Isolated dorsal dislocation of the trapezium is an exceptionally rare carpal injury, typically secondary to high-energy trauma, and carries a high likelihood of being overlooked during the initial radiographic evaluation. Its identification requires careful assessment of carpal alignment, particularly the Gilula arcs, as radiographic alterations may be subtle. Trapezial stability depends on a complex capsuloligamentous system, with the dorsoradial ligament playing a key role; disruption of this structure permits dorsoradial displacement of the bone and results in joint instability. We present

OBJECTIVES: The aim of the study was to assess the accessibility of the trapezium surface to arthroscopic resection related to the degenerative stage, to define the surgical approach and to determine the efficacy of the X-rays for evaluating the amount of resected bone.
METHODS: Fourteen trapezectomies were performed on seven cadaveric specimens using an arthroscopic procedure. Pre- and post-procedure X-rays were used to assess the initial trapeziometacarpal osteoarthritis and the resection performed. Each subject was randomized for using two posterior portals or two posterior portals and one

Fractures of the carpal bones involve only a single bone or complex bones with or without ligament rupture. However, fractures of the trapezoid are rarely seen. Because the trapezoid is fastened to the trapezium, capitate, and scaphoid by strong ligaments, fracture or dislocation is limited by this rigid fixation. The authors present a single bone fracture of the trapezoid in a 40-year-old man. A tomogram of the carpal bone was useful in diagnosing the trapezoid fracture. The mechanism for development of fracture of the trapezoid alone is unknown. However, fracture of the trapezoid seemed to o

The RegJoint™ (Scaffdex Oy, Finland) implant is a bio-absorbable poly-L/D-lactide implant which acts as a temporary support in resected joint spaces. It can be used in base of thumb surgery as a spacer to prevent first metacarpal subsidence. However, high rates of adverse tissue reactions and bone osteolysis have been reported recently by one group. The objective of this study was to investigate the outcome of patients treated in our institution with this implant. Patients underwent a postoperative clinical and radiological assessment. The QuickDASH questionnaire, Patient Evaluation Measure (P

Trapeziometacarpal joint osteoarthritis is a common degenerative disease. Arthrodesis is a widespread historical surgical technique to treat this pathology, providing pain relief and stability and strength of the thumb. Nevertheless, pantrapezial arthritis and non-union are not uncommon complications, leading in some cases to revision surgery. No gold-standard procedure has been described for revision of trapeziometacarpal arthrodesis failure. We describe two cases of failed arthrodesis treated with trapeziectomy and suspension ligamentoplasty, a well-known, biological, low-cost, successful, a

Trapeziectomy has been considered as the gold standard for treating trapeziometacarpal arthritis. But trapezial space collapse is responsible for thumb strength decrease and intracarpal deformities (with or without tendon interposition). Partial trapeziectomy with interposition of a chondrocostal autograft combines the advantages of trapeziectomy and a biological spacer without the disadvantages of arthroplasty. Partial trapeziectomy is performed by a dorsal approach, under regional anaesthesia. The graft is harvested by a direct approach of the 9th rib during a short bout of general anaesthes

Congenital synostosis has been described between nearly all of the carpal bones. It can occur in an isolated form or most commonly in relation between the lunatum and the triquetrum. It can also occur in a multiple form and be associated with other malformations. We report a rare case of scaphoid-trapezium fusion observed bilaterally in a nearly asymptomatic 63-year-old patient.

We describe the short-term outcomes of combination therapy involving ear cartilage transfer and suture-button suspension arthroplasty for the treatment of symptomatic thumb carpometacarpal joint arthritis. The concept is to reduce joint arthritis by separate independent approaches, which are renewing the damaged joint surface of the trapezium and first metacarpal bone suspension. Autologous ear cartilage is a desirable material for joint resurfacing arthroplasty without postoperative synovitis or infection, and it is expected to prevent bone impingement or recurrent joint arthritis when postop

OBJECTIVE: CT imaging precisely and quantitatively analyzes the kinematics of the carpal bones to evaluate the etiology of related osteoarthritis. Previous studies have investigated the kinematics of the trapeziometacarpal joint using static CT scans of various postures including the pinch position. This study analyzed the in-vivo kinematics of the trapeziometacarpal joint during dynamic pinch motion in young healthy volunteers using four-dimensional CT.
MATERIALS AND METHODS: Twelve healthy young volunteers participated in this study. Each participant held the pinch meter between their thumb

Considering the large number of procedures designed for the thumb trapeziometacarpal (TMC) joint, it is evident that none are completely satisfactory. Total arthroplasty attempts to improve the results of resection or space-occupying arthroplasties which, while providing good pain relief, have offered little thumb stability, strength, or normal motion. Conditions of surrounding joints indicate that this disease process is not always a straightforward clinical presentation and that a variety of surgical options are needed. When joint disease is limited to the TMC joint, bone stock is good and t

BACKGROUND: Manipulating the carpal arch width (i.e. distance between hamate and trapezium bones) has been suggested as a means to increase carpal tunnel cross-sectional area and alleviate median nerve compression. The purpose of this study was to develop a finite element model of the carpal tunnel and to determine an optimal force direction to maximize area.
METHODS: A planar geometric model of carpal bones at hamate level was reconstructed from MRI with inter-carpal joint spaces filled with a linear elastic surrogate tissue. Experimental data with discrete carpal tunnel pressures (50, 100,

BACKGROUND: Our goal was to analyze the movement of acute scaphoid waist fracture fragments and adjacent bones in a common coordinate system. Our hypothesis was that the distal scaphoid fragment flexes and pronates and the proximal fragment extends.
METHODS: Computed tomography (CT) scans of patients diagnosed with an acute scaphoid waist fracture were evaluated using a 3-dimensional (3D) model. The scans of 57 nondisplaced and 23 displaced fractures were compared with a control group of 27 scans showing no pathological involvement of the wrist. Three anatomical landmarks were labeled on the

An asymptomatic atypical U shaped cleft hand has been found in a 21-year-old lady attending OPD. On digital skiagram it was found that central digits were absent with remnants of bases of the metacarpals, which have fused with the carpal bones. Moreover, the scaphoid and trapezium had fused to form a single mass. There was no other anomaly in other limbs, so far searched for. An endeavor has been made to explain the anomaly with ontogenetic review.

Osteoarthritis of the carpometacarpal joint of the thumb ( CMC-1) is a common problem and many patients will seek medical attention and may eventually need surgical treatment. As no treatment option has shown consistent superiority, a variety of surgical procedures have been proposed. In this review paper we present our algorithm for the approach of symptomatic CMC-1 osteoarthritis, supported by scientific evidence and our personal experience. It is primarily based on patient demands, status of CMC-1 disease and associated problems around the thumb. The selection of a successful surgical treat

Trapeziometacarpal (TMC) joint arthroplasty has become an established and effective treatment option for advanced trapeziometacarpal osteoarthritis. Prosthetic replacement can also be employed in revision procedures following failed surgeries involving the trapezium or the base of the first metacarpal. Revision arthroplasty is technically demanding, as altered anatomical conditions, scar tissue, and compromised bone quality may complicate implantation. Patients and Surgical Techniques Based on specific cases, we present indications and techniques for revision arthroplasty of the TMC joint, inc

BACKGROUND: To perform a comprehensive osteologic investigation into trapezium anatomy and investigate the relationship between anatomical factors and osteophyte formation, focusing on sex-specific differences.
METHODS: This was a cadaveric study involving 1233 trapezia and first metacarpals. Two subgroups ("Control" and "Main Study") were established. The "Control" cohort was used to identify features of the trapezium in specimens devoid of osteophytic change. The prevalence and severity of osteophytic change were investigated in the "Main Study" cohort. Sex differences were specifically ass

Thumb carpometacarpal (CMC) arthroplasty with resection of the trapezium and soft tissue interposition, with or without ligament reconstruction, has historically proven to be an efficacious treatment for thumb CMC arthritis. The incidence of failure following primary thumb CMC arthroplasty is low; however, the evaluation and management of a patient experiencing an unsatisfactory outcome following CMC arthroplasty is challenging. If symptoms are refractory to conservative measures, then revision surgical treatment may be indicated. Clinical decision making becomes even more complicated in cases

Most carpal fractures are based on the same injury mechanism. In most cases the injury is caused by a fall on the dorsiflexed hand, which is raised as a protective measure, and less frequently by a fall on the clenched fist. The latter leads to passive palmar flexion, pronation and radial inclination of the hand; in the former, the hand is passively superextended by the impact, supinated, and brought into ulnar inclination. The instability caused by injury to various articular structures has four developmental stages. The bones or syndesmoses that are involved depend on the stage and quantitat
