Clinical and morphologic observations are described in 30 patients (23 [77%] of whom were in functional class III or IV) who underwent replacement of the mitral valve for mitral stenosis and either simultaneous replacement (13 patients, group I) or anuloplasty (17 patients, group II) of the tricuspid valve for pure tricuspid valve regurgitation. Comparison of the 13 patients in group I with the 17 patients in group II disclosed similar mean ages (55 vs 58 years), similar average preoperative right ventricular systolic pressures (64 vs 61 mm Hg), similar average right atrial mean pressures (10

In the landscape of valvular heart disease, the tricuspid valve has traditionally been dubbed the forgotten valve. However, over the last several years, studies have demonstrated that tricuspid valve disease is increasing in prevalence due to a growing aging population, and tricuspid regurgitation, in particular, is associated with increased mortality and morbidity. Traditional surgical tricuspid valve replacement or repair is often an undesirable option for patients with severe tricuspid regurgitation due to the high rates of concurrent comorbidities seen in this patient population; according

Between 1974 and January, 1986, 11 children underwent 13 tricuspid valve replacements at the Hospital for Sick Children in Toronto. Age at operation ranged from 24 hours to 14.5 years (mean, 6.9 years). Morphology of the tricuspid valves included Ebstein's anomaly (6 patients), congenital tricuspid regurgitation (3), tricuspid regurgitation and univentricular heart (1), and previous tricuspid valve excision for acute endocarditis (1). There were 4 early deaths: the 3 youngest infants in the series (age 1 day to 16 days) and another child who underwent emergency valve replacement died. On follo

The best means of managing tricuspid regurgitation associated with mitral or mitral and aortic valve disease is still to be determined. During the period 1972 to 1974, we treated 76 patients who had tricuspid regurgitation along with associated valvular dysfunction. Patients with mold regurgitation were treated conservatively, those with moderate regurgation underwent annuloplasty, and those with severe regurgitation had tricuspid valve replacement. We found the results to be less satisfactory in the group treated by annuloplasty than in the other two groups. We still manage conservatively tho

A total of 92 patients with tricuspid valvular disease (TR) had surgical repair of DeVega's annuloplasty in 80 patients (87%) and of valve replacement in 12 patients (13%) from January, 1978, to March, 1988. All of those patients were diagnosed by cardiac catheterization and angiogram, clinical findings and in recent cases, pulsed and color Doppler echocardiography were applied. Eighty-nine of 92 patients (97%) were in NYHA class III or IV before operation. There were 7 early death (8.5%) with DeVega procedure and one death (8.3%) in TVR and late deaths were noted in 3 patients (3.6%) (DeVega'

The role of valve replacement in the treatment of Ebstein's anomaly of the tricuspid valve remains controversial. Between 1965 and 1977, five patients with Ebstein's anomaly ranging in age from 11 to 57 years (mean 29) underwent tricuspid valve replacement in our institution. All of the patients were cyanotic but one, three were in Functional Class III, and two were in Class IV (New York Heart Association classification). The valve was placed above the coronary sinus. Plication of the atrialized portion of right ventricle was required in only one patient. Four patients had additional closure o

Background: Tricuspid valve surgery is indicated for severe symptomatic tricuspid valve regurgitation (TR) when performed concomitant to another cardiac operation. While percutaneous therapies for TR continue to advance, anatomic constraints may necessitate consideration of surgery in some patients. Methods: Herein, we provide a contemporary review of surgical techniques used to treat TR, which can serve as a complement to existing and emerging tricuspid valve treatment therapies. Results: We performed a literature review on established surgical tricuspid repair techniques. Conclusions: Surgic

Severe tricuspid regurgitation renders patients frail, and surgical treatment is associated with high mortality. Because most of the tricuspid regurgitation patients are functional and have significant annular dilation, large coaptation gaps are seen. This anatomy is best addressed with transcatheter tricuspid valve replacement (TTVR), and promising therapies are under clinical investigation. Most TTVR devices are in early clinical development with one transcatheter heart valve in pivotal trial; TTVR is expected to significantly affect tricuspid regurgitation and survival.

Tricuspid valve disease carries a very unfavorable prognosis when medically treated. Despite that, surgical intervention is still underperformed for tricuspid valve disease due to the reported high morbidity and mortality from a sternotomy approach. This had led to a shift towards maximizing medical therapy for right ventricular failure and, as a result, a more significant delay in surgical referrals with surgical risks when patients are finally referred. Tricuspid valve patients usually have other co-morbidities resulting from their systemic venous congestion and low flow cardiac output. Mini

Infective endocarditis is a common complication in patients using intravenous drugs. Fungal endocarditis is a rare clinical entity and diagnosis are often delayed. Only a few cases of candida dubliniensis have been reported in the literature so far. These cases usually present in patients with a history of intravenous drug abuse. If a patient with valvular endocarditis continues to have fever despite antibiotics, then fungal infection should be suspected. Fungal endocarditis is usually treated surgically along with a prolonged course of antifungal treatment. We report an interesting case of a

PURPOSE OF REVIEW: This review describes the normal structure and pathologic changes that affect the right-sided cardiac valves and chambers.
RECENT FINDINGS: The anatomy and pathology described have been known for many years. Knowledge of these findings has gained relevance. The pattern of endocarditis is changing. New diagnostic techniques have allowed better characterization of lesions responsible for cardiac dysfunction. Novel, less invasive interventions have made recognition of abnormalities more clinically relevant. There are many different pathologic entities that can affect the right

Only 75% of severe tricuspid regurgitation is classified as functional, or related primarily to pulmonary hypertension, right ventricular dysfunction, or a combination of both. Non-functional tricuspid regurgitation occurs when there is damage to the tricuspid leaflets, chordae, papillary muscles, or annulus, independent of right ventricular dysfunction or pulmonary hypertension. The entities that cause non-functional tricuspid regurgitation include rheumatic and myxomatous disease, acquired and genetic connective tissue disorders, endocarditis, sarcoid, pacing, RV biopsy, blunt trauma, radiat

PURPOSE OF REVIEW: Tricuspid valve disease has received much less attention in terms of intervention. The main reason for this is the widely held belief that treatment of left-sided valve disease leads to resolution of functional tricuspid regurgitation. Recent data show that tricuspid regurgitation is not benign and that many patients will benefit from intervention at the time of left-sided valve surgery, or in isolated tricuspid disease. This review describes the latest surgical and interventional options and strategies.
RECENT FINDINGS: Latest valve guidelines now recommend a more aggressi

When the incidence of tricuspid regurgitation is taken into account, along with its impact on functional status and long-term survival, tricuspid regurgitation is currently undertreated. Today, though transcatheter therapy of aortic, mitral- and pulmonic valve disease is well established, interventional treatment of tricuspid valve disease is still in its early stages. Currently, various promising devices are in different stages of development, but it is still too early to clarify which interventional approach in the future might result in functional and clinical success. Similarly, it is yet

OBJECTIVE: The objective was to analyze the short- and long-term results of patients with previous tricuspid valve repair who had valve dysfunction and required cardiac reoperations.
METHODS: Between 1976 and 2002, 74 patients with a mean age of 53.8 +/- 12.2 years underwent valve reoperations for dysfunction of previous tricuspid valve repair. Mitral and tricuspid lesions were diagnosed in 40 patients (54%), triple valve disease (mitral, aortic, tricuspid) was diagnosed in 26 patients (35.1%), isolated tricuspid disease was diagnosed in 6 patients (8.1%), and aortic and tricuspid lesions wer
