Management of complicated neurologic injuries.

Despite measures based on the Monro-Kellie principles for ICP reduction and optimization of cerebral perfusion pressure, the outcome from complicated neurologic injuries remains unsatisfactory. Many patients are "pulled through" the acute event only to remain minimally functional or vegetative for the remainder of their lives. Pharmacologic interventions to protect the brain against the toxic and metabolic consequences of neurologic injury seem to be the future of neurotrauma.

Early traumatic experiences impair the functioning of both components of the endogenous stress response system in adult people with eating disorders.

Childhood trauma is a non-specific risk factor for eating disorders (EDs). It has been suggested that this risk is exerted through trauma-induced long-lasting changes in the body stress response system. Therefore, we explored the activity of the hypothalamus-pituitary-adrenal axis and of the sympathetic nervous system in adult ED patients with or without a history of childhood trauma exposure. Salivary cortisol and alpha-amylase, a marker of the sympathetic nervous system activity, were measured at awakening and after 15, 30 and 60 min in 35 women with EDs. The Childhood Trauma Questionnaire (

[Hypoxia of the central nervous system in rats during acceleration brain injury].

The authors present the results of investigations of the effects of acceleration craniocerebral injury in rats on PaO2 and resulting central nervous system hypoxia. PaO2 fall by 30% of the control value occurred in 25% of rats and in the group of most severe injury this fall amounted to 36% already 1 minute after injury. After 4 minutes this fall rose to 70%. In all animals with injury-induced hypoxia autopsy examination demonstrated on gross inspection acute pulmonary oedema: the weight of the lungs and the lung-body weight index were nearly twice as high as in the animals without oedema. Craniocervical junction abnormalities in dogs.

Craniocervical junction abnormality (CJA) is a term that encompasses a number of developmental anatomical aberrations at the region of the caudal occiput and first two cervical vertebrae. Chiari-like malformation appears to be the most common CJA encountered in dogs, and there has been a tremendous amount of clinical investigation into this disorder in recent years. Other abnormalities in this region include atlanto-occipital overlap, dorsal constriction at C1/C2 and atlantoaxial instability. This review article presents an overview of the current understanding of CJA in dogs, as well as medic

Surgical anatomy of the thyroid and parathyroid glands.

From a surgical viewpoint there are several critical anatomic structures that lie in close proximity to the thyroid gland. These critical structures include the recurrent laryngeal nerves, the superior laryngeal nerves, and the parathyroid glands. Successful thyroid surgery depends on the technical skill of the surgeon to identify and preserve these vital structures.

The autoimmune basis of hypopituitarism in traumatic brain injury: fiction or reality?

Post-traumatic hypopituitarism has remained as an obscured cause of worsening morbidity and mortality in head injury patients. Researchers have for decades been puzzled by the mechanism of pituitary dysfunction in these cases. Amongst other causes like direct injury, vascular injury etc, an immunological basis of hypopituitarism has been suggested in some animal studies as well as human research. In this article, we have reviewed the latest articles and compiled the evidence which suggests for or against the role of autoimmunity in post-traumatic hypopituitarism or which defines the strength t

Neurovascular injury during removal of levonorgestrel implants.

Injury to major neurovascular structures of the upper arm is a complication to be kept in mind, especially during difficult levonorgestrel implant removal. The risk of such an injury may be higher if the implants are inserted deeply over the brachial groove, where four major structures, namely, the basilic vein, the ulnar and median nerves, and the brachial artery, are located.

The body and the brain keep the score: a data-driven conceptual model linking trauma and postural tachycardia syndrome.

Postural tachycardia syndrome (POTS) is a common, often disabling disorder of autonomic nervous system regulation without a unifying etiological account. Converging evidence suggests infectious, physical, and emotional threats frequently precede onset. We propose a hypothesis-generating model in which POTS may, in some individuals, involve threat-induced, centrally maintained disruption of brain-body communication that may be responsive to neuroplasticity-based behavioral medicine. In this paper, we synthesized multidisciplinary literature and our team's data spanning (a) autonomic and central

An anatomical and physiological basis for the cardiovascular autonomic nervous system consequences of sport-related brain injury.

Concussion is defined as a complex pathophysiological process affecting the brain that is induced by the application or transmission of traumatic biomechanical forces to the head. The result of the impact is the onset of transient symptoms that may be experienced for approximately 2weeks in most individuals. However, in some individuals, symptoms may not resolve and persist for a protracted period and a chronic injury ensues. Concussion symptoms are generally characterized by their emergence through changes in affect, cognition, or multi-sensory processes including the visual and vestibular sy

Microcirculatory Changes in Experimental Models of Stroke and CNS-Injury Induced Immunodepression.

Stroke is the second-leading cause of death globally and the leading cause of disability in adults. Medical complications after stroke, especially infections such as pneumonia, are the leading cause of death in stroke survivors. Systemic immunodepression is considered to contribute to increased susceptibility to infections after stroke. Different experimental models have contributed significantly to the current knowledge of stroke pathophysiology and its consequences. Each model causes different changes in the cerebral microcirculation and local inflammatory responses after ischemia. The vast

Autonomic nervous system evaluation of patients with vasomotor rhinitis.

OBJECTIVE: To demonstrate the utility of quantitative neurological laboratory testing of autonomic nervous system dysfunction and to apply this methodology to further study the relation of chronic vasomotor (nonallergic) rhinitis to the autonomic nervous system. METHODS: It has been suspected that vasomotor rhinitis is due either to a hyperactive parasympathetic nervous system or an imbalance between it and the sympathetic nervous system. The exact relation has not been determined. Recently neurological laboratories have been developed in which a battery of tests can be performed to determine

[Peripheral nerve injury and male sexual dysfunction].

The genital organ is innervated by autonomic and somatic nerve. The former is both sympathetic and parasympathetic nerve and the later is comprised by sensory and motor fibers. The symptoms of male sexual dysfunction are sexopathy, erectile dysfunction, disorder of ejaculation and orgasm, and pianism. Not only different symptom but the same symptom can be induced by different injured nerve. The relationship between peripheral nerve injury and male sexual dysfunction should be understood correctly.

Ocular motor abnormalities from head trauma.

Head injuries cause the hospitalization of 200-300 persons per 100,000 population per year. Ophthalmologists provide diagnostic and therapeutic care to those trauma victims with damage to the globe, optic nerve, orbit, and ocular motor system. Eye movements can be affected by damage at any level of the central nervous system or peripheral motor unit. Comprehensive ocular motor assessment of the trauma patient can substantially contribute to the understanding of the patient's injury, recovery, and rehabilitation. This review examines all aspects of head and face trauma that can lead to ocular m

Cardiac autonomic functioning and post-traumatic stress: A preliminary study in youth at-risk for PTSD.

The identification of robust, psychophysiological markers of trauma-related distress is critical for developing comprehensive, trauma-informed, mental health assessments for youth. Thus, the present study examined the clinical utility of cardiac autonomic balance (CAB) and cardiac autonomic regulation (CAR), two composite indices of the sympathetic and parasympathetic nervous system. We hypothesized that CAB/CAR would more reliably index post-traumatic stress (PTS) responses compared to measuring the parasympathetic (i.e., respiratory sinus arrhythmia; RSA) and sympathetic (i.e., pre-ejection

Neurologic complications of alcoholism.

Alcohol is still the most abused drug today and may affect the peripheral, central, and autonomic nervous system in many ways. There are several pathophysiologic mechanisms that include direct effects through intoxication, withdrawal effects, secondary effects from nutritional problems and systemic diseases, and, of course, syndromes of unknown etiology. The alcoholic is also susceptible to trauma to both the central and peripheral nervous system. In many of the clinical entities, multiple factors play a role.
