Transvertism: a disorder of the sense of self.

Transvestism is not simply a sexual disorder, but is best understood as primarily a disorder of the sense of self. A descriptive background of transvestism is provided through a review of developmental history, clinical course, presonality structure, and family history. The predominant transvestic fantasies and their modes of enactment are described. The disorder of the sence of self that gives rise to transvestic behavior is identified as a split in the ego into incompatible male and female gender identities. The split is attributed to an early identification with the mother as a defense agai

A further assessment of Blanchard's typology of homosexual versus non-homosexual or autogynephilic gender dysphoria.

In a series of important but now highly controversial articles, Blanchard examined associations of sexual orientation and transvestic fetishism among male-to-female (MTF) transgender persons in Toronto, Canada. Transvestic fetishism was rare among the homosexuals but prevalent among the non-homosexuals. Subtypes of non-homosexual MTFs (heterosexual, bisexual, and asexual) were consistently high with regard to transvestic fetishism. Non-linear associations of a continuous measurement of sexual attraction to women (gynephilia) and transvestic fetishism were interpreted in terms of an etiological

Distressed transvestites. Psychometric characteristics.

Transvestic fetishism is characterized by eroticized cross-dressing in a heterosexual male. Twenty-one transvestites who were seeking psychiatric consultation were evaluated using the Derogatis Sexual Functioning Inventory (DSFI) and the Brief Symptom Inventory (BSI). They were compared with 45 heterosexual married males. The transvestic group was significantly more distressed on most dimensions of the BSI. The transvestic group displayed a more negative body image, a more feminine gender role perception, and less experience than the comparison group on the DSFI. Those transvestites who had no

The diagnosis and treatment of transvestites and transsexuals.

Transvestism and transsexualism are defined. The incidence of both as well as male versus female ratios are discussed. Etiology of each is considered. Psychotherapists can be used for proper diagnosis and couseling and, on occasion, for therapy for transvestites. Methods for this therapy are suggested. The need for establishing rapport is emphasized, and items to cover in a sex history are enumerated. Reality factors in sex changes are items that the therapist must grapple with. Factors that delineate differential diagnosis of transvestism and transsexualism are pointed out. Prevention of tran

The ICD diagnoses of fetishism and sadomasochism.

In this article we discuss psychiatric diagnoses of sexual deviation as they appear in the International Classification of Diseases (ICD-10), the internationally accepted classification and diagnostic system of the World Health Organization (WHO). Namely, we discuss the background of three diagnostic categories: Fetishism (F65.0), Fetishistic Transvestism (F65.1), and Sadomasochism (F65.5). Pertinent background issues regarding the above categories are followed by a critique of the usefulness of diagnosing these phenomena today. Specifically, we argue that Fetishism, Fetishistic Transvestism,

Co-morbidity of alcoholism and the paraphilias.

Alcoholism has been related to a wide variety of crimes. Various authors have studied the prevalence of alcoholism in relationship to incest, rape and pedophilia. This study evaluates the co-morbidity of alcoholism and the specific paraphilias including sexual sadism, fetishism, incest, pedophilia, exhibitionism and transvestism. Rapists were included although by definition it is not strictly a paraphilic disorder. Seven hundred and twenty eight paraphilic individuals were evaluated. More than 50% of sexual sadists were alcoholics. Alcoholism in association with sexual sadists was statisticall

A sex caused inconsistency in DSM-III-R: the definition of mental disorder and the definition of paraphilias.

The DSM-III-R definition of mental disorder is inconsistent with the DSM-III-R definition of paraphilias. The former requires the suffering or increased risk of suffering some harm while the latter allows that deviance, by itself, is sufficient to classify a behavioral syndrome as a paraphilia. This inconsistency is particularly clear when examining the DSM-III-R account of a specific paraphilia, Transvestic Fetishism. The author defends the DSM-III-R definition of mental disorder and argues that the DSM-III-R definition of paraphilias should be changed. He recommends that the diagnostic crite

Gender identity disorder and courtship disorder.

A hypothetical connection between gender identity disorder and courtship disorder was investigated in 274 heterosexual male patients of the following types: nontransvestic fetishists for female attire; transvestic fetishists; patients with gender identity disorder of adolescence and adulthood, nontranssexual type; and transsexuals. Of these patients, 53 had also demonstrated one or more of the putative expressions of courtship disorder. The proportion each of these types contributed to this group of 53 patients with a courtship disorder was compared with the same type's proportional contributi

Two types of cross-gender identity.

A revision of the typology of male cross-gender identity was carried out by means of formalized, easily replicable methods. The results suggest (1) that there are two discrete types of cross-gender identity, one heterosexual, the other homosexual; (2) that transvestism, and closely related conditions of cross-gender identity, occur exclusively or almost exclusively in heterosexuals; (3) that of the two types of transsexualism distinguished in this study, type A is, in heterosexuals, very rare or completely nonexistent; (4) that (in the course of time) transvestites or borderline transsexuals (

[Modern images of womanhood and their relationship to fetishism].

The image of womanhood accepted by society has changed from the corseted lady at the beginning of the 20th century to the emancipated and booted woman of today. The dictates of fashion in clothing correspond to unconscious wishes nurtured by men; the present fashion enables latent fetishist tendencies to work themselves out within the social framework and is directed at creating the "phallic woman". The boot and leather fetishism masquerading under the guise of a fashionable trend sheds light on unconscious sexual behaviour in the male, the underlying psychodynamics of which are further illumi

Erotic target location errors: an underappreciated paraphilic dimension.

Based on studies of heterosexual male fetishists, transvestites, and transsexuals, Blanchard (1991) proposed the existence of a hitherto unrecognized paraphilic dimension, erotic target location errors (ETLEs), involving the erroneous location of erotic targets in the environment. ETLEs can involve preferential attention to a peripheral or inessential part of an erotic target, manifesting as fetishism, or mislocation of an erotic target in one's own body, manifesting as the desire to impersonate or become a facsimile of the erotic target (e.g., transvestism or transsexualism). Despite its pote

Cross-gender identity in transvestites and male transsexuals.

A self-theory of transvestism and secondary transsexuality in which gender identity is a major self subsystem has been advanced in previous research. Within this framework transsexuals and transvestites were compared on a number of developmental characteristics. While early-onset transsexuals (n = 103) were dominantly female, both late-onset transsexuals (n = 52) and transvestites (n = 36) showed much more feminine behavior than expected. This was interpreted as a sign that they were already developing a feminine gender identity in their early years. Implications for this theory were discussed

The social organization of transvestite prostitution and AIDS.

This paper reports on the social organization of 53 transvestite prostitutes in Atlanta (GA), U.S.A. The central focus of the paper is on the relationship between social organization, social networks, and HIV risk behaviors in three geographic areas. In one of the areas the HIV infection rate was significantly higher than in the other two areas (81.1% vs 62.5% and 12.5%). Transvestites from the high prevalence area are strongly committed to transvestism and, consequently, are socially isolated. They report the lowest income, engaged in passive anal sex, have a history of syphilis and report in

Early history of the concept of autogynephilia.

Since the beginning of the last century, clinical observers have described the propensity of certain males to be erotically aroused by the thought or image of themselves as women. Because there was no specific term to denote this phenomenon, clinicians' references to it were generally oblique or periphrastic. The closest available word was transvestism. The definition of transvestism accepted by the end of the twentieth century, however, did not just fail to capture the wide range of erotically arousing cross-gender behaviors and fantasies in which women's garments per seplay a small role or n

Buspirone hydrochloride in the treatment of an atypical paraphilia.

A case report involving buspirone hydrochloride in the successful treatment of a patient with an atypical paraphilia and transvestic fetishism is presented. Treatment outcome was assessed by the patient's self-report as well as by retrospective examination of detailed notes about paraphilia fantasies which unknown to the therapists, had been kept by the patient. Preliminary evidence indicates that buspirone appears to effectively treat some paraphilias.

Stigma, sex and society: a new look at gender differentiation and sexual variation.

This paper takes a critical look at the hormonal basis of sexual behavior and concludes that it plays no part in determination of sexual activity or sexual preference in any of its forms including homosexuality and transsexualism. The empirical evidence of psychosexual functioning does not support Money's schema which suggest that sexual variation (homosexuality), transsexualism, and transvestism are incongruities of gender identity. At alternative approach which places gender differentiation and sexual variation in a social context is presented.
