Tracer thyroxine (T4), 3.3',5-triiodothyronine (T3), and 3,3',5'-triiodothyronine (rT3) kinetic studies were performed in normal T4 substituted subjects before and during oral D-propranolol treatment to determine whether changes in thyroid hormone metabolism in a propranolol-induced low-T3 syndrome result from inhibition of 5'-deiodination or inhibition of transport of iodothyronines into tissues. Thyroxine and triiodothyronine (T3) are classical thyroid hormones and with relatively well-understood actions. In contrast, the physiological role of thyroid hormone metabolites, also circulating in the blood, is less well characterized. Twenty-five patients with newly diagnosed thyrotoxicosis participated in a double-blind study of the added effect of propranolol during tiamazol treatment. During the trial, the effect of treatment was ascertained by estimation at regular intervals of the clinical score (Crooks-Wayne index), free thyroxine index, serum triiodothyronine, serum reverse triiodothyronine, serum thyroglobulin and serum thyroglobulin antibodies. Further analysis of sera from euthyroid subjects with dominantly-inherited, elevated serum total thyroxine (T4) and free T4 index but with normal free T4 levels was performed as an extension of a previous study (Hennemann et al., 1979a). Hypothyroidism was diagnosed in an infant with massive hepatic hemangioendothelioma. Markedly elevated serum reverse-triiodothyronine levels confirmed consumptive hypothyroidism. Large doses of thyroxine were initially required to normalise TSH. Thyroxine was tapered as the tumor regressed and was discontinued at 16 months old. Thyroid function remains normal without treatment.

Iodine-induced thyrotoxicosis (ITT) has not been reported in Japan. We found that excessive intake of Japanese kombu could elicit ITT. Two Japanese women, 42 and 59 years old, developed thyrotoxicosis one month and one year, respectively, after having eaten foods containing 28-140 mg/day of iodine, calculated from their daily diet. Serum concentrations of T4, T3 and reverse T3 were studied in two hyperthyroid groups (n = 13 and 11), in a group of normals (n = 9) and in a group of L-T4 substituted patients (n = 7) with severe pretreatment hypothyroidism. Serum T4 did not change except in one of the hyperthyroid groups change to in which a slight decrease was found. In all groups a significant fall in serum T3 and a significant rise in serum reverse T3 were found. An expected increase in serum TSH in the normal and in the L-T4 substituted groups could not be demonstrated.

The effects of long-term treatment with antithyroid drugs, carbimazole (CMI) or propylthiouracil (PTU), on serum reverse triiodothyronine (rT3) levels were studied in 23 patients with Graves' disease.
