Maternal weight and ethnic adjustment within a first-trimester Down syndrome and trisomy 18 screening program.

OBJECTIVE(S): To estimate weight and ethnic group correction factors for first-trimester screening markers. METHODS: Ethnic-specific median MoM free beta hCG and pregnancy associated plasma protein A (PAPP-A) and delta nuchal translucency values were calculated for cohorts of maternal weight (20 lb each) using data from 51,206 patients undergoing first-trimester screening. False-positive rates for Down syndrome and trisomy 18 were evaluated both prior to and after weight and ethnicity adjustment. RESULTS: Free beta hCG and PAPP-A significantly decreased with increasing maternal weight while

[Between ethics and practice : thoughts about the clinical management of an infant with trisomy 18 (Edwards syndrome)].

Trisomy 18, also known as Edwards syndrome, is the second most common autosomal trisomy. The phenotype includes dysmorphia, congenital malformations and severe neurological impairment. The most common malformation is congenital heart disease. With a median survival of 2 to 14.5 days, trisomy 18 has been historically considered as lethal and care has long been palliative. This approach was proposed to a little girl with postnatal diagnosis. Initially, the child's parents disagreed with the management strategy. After further discussions, the parents agreed on a therapeutic approach based on comf

Facial markers in second- and third-trimester fetuses with trisomy 18 or 13, triploidy or Turner syndrome.

OBJECTIVE: To examine the effectiveness of nasal bone (NB) evaluation (including NB length (NBL)), prenasal thickness (PT) measurement, the PT:NBL ratio and the prefrontal space ratio (PFSR) in the identification of fetuses with trisomy 18 or 13, triploidy or Turner syndrome. METHODS: This was a retrospective study using stored midsagittal two-dimensional images of the facial profile of fetuses with trisomy 18 or 13, triploidy or Turner syndrome in the second and third trimesters. For images of acceptable quality, measurements were obtained of NBL (where NB was present), PT, the PT:NBL ratio

Second-trimester maternal serum progesterone levels in Turner syndrome with and without hydrops and in trisomy 18.

Placental proteins, such as inhibin A and hCG and its subunits, as well as the placental steroid progesterone, are elevated in second-trimester maternal serum from cases of fetal Down syndrome. Since different cellular mechanisms are required for protein versus steroid synthesis and secretion, these data suggest that a generalized placental hypersecretory phenomenon is associated with Down syndrome. Inhibin A and hCG are also elevated in cases of Turner syndrome with hydrops, and are reduced in cases of Turner syndrome without hydrops and in trisomy 18. The objective of the present study was t

Parachute mitral valve, coarctation of aorta, radius aplasia, and omphalocele in an infant with the trisomy 18 syndrome.

A premature infant with trisomy 18 had cardiac defects including parachute mitral valve, coarctation of the aorta, and a subcristal ventricular septal defect. The parachute mitral valve was funnel-shaped with a moderately-thick cone of fibrous tissue around the annulus and an eccentric orifice. Many forms of congenital cardiac defects have been described in the 18 trisomy syndrome. However, parachute mitral valve has not, to our knowledge, been part of the spectrum of cardiac defects in trisomy 18. Other anomalies were absent radii, omphalocele, and cleft palate and cleft lip; these have been

First non-mosaic case of isopseudodicentric chromosome 18 (psu idic(18)(pter --> q22.1::q22.1 --> pter) is associated with multiple congenital anomalies reminiscent of trisomy 18 and 18q- syndrome.

Isopseudodicentric chromosome 18 is very rare and results in a combination of partial trisomy and partial monosomy of chromosome 18. We report here a hypotrophic newborn with a lateral cleft lip and palate and multiple craniofacial dysmorphisms, a combined heart defect, unilateral hypoplasia of the kidney, bilateral aplasia of thumbs, and generalized contractures. Cytogenetic analysis revealed an isopseudodicentric chromosome 18 with breakpoint in 18q (46,XX,psu idic(18)(pter --> q22.1::q22.1 --> pter)). The isopseudodicentric chromosome 18 was observed in 100% of blood lymphocytes and umbilic

Heat-stable and immunoreactive placental alkaline phosphatase in maternal serum from Down's syndrome and trisomy 18 pregnancies.

Placental alkaline phosphatase (ALP) activity was investigated in second-trimester maternal sera from 37 pregnancies with Down's syndrome, 28 pregnancies with trisomy 18, and in a series of 497 controls using a fluorimetric heat inactivation assay and specific immunoassay. After conversion of individual analyte values to multiples of the normal gestational median (MOM), no significant differences in total or placental ALP activities were found in the trisomy 21 or trisomy 18 cases (P > 0.01). In the Down's syndrome pregnancies, total ALP activity was 0.93 MOM, heat-stable ALP activity was 1.09

Prune-belly syndrome with prostatic hypoplasia, bladder wall rupture, and massive ascites in a fetus with trisomy 18.

An association has been noted between trisomy 18 and genitourinary abnormalities, with six previous reports of the prune-belly syndrome occurring in patients with trisomy 18. We have observed a 120-g fetus of 18 weeks' gestational age at autopsy in whom there was severe prune-belly syndrome and trisomy 18. Serial histologic sections and reconstruction of the lower urinary tract demonstrated severe prostatic hypoplasia with a dilated, angulated prostatic urethra. Obstruction appeared to be present at the internal sphincter as a result of loss of prostatic support of the bladder. The bladder was

Second-trimester maternal serum inhibin A levels in fetal trisomy 18 and Turner syndrome with and without hydrops.

The objective was to investigate whether cases of fetal trisomy 18 and Turner syndrome with and without hydrops were associated with alterations in the second-trimester levels of maternal serum inhibin A. Twenty-one cases of trisomy 18, 10 cases of Turner syndrome without hydrops and 12 cases of Turner syndrome with hydrops were identified. Five control samples were matched to each case for date of sample collection and completed week of gestation. Inhibin A levels were modestly, but significantly reduced in cases of trisomy 18 (median = 0.88 MoM) and Turner syndrome without hydrops (median =

Two-Dimensional Differential Gel Electrophoresis to Identify Protein Biomarkers in Amniotic Fluid of Edwards Syndrome (Trisomy 18) Pregnancies.

BACKGROUND: Edwards syndrome (ES) is a severe chromosomal abnormality with a prevalence of about 0.8 in 10,000 infants born alive. The aims of this study were to identify candidate proteins associated with ES pregnancies from amniotic fluid supernatant (AFS) using proteomics, and to explore the role of biological networks in the pathophysiology of ES. METHODS: AFS from six second trimester pregnancies with ES fetuses and six normal cases were included in this study. Fluorescence-based two-dimensional difference gel electrophoresis (2D-DIGE) and matrix-assisted laser desorption/ionization time

Trisomy 17 in a bonobo (Pan paniscus) and deletion of 3q in a lowland gorilla (Gorilla gorilla gorilla): comparison with human trisomy 18 and human deletion 4q syndrome.

Viral infections are controlled, and very often cleared, by activated T lymphocytes. The inducible co-stimulator (ICOS) mediates its functions by binding to its ligand ICOSL, enhancing T-cell activation and optimal germinal center (GC) formation. Here, we show that ICOSL is heavily downmodulated during infection of antigen-presenting cells by different herpesviruses. We found that, in murine cytomegalovirus (MCMV), the immunoevasin m138/fcr-1 physically interacts with ICOSL, impeding its maturation and promoting its lysosomal degradation. This viral protein counteracts T-cell responses, in an

Anatomical analysis of the developmental effects of aneuploidy in man--the 18-trisomy syndrome: II. Anomalies of the upper and lower limbs.

We report the anatomical variations of the limbs in eight infants with the trisomy-18 syndrome that were dissected and studied in detail. In each case, the upper limbs showed defects which further define the specific influence of this aneuploidy on the development of its preaxial (radial) component, and the tendency towards reduction defects. Abnormalities included muscle variations concentrated along the radial margin of the forearm and hand, the absence of the definitive musculocutaneous nerve in all of the limbs, and reductions of the radial artery in four of the bodies. Pathogenetic mechan

[Trisomy 18 or postnatal Edward´s syndrome: descriptive study conducted at the University Hospital Center of Casablanca and literature review].

Trisomy 18 is a chromosomal disease, caused by the presence of a supernumerary chromosome 18. Mortality among infants with trisomy 18 is high, secondary to lethal malformations associated with this syndrome. The purpose of this study was to describe the clinical and cytogenetic features of these patients, as well as the role of genetic counselling. We conducted a cross-sectional descriptive study over a 5-year period, from July 2015 to April 2019. The study involved, patients followed up in the Department of Medical Genetics at the University Hospital Center Ibn Rochd of Casablanca, having abn

Advocating a specific risk calculation of trisomy 18 in case of low maternal serum markers during screening for fetal Down syndrome.

BACKGROUND: In France, legislation concerning pregnancy monitoring only considers screening for Down syndrome (T21), while the contingent introduction of the circulating cell free DNA test (DPNI) also allows screening for trisomies 13 and 18 with similar performances. METHODS: We retrospectively studied more than 800,000 patients among whom 7971 presented serum markers suggestive of T18 (but without increased risk of T21), of which 438 benefited from NIPT and of a complete pregnancy follow-up. RESULTS: We show that the use of a specific risk calculation for T18 would have improve the relevan
