How do we remediate blood banking/transfusion medicine fellows?

BACKGROUND: Remediation of physician learners is not infrequently discussed in the graduate medical education (GME) literature. However, none of this literature is specific to blood banking/transfusion medicine (BB/TM) fellows and their typical 1-year fellowship program. Identifying a fellow's deficiencies and establishing an effective remediation program during a time-limited BB/TM fellowship is challenging. STUDY DESIGN AND METHODS: GME literature on remediation of physician learners was reviewed. A group of current and former BB/TM fellowship directors convened virtually to discuss if and

[Twin-twin transfusion syndrome--front line in fetal medicine].

Twin-twin transfusion develops in 10-20% of monochorionic twin pregnancies. As far as we know the underlying disturbance is an unbalanced anastomosis between the two fetuses on the placental surface. Without treatment mortality is as high as 80% if diagnosed before viability. One of this article's authors spent two months in Hamburg at the 'Allgemeine Krankenhaus', Barmbeck in Germany and describes the technique used there to coagulate these anastomoses via fetoscope. The outcome seems very promising with overall survival of 80% using this method. In the article pathogenesis and alternative tr

How we treat: transfusion medicine support of obstetric services.

BACKGROUND: Obstetric services depend on the transfusion service (TS) to provide diagnostic testing and blood component therapy for clinical care pathways. STUDY DESIGN AND METHODS: We describe three quality improvement (QI) initiatives implemented to improve TS support of obstetric services. RESULTS: We implemented a pathway for patients requiring an ABO/Rh order for every admission to obstetric services, along with reconciliation of the daily hospital birth manifest and TS umbilical cord log to identify every woman eligible for RhIG. After assessment over 6 months, 21 (1%) of 2041 women la

Quality improvement to decrease specimen mislabeling in transfusion medicine.

CONTEXT: Proper specimen identification and labeling is a critical preanalytic step in pretransfusion compatibility testing. OBJECTIVE: To gather baseline data for specimen mislabeling, specifically targeting major mislabeling events, and to design and implement a plan of corrective action. DESIGN: All mislabeled specimens received by the transfusion service for a type and screen were recorded and classified into minor and major mislabeling categories. Major mislabeling events were tracked by origin of the specimen. Locations with a high proportion of major mislabeling were given timely feed

Prion biology in transfusion medicine: implications for lab testing.

Although eerily silent for many years after the recognition of scrapie in 1759, TSEs remained present within the genome of some mammals. Not since the mid-1950s when Dr. Carleton Gadjusek visited the Fore Indians of New Guinea to study kuru, however, has there been a more frenetic interest by governmental investigators. Certainly, the U.K. experience has heralded a renewed interest in TSEs due to the notoriety associated with younger subjects succumbing to a variant CJD traced to the ingestion of beef. Human TSEs and the potential for their transmission among and across species of mammals has

Feline transfusion medicine. Blood types and their clinical importance.

Anemia is the most common indications for a blood transfusion in cats and fresh whole blood is typically administered. Recent experimental studies and clinical reports have elucidated the importance of feline blood types in transfusion medicine. The AB blood group system, the only recognized feline blood group system, consists of three blood types: type A is most common, type B is frequently found in certain breeds, and type AB occurs rarely. The blood type frequently varies geographically among domestic shorthair cats and between breeds. Because of the presence of naturally occurring alloanti

[Molecular biological screening of viruses important to transfusion medicine].

The usefulness of the direct virus detection by polymerase chain reaction (PCR) and reverse transcription/polymerase chain reaction (RT PCR) for blood donor screening was investigated, including the following viruses: cytomegalovirus (CMV), hepatitis B virus (HBV), hepatitis C virus (HCV) and human immunodeficiency type 1 virus (HIV1). Hepatitis C viraemia was detected by RT PCR in 97% of anti-HCV-positive haemophiliacs, in 48% of anti-HCV-positive hepatitis patients, in only 21% of anti-HCV-positive blood donors from North-West Germany, and not at all in 945 blood donors with elevated serum A

Beyond transfusion: Platelet-derived therapeutic products as a new frontier for blood establishments and transfusion medicine.

BACKGROUND AND OBJECTIVES: Allogeneic blood components and plasma-derived medicinal products (PDMPs) are cornerstone therapies in modern medicine, setting standards for quality, safety and efficacy. While platelets are traditionally transfused to prevent or control bleeding, they also serve as reservoirs of bioactive molecules with regenerative, anti-inflammatory, anti-oxidative and neuroprotective properties. This review examines how pooled human platelet lysates (HPLs) and platelet-derived extracellular vesicles (p-EVs) could be developed as therapeutic products, building on the principles e

Precision Medicine in Transfusion-Dependent and Non-Transfusion-Dependent β-Thalassemia: Toward Personalized Diagnosis and Therapy.

β-thalassemia comprises a clinically heterogeneous group of disorders in which anemia severity, transfusion exposure, iron loading, and organ complications vary widely among individuals. This structured narrative review summarizes practical applications of precision medicine in transfusion-dependent thalassemia (TDT) and non-transfusion-dependent thalassemia (NTDT), with explicit attention to which strategies apply to each clinical category. Literature indexed in PubMed and Scopus from 2000 to 2025 was reviewed using terms related to thalassemia, precision medicine, magnetic resonance imaging

Transfusion-associated Chagas disease (American trypanosomiasis) in Mexico: implications for transfusion medicine in the United States.

BACKGROUND: Trypanosoma cruzi, the protozoan cause of Chagas disease, causes life-long infection and is easily transmitted by blood transfusion. Our goals were to determine the prevalence of Chagas disease among donors in five Mexican blood banks, to look for evidence of transmission of T. cruzi by transfusion, and to evaluate two serologic assays for Chagas disease. STUDY DESIGN AND METHODS: Blood samples from donors were tested initially with the Abbott Chagas EIA or the Meridian Chagas' IgG ELISA. Samples giving readings that were at least 50% of the cutoffs were run in a confirmatory radi

Viral Metagenomics for Identification of Emerging Viruses in Transfusion Medicine.

Viral metagenomics has revolutionized our understanding for identification of unknown or poorly characterized viruses. For that reason, metagenomic studies gave been largely applied for virus discovery in a wide variety of clinical samples, including blood specimens. The emerging blood-transmitted virus infections represent important problem for public health, and the emergence of HIV in the 1980s is an example for the vulnerability of Blood Donation systems to such infections. When viral metagenomics is applied to blood samples, it can give a complete overview of the viral nucleic acid abunda

James Blundell, Obstetrical Hemorrhage, and the Origins of Transfusion Medicine.

Although James Blundell is rightly acknowledged as the father of modern transfusion therapy, a review of the events surrounding the initial human-to-human transfusions in the first decades of the 19th century reveals substantial contributions by Blundell's collaborators. Bundell's uncle John Haighton provided substantial support for animal experimentation in the growing field of physiology studies. John Leacock of Barbados provided the essential original experiments that focused the path of investigation on within-species transfusions. London instrument makers such as Laundy and Lloyd provided
