Hemodynamically unstable non-compressible penetrating torso trauma: a practical surgical approach.

Penetrating torso trauma is the second leading cause of death following head injury. Traffic accidents, falls and overall blunt trauma are the most common mechanism of injuries in developed countries; whereas, penetrating trauma which includes gunshot and stabs wounds is more prevalent in developing countries due to ongoing violence and social unrest. Penetrating chest and abdominal trauma have high mortality rates at the scene of the incident when important structures such as the heart, great vessels, or liver are involved. Current controversies surround the optimal surgical approach of these

Selective nonoperative management of penetrating torso injury from combat fragmentation wounds.

BACKGROUND: Historically, military surgical doctrine has mandated exploratory laparotomy for all penetrating fragmentation wounds. We hypothesized that stable patients with abdominal fragmentation injuries whose computerized tomography (CT) scans for intraperitoneal or retroperitoneal penetration disclosed nothing abnormal, can be safely observed without therapeutic laparotomy. METHODS: We retrospectively studied all hemodynamically stable patients with penetrating fragmentation wounds to the back, flank, lower chest, abdomen, and pelvis evaluated by abdominal physical examination (PE), CT, o

Neuroinflammation in primary blast neurotrauma: Time course and prevention by torso shielding.

Mechanisms of primary blast injury caused by overpressure are not fully understood. In particular, the presence and time course of neuroinflammation are unknown and so are the signatures of reactive inflammatory cells, especially the neuroprotective versus injurious roles of microglia. In general, chronic microglial activation in the injured brain suggests a pro-degenerative role for these reactive cells. In this study, we investigated the temporal dynamics of microglial activation in the brain of mice exposed to mild-moderate blast in a shock tube. Because, in our previous work, we had found

Surgical adjuncts to noncompressible torso hemorrhage as tools for patient blood management.

Despite the tremendous advances and successes in the care of combat casualties over the past 15 years of war, noncompressible torso hemorrhage (NCTH) remains the most likely source of potentially preventable death (approx. 25%) on the battlefield. This is also likely true for civilian victims of blunt and penetrating trauma. Various devices and therapeutic interventions have been, and are being, developed in an attempt to reduce morbidity and mortality for patients with NCTH. Examples include the use of prehospital blood and blood products, tranexamic acid, specially designed tourniquets for j

Finite element modeling of subcutaneous implantable defibrillator electrodes in an adult torso.

BACKGROUND: Total subcutaneous implantable subcutaneous defibrillators are in development, but optimal electrode configurations are not known. OBJECTIVE: We used image-based finite element models (FEM) to predict the myocardial electric field generated during defibrillation shocks (pseudo-DFT) in a wide variety of reported and innovative subcutaneous electrode positions to determine factors affecting optimal lead positions for subcutaneous implantable cardioverter-defibrillators (S-ICD). METHODS: An image-based FEM of an adult man was used to predict pseudo-DFTs across a wide range of techni

[Venous interventions. Part 2: Thromboses of the upper torso and mesenteric veins].

This article focuses on the percutaneous treatment options of venous thromboses and occlusions. Interventional therapy modalities for mediastinal, brachial, axillar, cerebral, and mesenteric thromboses are reviewed. Stenoses of the superior vena cava are most often caused by mediastinal malignomas. Balloon angioplasty together with stent implantation is an effective method for revascularisation in cases of upper venous congestion. Benign central venous stenoses are common in dialysis patients for whom interventional therapy using balloon angioplasty has proven excellent results. In the treatme

Motion sickness provoked by torso rotation predicts that caused by head nodding.

INTRODUCTION: Some degree of space motion sickness is experienced by at least 50% of astronauts early in flight. It is unpleasant and could be hazardous during an extravehicular activity or urgent re-entry in the first few days after launch. To date, there is no reliable ground-based test to predict who will be affected. METHODS: Head nodding (HN) in a supine position was used as a model of motion sickness caused by an unusual gravito-inertial environment. Torso rotation (TR) was used as a means of predicting susceptibility to development of symptoms caused by HN. Motion sickness was quantifi

Torso injury patterns and mechanisms in car crashes: an additional diagnostic tool.

Torso injuries among car occupants involved in sever collisions are often life threatening. This study considers whether knowledge of the type of impact and associated trends of torso injuries would assist the rapid diagnosis of injuries by emergency personnel. For the purpose of this analysis the pelvis was included in the torso. The data source was the Cooperative Crash Injury Study which holds detailed injury information on over 6200 car crash occupants. Restrained front seat occupants with an ISS of 16+ who were involved in frontal or side collisions were selected. A total of 286 occupants

Injectable Montmorillonite Nanoclay-Reinforced Hydrogel for Rapid Hemostasis in Noncompressible Torso Hemorrhage.

Noncompressible torso hemorrhage (NCTH) presents a significant challenge in trauma care, necessitating innovative hemostatic approaches. This study reports the development of an injectable composite hydrogel incorporating montmorillonite nanoclay, carboxymethyl cellulose, and oxidized xanthan gum cross-linked via acyl hydrazone linkages. The hydrogel demonstrates exceptionally rapid gelation, solidifying within 5 s at the wound site, thereby enabling immediate hemostatic action. Integration of montmorillonite nanoclay significantly enhances both the hemostatic efficacy and mechanical stability

Immediate versus delayed fluid resuscitation for hypotensive patients with penetrating torso injuries.

BACKGROUND: Fluid resuscitation may be detrimental when given before bleeding is controlled in patients with trauma. The purpose of this study was to determine the effects of delaying fluid resuscitation until the time of operative intervention in hypotensive patients with penetrating injuries to the torso. METHODS: We conducted a prospective trial comparing immediate and delayed fluid resuscitation in 598 adults with penetrating torso injuries who presented with a pre-hospital systolic blood pressure of < or = 90 mm Hg. Assessment of Muscular Fatigue on Hip and Torso Biomechanics in Adolescent Baseball Pitchers.

Johnson, AL, Kokott, W, Dziuk, C, and Cross, JA. Assessment of muscular fatigue on hip and torso biomechanics in adolescent baseball pitchers. J Strength Cond Res 39(8): 893-899, 2025-Muscular fatigue is an injury risk indicator, and its effect on pitching biomechanics warrants investigation. This study evaluated hip and torso biomechanics to measure muscular fatigue after a limited number of pitches thrown. We hypothesized that postpitching tests would reveal reduced stride length and hip and torso strength, altering hip and torso kinematics and increasing throwing arm kinetics. Adolescent ma

Design and Preliminary Evaluation of a Torso Stabiliser for Individuals with Spinal Cord Injury.

Spinal cord injuries generally result in sensory and mobility impairments, with torso instability being particularly debilitating. Existing torso stabilisers are often rigid and restrictive. We present an early investigation into a non-restrictive 1 degree-of-freedom (DoF) mechanical torso stabiliser inspired by devices such as centrifugal clutches and seat-belt mechanisms. First, the paper presents a motion-capture (MoCap) and OpenSim-Based kinematic analysis of the cable-based system to understand the requisite device characteristics. The evaluation in simulation resulted in the cable-based

Multiple impalement injuries of the torso with two metal bars: A case report.

INTRODUCTION AND IMPORTANCE: Impalement injury is a type of penetrating trauma that is rarely mentioned and studied in the medical literature. Patients who could show up at the emergency room are those who got an isolated stab wound in their extremities. The torso injuries are usually fatal in the scene of the accident. We report two metal bars that impaled in the pelvic, back, abdomen and thorax of the patient who was admitted in OR directly. CASE PRESENTATION: A 20 year-old construction worker fell down on two metal bars. At the scene; we cut the metallic bars off the column and then we tra

Resuscitative endovascular balloon occlusion of the aorta: an option for noncompressible torso hemorrhage?

PURPOSE OF REVIEW: Hemorrhage is the major cause of early death in severely injured patients. In civilian emergency medical services, the majority of life-threatening bleedings are found in noncompressible body regions (e.g. abdomen and pelvis). Resuscitative endovascular balloon occlusion of the aorta (REBOA) has therefore been discussed in recent years as a possible lifesaving procedure and numerous studies, meta-analyses and guidelines have been published. In this review, the data situation of REBOA in the management of bleeding trauma patients is discussed and practical implementation is d

Efficacy of Head and Torso Rewarming Using a Human Model for Severe Hypothermia.

INTRODUCTION: To evaluate the rewarming effectiveness of a similar amount of heat (from a charcoal heater) applied to either the head or torso in a human model for severe hypothermia in which shivering is pharmacologically inhibited in mildly hypothermic subjects. METHODS: Six male subjects were cooled on 3 different occasions, each in 8°C water for 60 min, or to a lowest core temperature of 35°C. Shivering was inhibited by intravenous meperidine (1.5 mg·kg-1), administered during the last 10 min of the cold-water immersion. Subjects then exited from the cold water, were dried, and were place

Accuracy of 3D surface scanners for clinical torso and spinal deformity assessment.

Externally visible deformities are cosmetic features of great concern for Adolescent Idiopathic Scoliosis (AIS) patients. Current assessment techniques for AIS do not fully encompass the external deformity. A non-invasive method capable of capturing superficial anatomy, such as 3D scanning, would enable better qualitative and quantitative evaluation of cosmesis. This study aimed to quantify the accuracy of commonly available scanners, in assessing posterior asymmetry in AIS. The technique of 3D surface deviation analysis was proposed as a suitable method for comparing the models created by eac

Morphometric analysis of torso arterial anatomy with implications for resuscitative aortic occlusion.

BACKGROUND: Hemorrhage is a leading cause of death in military and civilian trauma. Despite the importance of the aorta as a site of hemorrhage control and resuscitative occlusion, detailed knowledge of its morphometry is lacking. The objective of this study was to characterize aortic morphometry in a trauma population, including quantification of distances as well as and diameters and definition of relevant aortic zones. METHODS: Center line measures were made (Volume Viewer) from contrast computed tomography (CT) scans of male trauma patients (18-45 years). Aortic zones were defined based o

Injury pattern and mortality of noncompressible torso hemorrhage in UK combat casualties.

BACKGROUND: Hemorrhage following traumatic injury is a leading cause of military and civilian mortality. Noncompressible torso hemorrhage (NCTH) has been identified as particularly lethal, especially in the prehospital setting. METHODS: All patients sustaining NCTH between August 2002 and July 2012 were identified from the UK Joint Theatre Trauma Registry. NCTH was defined as injury to a named torso axial vessel, pulmonary injury, solid-organ injury (Grade 4 or greater injury to the liver, kidney, or spleen) or pelvic fracture with ring disruption. Patients with ongoing hemorrhage were identi

The insect neuropeptide PTTH activates receptor tyrosine kinase torso to initiate metamorphosis.

Holometabolous insects undergo complete metamorphosis to become sexually mature adults. Metamorphosis is initiated by brain-derived prothoracicotropic hormone (PTTH), which stimulates the production of the molting hormone ecdysone via an incompletely defined signaling pathway. Here we demonstrate that Torso, a receptor tyrosine kinase that regulates embryonic terminal cell fate in Drosophila, is the PTTH receptor. Trunk, the embryonic Torso ligand, is related to PTTH, and ectopic expression of PTTH in the embryo partially rescues trunk mutants. In larvae, torso is expressed specifically in the
