Transitions of care and changes in distressing pain.

This study employed a 22-state mortality follow-back survey to examine bereaved family members' perception of the level and pattern of distressing pain in decedents with cancer at the last two sites of care. Of the 1,578 individuals interviewed, 423 of their family members had cancer listed as the leading cause of death on the decedent's death certificate. Decedents were treated at home, hospitals, hospices, or nursing homes, with more than half of the respondents (n = 216) reporting that the decedent was at more than one site of care in the last month. Forty-two percent of decedents had distr

Transitioning to Adulthood from Foster Care.

Transitional age foster youth do not typically receive the types of family supports their nonfoster peers enjoy. Many foster youth experience multiple adversities and often fare worse than nonfoster peers on long-term functional outcomes. Governments increasingly recognize their responsibility to act as parents for state dependents transitioning to adulthood and the need to provide services to address social/emotional supports, living skills, finances, housing, education, employment, and physical and mental health. More research is needed to inform the development of effective programs. Transi

The psychophysics of transition to long term care.

More than a decade after the publication of To Err Is Human, cognitive error remains a mystery to physicians. Competent and conscientious physicians rarely recall making a single cognitive error, yet this must be central to the explanation for inappropriate physician nonadherence to evidence-based guidelines. Published information regarding cognitive error in the medical literature is scarce and widely scattered. We do know that cognitive error is induced by complexity, duress, and uncertainty, conditions that regularly confront long term care physicians when they assume care of a new patient

Hemodynamic phenotyping of bronchopulmonary dysplasia: from transitional circulation to precision cardiopulmonary care.

OBJECTIVES: Bronchopulmonary dysplasia (BPD) remains one of the most important complications of extreme prematurity and a leading cause of long-term respiratory, cardiovascular, and neurodevelopmental morbidity. Increasing evidence suggests that BPD should be viewed not only as a parenchymal lung disorder but as a complex cardiopulmonary syndrome involving disrupted vascular development, abnormal transitional circulation, and ventricular dysfunction. This review aimed to summarize current evidence on the hemodynamic mechanisms underlying BPD, emphasizing pulmonary vascular disease (PVD), bronc

Coordinated-Transitional Care for Veterans with Heart Failure and Chronic Lung Disease.

OBJECTIVES: Patients with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) account for most 30-day hospital readmissions nationwide. The Coordinated-Transitional Care (C-TraC) program is a telephone-based, nurse-driven intervention shown to decrease readmissions in Veterans Affairs (VA) and non-VA hospitals. The goal of this project was to assess the feasibility and efficacy of adapting C-TraC to meet the needs of complex patients with CHF and COPD in a large urban tertiary care VA medical center. DESIGN: We used the Replicating Effective Programs model to guide

Patients with juvenile idiopathic arthritis become adults: the role of transitional care.

Most juvenile idiopathic arthritis (JIA) patients need to attend adult rheumatology centres to continue the clinical management of their disease and to receive adequate long-term treatment. Transition from the paediatric to the adult health care team is a critical moment in the clinical history of these patients, but unfortunately, about 50% of the transfer processes to adult rheumatology are not successful, putting these patients at high risk of unfavourable outcomes. There are several obstacles to the success of transitional care for JIA patients, such as the absence of specific criteria for

Enhancing continuity of care: Codevelopment of an intensive care unit transitions in care bundle.

INTRODUCTION: Transitions from the adult intensive care unit (ICU) to other care settings can be challenging for patients, families, and healthcare providers, with risks to communication, care continuity, and psychosocial well-being. OBJECTIVE: We aimed to codevelop patient- and family-centred resources to educate, empower, and engage adult ICU patients and their families during their ICU-to-ward and ICU-to-home transitions, supporting them as informed partners in their care journey. METHODS: Building on previously published scoping reviews and end-user input that identified 76 candidate ICU

Person-centred care transitions for people with stroke: study protocol for a feasibility evaluation of codesigned care transition support.

BACKGROUND: Care transitions following stroke should be bridged with collaboration between hospital staff and home rehabilitation teams since well-coordinated transitions can reduce death and disability following a stroke. However, health services are delivered within organisational structures, rather than being based on patients' needs. The aim of this study protocol is to assess the feasibility, operationalised here as fidelity and acceptability, of a codesigned care transition support for people with stroke. METHODS: This study protocol describes the evaluation of a feasibility study using

Evolving management of upper-tract transitional-cell carcinoma at a tertiary-care center.

BACKGROUND AND PURPOSE: Traditional management of upper-tract transitional-cell carcinoma (TCC) has been open nephroureterectomy. Minimally invasive options, including laparoscopic and endoscopic techniques, are being applied with increasing frequency, however. To assess the impact of these techniques on the current management of upper-tract TCC, we reviewed our experience managing this problem over the last 3 years. PATIENTS AND METHODS: Since January 1998, 84 patients underwent definitive management of upper-tract TCC using open, laparoscopic, or endoscopic techniques. This study group incl

Clinical Outcomes and Transitional Care Following Endoscopic Variceal Treatment in Pediatric Portal Hypertension.

BACKGROUND: Esophagogastric variceal (EGV) bleeding is a serious complication of pediatric-onset liver disease; however, evidence regarding endoscopic management and long-term outcomes remains limited. This study aimed to evaluate clinical outcomes following endoscopic therapy for pediatric EGV and to clarify challenges related to adherence and transitional care. METHODS: We retrospectively reviewed patients with pediatric-onset liver disease who underwent endoscopic treatment for EGV between 2012 and 2024. Patients were categorized into a childhood group (≤ 15 years) and a young adult group

Implementation of a billable transitional care model for stroke patients: the COMPASS study.

BACKGROUND: The COMprehensive Post-Acute Stroke Services (COMPASS) pragmatic trial compared the effectiveness of comprehensive transitional care (COMPASS-TC) versus usual care among stroke and transient ischemic attack (TIA) patients discharged home from North Carolina hospitals. We evaluated implementation of COMPASS-TC in 20 hospitals randomized to the intervention using the RE-AIM framework. METHODS: We evaluated hospital-level Adoption of COMPASS-TC; patient Reach (meeting transitional care management requirements of timely telephone and face-to-face follow-up); Implementation using hospi

Project ACHIEVE - using implementation research to guide the evaluation of transitional care effectiveness.

BACKGROUND: Poorly managed hospital discharges and care transitions between health care facilities can cause poor outcomes for both patients and their caregivers. Unfortunately, the usual approach to health care delivery does not support continuity and coordination across the settings of hospital, doctors' offices, home or nursing homes. Though complex efforts with multiple components can improve patient outcomes and reduce 30-day readmissions, research has not identified which components are necessary. Also we do not know how delivery of core components may need to be adjusted based on patien

Gaps in transitional care: what are the perceptions of adolescents, parents and providers?

BACKGROUND: Several studies have investigated preferences and experiences of adolescents with different chronic conditions and their parents. Some have included the provider's perspective. Studies comparing the three perspectives on satisfaction with (transitional) care for different chronic conditions, however, are lacking. The main aim of this paper was to explore differences and similarities in perspectives between adolescents with chronic conditions, their parents and providers on transitional care. A secondary aim was to explore the extent to which such perspectives are disease-specific.
