SUMMARY: We report a case of a woman with a diagnosis of breast cancer who unintentionally started gaining weight, feeling tired, and constipated 44 weeks after the initiation of trastuzumab. Hypothyroidism secondary to an autoimmune thyroiditis associated with trastuzumab was diagnosed, the first case described in Portugal and the fourth case described worldwide. Our intention regarding the publication of this case report is to alert the clinicians treating people with trastuzumab that they should ask the patients about symptoms of hypothyroidism and should screen the thyroid function of the

The review discusses mechanisms of trastuzumab cardio- toxicity. There are presented risk factors of development of the left ventricular dysfunction in treatment of a drug, among which the most important are the simultaneous administration of anthracycline antibiotics and the presence of associated cardiovascular diseases. There are discussed the features of cardiac management of patients receiving trastuzumab therapy that focuses on early detection of abnormalities of left ven- tricular function from the normality as well as measures aim- ing at preventing the occurrence of cardiac dysfunctio

BACKGROUND: A 29-year-old woman was diagnosed with lymph-node-positive, steroid-hormone-receptor-negative and HER2/neu-positive breast cancer. She underwent surgery followed by cyclophosphamide, epirubicin, and 5-fluorouracil chemotherapy, and received radiotherapy followed by trastuzumab therapy for 1 year. During the tenth month of trastuzumab therapy, the patient reported a missed period and had a positive pregnancy test.
INVESTIGATIONS: Physical examination, pregnancy test, echocardiography.
DIAGNOSIS: Unintended pregnancy during adjuvant trastuzumab therapy.
MANAGEMENT: Cessation of tr

BACKGROUND: Since the clinical introduction of trastuzumab (Herceptin) for metastatic breast cancers that overexpress human epidermal growth factor receptor 2 (HER2), this anticancer agent has played an important role in breast cancer treatment. We examined the effects of trastuzumab and vinorelbine (Navelbine) as a second- or third-line therapy in 24 patients whose HER2-positive tumors did not respond to or relapsed after administration of trastuzumab alone or in combination with taxane.
METHODS: Trastuzumab was administered at 2 mg/kg (loading dose 4 mg/kg) once weekly and vinorelbine at 25

OBJECTIVE: To evaluate the efficacy and safety of T-DM1 versus trastuzumab plus pertuzumab in adjuvant treatment of HER2-positive breast cancer patients with residual invasive disease after neoadjuvant therapy.
METHODS: This study retrospectively analyzed 184 patients treated at Renmin Hospital of Wuhan University from January 2019 to January 2022. The patients were divided into a T-DM1 treatment group and a dual-target treatment group based on whether they received T-DM1 or trastuzumab plus pertuzumab during adjuvant therapy. Clinical and pathological data of the two groups were collected be

PURPOSE: The ATEMPT trial was designed to determine if treatment with trastuzumab emtansine (T-DM1) caused less toxicity than paclitaxel plus trastuzumab (TH) and yielded clinically acceptable invasive disease-free survival (iDFS) among patients with stage I human epidermal growth factor receptor 2-positive (HER2+) breast cancer (BC).
METHODS: Patients with stage I centrally confirmed HER2+ BC were randomly assigned 3:1 to T-DM1 or TH and received T-DM1 3.6 mg/kg IV every 3 weeks for 17 cycles or T 80 mg/m2 IV with H once every week × 12 weeks (4 mg/kg load →2 mg/kg), followed by H × 39 weeks

Trastuzumab has been widely used in breast cancers. Despite its efficacy, trastuzumab-induced pneumonitis (TIP) remains a rare yet potentially fatal complication. While corticosteroids are the mainstay of treatment, steroid-refractory cases pose significant challenges due to the lack of standardised guidelines and limited data on additional therapies. We describe a case of grade 4 TIP in a female in her early 70s, initially managed with steroids and intravenous immunoglobulin, resulting in minimal clinical improvement. Due to the lack of response, advanced immunosuppressive therapy with inflix

HER2 is overexpressed in 20-25% of breast cancers and is associated with an aggressive phenotype and poor prognosis. Lapatinib is a dual tyrosine kinase inhibitor selective for inhibition of epidermal growth factor receptor (EGFR1/ErbB1) and HER2/ErbB2. Having more targets, probably its antitumor activity could be more efficient. Preclinical data reveal that lapatinib has activity in trastuzumab-resistant cell lines as well as synergistic activity with trastuzumab. Phase I clinical trials have also shown that lapatinib is well tolerated, with mild diarrhea and skin rush as common toxic effects

BACKGROUND: Trastuzumab has significantly improved the prognosis of breast cancer patients overexpressing the human epidermal growth factor receptor 2 (HER2). This result has been achieved in all disease settings, by increasing overall survival in early stage and advanced disease and by increasing pathological complete responses in neoadjuvant disease.
OBJECTIVE: Although the greatest impact of this monoclonal antibody has been seen in the adjuvant setting, by increasing disease-free survival and overall survival rates an increased rate of both symptomatic and non-symptomatic cardiac toxicity

BACKGROUND: The efficacy of irinotecan plus continuous trastuzumab beyond progression in patients with gastric cancer previously treated with trastuzumab plus standard first-line chemotherapy has not been reported.
METHODS: Patients with human epidermal growth factor receptor 2 (HER2)-positive advanced gastric cancer who were previously treated with trastuzumab received trastuzumab every 3 weeks and irinotecan every 2 weeks. The primary endpoint was the overall response rate (ORR), and the secondary endpoints included progression-free survival (PFS), 6-month survival rates, safety, and subgro

Here, we report that treatment of syngeneic mouse tumors transduced to overexpress human epidermal growth factor receptor-2 (HER2) with the anti-human HER2 antibody trastuzumab upregulated the level of programmed death-ligand 1 (PD-L1), an important negative regulator of T-cell response, in a transgenic mouse model immune-tolerant to human HER2. We further found that trastuzumab alone had no detectable effect on the level of PD-L1 expression in monocultures of HER2-overexpressing human breast cancer cells but upregulated PD-L1 in the same panel of HER2-overexpressing breast cancer cells when t

INTRODUCTION: Trastuzumab biosimilar EG12014 (Herwenda®; EirGenix/Sandoz) was approved by the European Medicines Agency (EMA) in 2023. We aim to demonstrate equivalent efficacy, and to compare safety, immunogenicity, and pharmacokinetic (PK) profiles, of EG12014 and reference trastuzumab (ref-TRA) in early breast cancer (EBC).
METHODS: This Phase III, randomized, multicenter, double-blind study included adult patients with EBC. In the neoadjuvant phase, patients received epirubicin and cyclophosphamide for four cycles, then were randomized (1:1) to four cycles of EG12014 or ref-TRA (loading/m

BACKGROUND: This study aims to evaluate the cost-effectiveness of Trastuzumab deruxtecan (T-DXd), compared to trastuzumab emtansine (T-DM1) as second-line treatments for human epidermal growth factor receptor 2 (HER2)-positive metastatic breast cancer (MBC) patients in Iran.
METHODS: A partitioned survival model was developed based on time-to-event data from the DESTINY-Breast 03 trial to evaluate the cost-effectiveness of T-DXd versus T-DM1 from a societal perspective and over a lifetime horizon. Costs and utility inputs were derived from published literature and official Iranian sources. Ke

INTRODUCTION: Trastuzumab, as the gold standard for HER2-positive BC treatment, was the first-line HER2 targeted drug. However, some studies reported patients benefited more from lapatinib and lapatinib plus trastuzumab therapy than standard trastuzumab therapy. This study presents an update of a systematic review and meta-analysis involving comparison of lapatinib and lapatinib plus trastuzumab therapy versus trastuzumab therapy.
AIM: We determined whether trastuzumab plus lapatinib or lapatinib therapy is not inferior to trastuzumab therapy in HER2-positive breast cancer patients.

IMPORTANCE: Clonal hematopoiesis of indeterminate potential (CHIP) is linked to an increased incidence of cardiovascular and malignant diseases.
OBJECTIVE: To determine whether CHIP is associated with cardiotoxic effects in patients with breast cancer receiving trastuzumab.
DESIGN, SETTING, AND PARTICIPANTS: Analyses of human cohorts with complementary animal experimentation were performed using a nationwide population-based cohort (UK Biobank), 1 tertiary referral center (Seoul National University Hospital [SNUH]), and a controlled laboratory setting. UK Biobank participants were enrolled b

Ado-trastuzumab emtansine (T-DM1) is a monoclonal antibody drug conjugate approved for the treatment of HER2-positive breast cancers. Presented here is a case report of a patient who developed fatal pulmonary toxicity in the form of acute eosinophilic pneumonia while undergoing treatment with T-DM1. Prior to beginning T-DM1 therapy, this patient had been treated with two HER2-targeted agents (trastuzumab, pertuzumab) per National Comprehensive Cancer Network (NCCN) guidelines. This case represents a novel presentation of toxicity associated with T-DM1 while perhaps demonstrating additive toxic

BACKGROUND: Significant and symptomatic cardiac comorbidity is a contraindication to adjuvant trastuzumab in breast cancer patients. However, some patients with asymptomatic, nonlimiting cardiac comorbidity and normal baseline left ventricular ejection fraction (LVEF) receive adjuvant trastuzumab in the clinical practice. We sought to describe the tolerability of trastuzumab in these patients.
PATIENTS AND METHODS: Retrospective analysis of patients with baseline asymptomatic, nonlimiting cardiac comorbidity receiving adjuvant trastuzumab at six Institutions between July 2007 and January 2016

Trastuzumab-induced cardiomyopathy is a known complication of its use in breast cancer treatment, but it remains mostly asymptomatic and often reversible. Non-myopathic cardiac complications have been rarely reported with trastuzumab. These include left and right bundle branch block, arrhythmias and sinus node dysfunction. We report a case of a 52-year-old female breast cancer patient with trastuzumab-induced asymptomatic intermittent left bundle branch block recurring nearly a year after the initial diagnosis and resolution of trastuzumab-related cardiomyopathy.
