Influence of Different Incision Designs on Flap Extension: A Cadaveric Animal Model.

It is well known that wound dehiscence is one of the most frequent complications in guided bone regeneration. The main cause of this complication may be a lack of tension-free and primary wound closure. The aim of this study was to evaluate and compare the effect of periosteal releasing incisions (PRI) on the extension of 3 different flap designs: envelope, triangular, and trapezoidal. Twelve pig mandibles were used to quantify extension of the flap designs. The mandibles were equally and randomly distributed into the 3 flap groups. Each mandible was divided into 2 sides: 1 was subjected to a

Evaluation of 3D Trapezoidal Plate Versus 2 Miniplates in Treatment of Subcondylar Fracture.

INTRODUCTION: Functional rehabilitation following functional therapy without MMF, closed reduction with maxillomandibular fixation (MMF), and open reduction with/without MMF are the 3 primary recommended adult condylar fractures treatments. The purpose of this study was to assess the 3D trapezoidal plate efficacy in the fixation of extracapsular mandibular subcondylar fractures in comparison with the 2 miniplates. METHODS: This comparative, randomized clinical study involved 20 cases that were selected from the outpatient clinic of the Oral and Maxillofacial Surgery Department, Faculty of Den

Trapeziometacarpal joint dislocation complicated by a trapezium fracture: A case report and literature review.

The authors report and discuss the management of a 25-year-old male patient with a trapeziometacarpal joint dislocation complicated by a trapezium fracture. This injury combination is rare with only reported 15 cases. After closed reduction resulted in dorsal instability, anatomical reduction of the trapezium fracture and internal screw fixation were performed using an open approach. This fixation stabilized the joint but was fragile due to the small fragment size. For this reason, ligament reconstruction using half of the flexor carpi radialis tendon was added. At the last follow-up, 16 month

[The trapezio-metacarpal joint: the strain of the ligaments as a function of the thumb position. Study on an enlarged model].

INTRODUCTION: The aim of this paper was to develop an enlarged anatomical model of the trapezio-metacarpal joint in order to measure the strains on the ligaments when this joint was passively moved in several directions under constant loading. MATERIAL AND METHOD: A model of the two first rays of the hand was made in polystyrene, at a X3 enlargement, and the ligaments substituted by rubber bands with well characterized mechanical properties so as to reproduce the actual ratio of stiffness (approximately = 10) of the different tissues (bones and ligaments) found in real life. The first metacar

Coalition of trapezoid, capitate, and metacarpal: a case report.

Carpal coalition is a rare wrist anomaly. A coalition can be an isolated condition, but it can be observed as a component of a congenital syndrome as well. Carpal coalition is usually asymptomatic and detected incidentally. It is most frequently encountered as lunate-triquetral coalition, followed by capito-hamate coalition. Other coalition types are very rare. In this case report, we present a case in which the patient applied with complaints of pain, and in whom a coalition of capitate-trapezoid- second and third metacarpal was detected.

The modified Zancolli arthroplasty for basal thumb arthritis.

The modified Zancolli arthroplasty for basal thumb arthritis is an alternative to different procedures described for this pathology. As most of the methods, it includes complete trapeziectomy. The main difference is that the interposition is made with capsule (resembling Colonna's method for the hips) and that the base of the metacarpal is stabilized in the correct place by a tendon transfer (performed with an accessory of abductor pollicis longus). The main advantage of this method is the short immobilization time (2 wk) and high percentage (95%) of complete pain-free results and a stable and

Biomechanical comparison of three-dimensional titanium and CFR-PEEK plates for mandibular subcondylar fracture fixation: a three-dimensional finite element analysis.

BACKGROUND: Fixation of mandibular subcondylar fractures presents challenges due to limited surgical access and the small size of the proximal fragment; therefore, single-piece and easily applicable three-dimensional (3D) plates may be considered as an alternative. Although titanium is frequently used as a plate material, it has been reported to have certain clinical and imaging-related limitations. In comparison, Carbon Fiber-Reinforced Polyetheretherketone (CFR-PEEK) has emerged as a favorable alternative for mandibular fracture fixation due to its bone-like elastic properties, radiolucency,

Osteotomy for "delta-phalanx" deformity.

A "delta phalanx" is often not triangular but may be trapezoidal. A "delta phalanx" may be present without a supernumerary digit. It is a form of complex syndactyly. Correction of the deformities of "delta phalanz" is obtained by open wedge osteotomy with excised bone fragments used as a graft and Z-plasty of the skin on the ulnar side of the finger.

Osteosynthesis Using the Anatomical Plate With Grasping Arms for Unstable Distal Clavicle Fractures: A Technical Trick and Clinical Experience.

Plate fixation is an established method of treating distal clavicle fractures. However, it is often difficult to maintain the reduction of distal fragments with conventional plates, especially in cases where the distal fragments are small or comminuted. This study aimed to introduce a technical trick and clinical experience of osteosynthesis using an anatomical nonlocking plate with grasping arms (SCORPION NEO plate) for unstable distal clavicle fractures. During fixation, distal fragments are grasped by the plate arms and fixed with 2 screws. We retrospectively reviewed 41 patients who underw

A finite element analysis of the trapezoidal plate. How to get a stable fixation at different fracture lines?

The fractures in the condylar area are a challenge for every surgeon, for the treatment of which trapezoidal condylar plate is used in most cases. However, it is not possible to position the plate in the ideal osteosynthesis lines according to Meyer et al. in every clinical situation. In many cases, the fracture line is also not in the centre of the trapezoidal plate. The aim of this study is to investigate the osteosynthesis rigidity and the effect of plate localisation relative to the fracture line. In a simulation model in the first group the plate was positioned in the ideal position in th

A finite element study and mathematical modeling of lumbar pedicle screw along with various design parameters.

BACKGROUND: Lumbar pedicle screw is one of the most common and important elements in the field of lumbar surgery. It plays a great role in rectifying the spinal alignment and stabilization providing strength and stability to the affected area of spine. In spinal surgery, minimally invasive techniques and minor incisions are made which makes it less painful for the patients than the traditional methods. Moreover, the screws are not needed to be removed after the surgery which is yet another great advantage of the pedicle screw. METHOD: In this study, 3D Finite Element (FE) model of human L4 ve

Limited wrist arthrodeses. I. The triscaphoid joint.

Limited wrist arthrodesis is the surgical fusion of selected bones of the wrist. In each case the extent of the fusion is determined by the extent of the disease process. Limited wrist arthrodesis of the joints between the scaphoid, trapezium, and trapezoid is indicated in localized degenerative arthritis, radial hand dislocations, and certain instability patterns following ligament rupture (rotary subluxation of the scaphoid). Fusion is easily obtained utilizing a local bone graft donor site. Fusion of the three-bone unit leaves approximately 80% of flexion extension and 66% of radial and uln

Trapezoidal osteotomy of the distal radius for unacceptable articular angulation after Colles' fracture.

Loss of radial palmar tilt in malunion of Colles' fractures alters wrist biomechanics, abnormally loading the tenuous dorsal ligament complex. This can result in midcarpal instability with synovitis, pain, weakness, and possible articular degeneration. An osteotomy to correct radius alignment using a trapezoidal corticocancellous bone graft has been devised. The graft is harvested from the dorsal radius adjacent to the osteotomy and malunion site. When turned 90 degrees, a properly designed trapezoidal graft restores radial length, inclination, and palmar tilt in one step. Functional results i

[The "candlestick" technique for the correction of certain types of congenital metacarpal synotosis].

Metacarpal synostosis is a rare congenital hand malformation requiring only occasionally a surgical correction. However in case of divergent epiphyses there is a progressive accentuation of the deformity. In the "Y" type of symmetrical synostosis, the authors propose a trapezoidal osteotomy with upside down relocation allowing realignement of the epiphyses without distant bone donor site.

Volar Dislocation of Trapezoid with Coronal Fracture - A Case Report and Review of Literature.

Volar dislocation of the trapezoid is a rare injury and is easily missed. It is more common to have a dorsal dislocation of trapezoid or multiple carpometacarpal joint dislocations. The rare nature of the injury also means that there is little guidance in literature regarding optimal treatment. We are reporting the presentation, management and a 3-year follow-up of this rare injury in a 19-year-old male. Level of Evidence: Level V (Therapeutic).

Carpal Tunnel Cross-Sectional Area Affected by Soft Tissues Abutting the Carpal Bones.

The carpal tunnel accommodates free movement of its contents, and the tunnel's cross-sectional area is a useful morphological parameter for the evaluation of the space available for the carpal tunnel contents and of potential nerve compression in the tunnel. The osseous boundary of the carpal bones as the dorsal border of the carpal tunnel is commonly used to determine the tunnel area, but this boundary contains soft tissues such as numerous intercarpal ligaments and the flexor carpi radialis tendon. The aims of this study were to quantify the thickness of the soft tissues abutting the carpal
