodromal, acute, and chronic. During the prodromal stage, reactivation in the maxillary branch of the trigeminal nerve closely mimics odontalgia, and HZ should be in the differential diagnosis. Patients with HZ develop painful lesions following the affected dermatome. Laboratory testing confirms the diagnosis; treatment is with antiviral agents. Early detection and treatment shorten the c

A two-sample bidirectional Mendelian randomization study on gut microbiota, dental caries, and toothache.

We aimed to evaluate whether gut microbiotas have an effect on caries and toothache and explore whether dental caries and toothache have any impact on the identified significant bacterial genera. We first used genetic instruments of gut microbiota from the MiBioGen consortium to investigate the relationship with toothache and dental caries. The genome-wide association study summary data were from the UK Biobank and the FinnGen consortium, respectively. Mendelian randomization (MR) Egger regression, weighted median, inverse-variance weighted, simple mode, and weighted mode were used in the anal

High prevalence of toothache among Great East Japan Earthquake survivors living in temporary housing.

OBJECTIVES: The Great East Japan Earthquake and subsequent tsunami cause large-scale destruction in the north-eastern coastal areas in Japan, and forced many survivors to relocate to prefabricated temporary housing, a typical emergency accommodation. Based on the hypothesis that higher toothache prevalence among the disaster survivors is associated with postdisaster distress, we determined the impact of temporary residential environment as potential stressor on the subjective toothache prevalence. METHODS: A repeated cross-sectional health survey based on self-reported questionnaire was condu

Diagnostic Accuracy of Quantitative Sensory Testing to Discriminate Inflammatory Toothache and Intraoral Neuropathic Pain.

INTRODUCTION: A differential diagnosis between inflammatory toothache (IT) and intraoral neuropathic pain is challenging. The aim of this diagnostic study was to quantify somatosensory function of subjects with IT (acute pulpitis) and atypical odontalgia (AO, intraoral neuropathic pain) and healthy volunteers and to quantify how accurately quantitative sensory testing (QST) discriminates an IT or AO diagnosis. METHODS: The sample consisted of 60 subjects equally divided (n = 20) into 3 groups: (1) IT, (2) AO, and (3) control. A sequence of 4 QST methods was performed over the dentoalveolar mu

Mirrors and toothaches: commonplace manipulations of non-auditory feedback availability change perceived speech intelligibility.

This paper investigates the impact of two non-technical speech feedback perturbations outside the auditory modality: topical application of commercially-available benzocaine to reduce somatosensory feedback from speakers' lips and tongue tip, and the presence of a mirror to provide fully-detailed visual self-feedback. In experiment 1, speakers were recorded under normal quiet conditions (i.e., baseline), then again with benzocaine application plus auditory degradation, and finally with the addition of mirror feedback. Speech produced under normal and both feedback-altered conditions was assess

Marfan Syndrome With Recurrent Lower Left Posterior Toothache as the First Symptom: A Report on a Rare Case.

Marfan syndrome (MFS) is a life-threatening connective tissue disorder that affects multiple organs and systems. We report a case of MFS with recurrent lower left posterior toothache as the first symptom. A 23-year-old Asian man walked into the dental emergency room with a chief complaint of recurrent spontaneous and intermittent toothache in his lower left posterior tooth region, mimicking acute symptomatic pulpitis. He self-reported a relatively healthy medical status without any hereditary disease. However, his disproportionately elongated body structure, high myopia, and positive wrist sig

Diagnosis dilemmas in vital pulp therapy: treatment for the toothache is changing, especially in young, immature teeth.

The literature is almost devoid of scientific studies of diagnosis of pulpal pathology in primary and permanent teeth with open apices. Most reports are empirical or retrospective studies without adequate prior knowledge of preexisting conditions or histologic findings leading to the necessity of pulpal procedures. Appropriate diagnostic tests and their effectiveness are documented for both groups. This article reviews the available literature and current techniques of indirect pulp therapy, pulp capping, and pulpotomy for primary teeth and permanent teeth with open apex. The apical barrier wi

Trigeminal herpes zoster and Ramsay Hunt syndrome in an elderly adult: Presentation with prodromal toothache.

OBJECTIVE: This article describes an atypical herpes zoster (HZ) infection heralded by prodromic dental pain. BACKGROUND: Trigeminal HZ is associated with any of the three divisions of the trigeminal nerve, and Ramsay Hunt syndrome is a rare variant of HZ infection of the ear, often accompanied with unilateral facial nerve palsy. MATERIALS AND METHODS: An 87-year-old man sought remedy for tooth-like pain. Within a week, he developed cutaneous, intraoral and otic eruptions, hearing loss, and mild facial palsy. RESULTS: After ruling out odontogenic pain, he was diagnosed with trigeminal HZ an

Accidental finding of COVID-associated mucormycosis (CAM) in a patient presenting as toothache: A case report and review of literature.

UNLABELLED: Mucormycosis is an angio-invasive rapidly progressing fungal infection, usually reported in immunocompromised individuals. We present a case of COVID-associated mucormycosis in a patient with a presenting symptom of toothache in the maxilla with a possible mild case of COVID-19. ABSTRACT: Coronavirus-associated mucormycosis (CAM) had reached epidemic status, especially during the second wave of COVID-19. It was especially prevalent in India with a large mortality rate. Mucormycosis, particularly the rhinocerebral type is seen to be greatly associated with COVID-19, especially in p
