Saudi guidelines for testing and treatment of latent tuberculosis infection.

Pulmonary tuberculosis is a common disease in Saudi Arabia. As most cases of tuberculosis are due to reactivation of latent infection, identification of individuals with latent tuberculosis infection (LTBI) who are at increased risk of progression to active disease, is a key element of tuberculosis control programs. Whereas general screening of individuals for LTBI is not cost-effective, targeted testing of individuals at high risk of disease progression is the right approach. Treatment of those patients with LTBI can diminish the risk of progression to active tuberculosis disease in the major

Therapy and prophylaxis of opportunistic infections in HIV-infected patients: a guideline by the German and Austrian AIDS societies (DAIG/ÖAG) (AWMF 055/066).

INTRODUCTION: There was a growing need for practical guidelines for the most common OIs in Germany and Austria under consideration of the local epidemiological conditions. MATERIALS AND METHODS: The German and Austrian AIDS societies developed these guidelines between March 2010 and November 2011. A structured Medline research was performed for 12 diseases, namely Immune reconstitution inflammatory syndrome, Pneumocystis jiroveci pneumonia, cerebral toxoplasmosis, cytomegalovirus manifestations, candidiasis, herpes simplex virus infections, varizella zoster virus infections, progressive multi

Reaching the limits of tuberculosis prevention among foreign-born individuals: a tuberculosis-control program perspective.

Analysis of whether assiduous implementation of American Thoracic Society/Centers for Disease Control and Prevention/Infectious Diseases Society of America guidelines for targeted testing and treatment of latent tuberculosis infection could have prevented any of 223 cases of active tuberculosis in foreign-born persons in San Francisco during the period 2002-2003. We report that 62% of these cases were not preventable and conclude that a further reduction in the incidence of tuberculosis among foreign-born persons will be modest without modification of current guidelines.

[LEGACIES OF SURGERY FOR TUBERCULOSIS AND SUCCESSION TO THE NEXT GENERATION].

A symposium entitled "Legacies of surgery for tuberculosis and succession to the next generation" was held at the 89th annual meeting of The Japanese Society for Tuberculosis in Gifu. The purpose of the symposium was to look back at the history of surgery for tuberculosis and development of surgical techniques. The contribution of those techniques to the next generation was also discussed. Many unique and universal techniques such as segmentectomy, thoracoplasty, muscle flap plombage, greater omental plom- bage, open window thoracotomy, cavernostomy, and decortication have matured during a lon

Skeletal tuberculosis in New Zealand since the introduction of chemotherapy.

The incidence of skeletal tuberculosis in New Zealand as measured by both hospital morbidity and notification data has fallen since the introduction of chemotherapy to low levels comparable with other developed societies. The decline has been greatest in the 5-14 year age group and least in the over 65 group. Individual surgeons can now expect to see few cases in a practising lifetime. Effective chemotherapy is the key to successful treatment. Adequate biopsy is essential, but apart from the drainage of large abscesses the place of surgery in the early management of osteoarticular tuberculosis

Personal and societal health quality lost to tuberculosis.

BACKGROUND: In developed countries, tuberculosis is considered a disease with little loss of Quality-Adjusted Life Years (QALYs). Tuberculosis treatment is predominantly ambulatory and death from tuberculosis is rare. Research has shown that there are chronic pulmonary sequelae in a majority of patients who have completed treatment for pulmonary tuberculosis (PTB). This and other health effects of tuberculosis have not been considered in QALY calculations. Consequently both the burden of tuberculosis on the individual and the value of tuberculosis prevention to society are underestimated. We e

[Practice guidelines for diagnosis, treatment, and prevention of tuberculosis in patients with rheumatic disease (2025 edition)].

China is a high-burden country for tuberculosis (TB), and patients with rheumatic diseases (RD) are at significantly increased risk of TB infection and progression. Currently, there are no comprehensive clinical practice guidelines providing recommendations for the screening, diagnosis, treatment, and prevention of TB in RD patients. To address this issue, the Collaborative Group for TB Prevention and Control in Patients with RD in China, in collaboration with the Chinese Society of Clinical Epidemiology and Evidence-Based Medicine, Chinese Society of Rheumatology, Chinese Society of Tuberculo

The emerging problem of biological treatment in migrant and travelling populations: it is time to extend guidelines for the screening of infectious diseases.

The use of biological agents in the treatment of rheumatic diseases has been widely associated with an increased risk of reactivation of several latent infections. National and international guidelines recommend screening for infectious diseases before starting these drugs. In Western countries screening is limited to latent tuberculosis infection, HIV and viral hepatitis. However, the increasing globalisation and the remarkable number of migrating and travelling people worldwide make this approach no longer adequate. The Italian and Spanish Societies of Rheumatology and Tropical Medicine wish

Second episode of tuberculosis in an HIV-infected child: relapse or reinfection?

We report a case of an HIV-infected child with a second episode of tuberculosis 22 months after completing antituberculosis treatment. DNA fingerprinting of organisms from both episodes showed an identical strain of Mycobacterium tuberculosis. We believe this to be the first case of confirmed relapsed tuberculosis in an HIV-infected child, and suggest that a longer course of antituberculosis treatment be given to such children. ¿ 2000 The British Infection Society.

Clinical characteristics and treatment outcomes of patients with isoniazid-monoresistant tuberculosis.

BACKGROUND: Risk factors and treatment outcomes under program conditions for isoniazid (INH)-monoresistant tuberculosis have not been well described. METHODS: Medical charts were retrospectively reviewed for all cases of culture-confirmed, INH-monoresistant tuberculosis ( n = 137) reported to the San Francisco Department of Public Health Tuberculosis Control Section from October 1992 through October 2005, and those cases were compared with a time-matched sample of drug-susceptible tuberculosis cases (n = 274) RESULTS: In multivariate analysis, only a history of treatment for latent tuberculo

[The importance of a careful examination of all persons in close contact with newly diagnosed infectious tuberculosis].

The diagnosis of infectious pulmonary tuberculosis was made in a 53-year-old Macedonian man. A careful examination of all persons in close contact with him according to the guidelines of the Swiss respiratory society was performed. Pulmonary tuberculosis was diagnosed in his 22-year-old son, in his 20-year-old daughter and in his 3-year-old granddaughter. Seven persons had isoniazid preventive treatment for latent tuberculosis. These data shows very clearly that a careful examination of all persons in close contact with contagious tuberculosis is very important.

Screening for Latent Tuberculosis Infection in Patients who are Candidate for Biological Therapies in Spain? A Multidisciplinary Survey.

INTRODUCTION: Treatment with biological therapies increases the incidence of tuberculous disease. The introduction of systematic screening for latent tuberculosis infection in patients who are to receive these therapies has reduced this risk. In 2016, the consensus document on the prevention and treatment of tuberculosis in patients who are candidates for biological treatment was published in Spain. The main objective of this study was to evaluate adherence to these guidelines. METHODS: Multicenter, descriptive, observational study via an anonymous online survey sent to medical societies invo

Diagnosis and management of latent tuberculosis infection in Asia: Review of current status and challenges.

Asia has the highest burden of tuberculosis (TB) and latent TB infection (LTBI) in the world. Optimizing the diagnosis and treatment of LTBI is one of the key strategies for achieving the WHO 'End TB' targets. We report the discussions from the Asia Latent TubERculosis (ALTER) expert panel meeting held in 2018 in Singapore. In this meeting, a group of 13 TB experts from Bangladesh, Cambodia, Hong Kong, India, Indonesia, Malaysia, Myanmar, the Philippines, Singapore, Taiwan, Thailand and Vietnam convened to review the literature, discuss the barriers and propose strategies to improve the manage

Epidemic of tuberculosis in the former Soviet Union: social and biological reasons.

The tuberculosis epidemic was evidenced in the states of the former Soviet Union at the end of the 20th century. The increase of epidemiological indicators for tuberculosis was accelerated by socio-economic changes in the society. Laboratory analysis also revealed new biological properties of the Mycobacterium tuberculosis isolates in the region, in particular the high level of resistance to antibiotics. Tuberculosis in Russia and the other newly independent states has been caused by M. tuberculosis strains of the W-Beijing family in recent years. These bacteria have rapidly developed resistan

Cohort profile: a multicenter hospital-based cohort of tuberculosis and non-tuberculous mycobacterial pulmonary disease in Korea, 2015-2024.

Tuberculosis and non-tuberculous mycobacterial pulmonary disease (NTM-PD) are major causes of chronic respiratory morbidity, yet detailed multicenter clinical data remain limited. We established a retrospective multicenter cohort using electronic medical records from 4 national university-affiliated tertiary hospitals in Korea. Eligible participants included patients with tuberculosis registered in the Korean National Tuberculosis Surveillance System and patients who met modified 2007 American Thoracic Society/Infectious Diseases Society of America microbiological criteria for NTM-PD between 2

Respiratory abnormalities and ventilatory capacity in a Papua New Guinea Island community.

On Karkar Island, off the coast of Papua New Guinea, 87 per cent of the 1,026 resident adults of 3 villages were surveyed for respiratory abnormalities. The prevalence of abnormalities suggestive of chronic lung disease was similar for each sex and increased with age. After 35 years of age, the combined rates were: adventitious breath sounds, 29 per cent; positive loose cough sign, 33 per cent; chronic cough, 11 per cent; and shortness of breath on exertion, 12 per cent. All of these abnormalities were associated with an obstructive ventilatory defect. Smoking began in late adolescence and was

[Tuberculosis in Europe and Poland--new molecular families and new resistance patterns].

At present despite methods of fast recognition of the disease and efficient antituberculosis drugs not only we cannot contain the epidemic but we can see an increase in new cases of tuberculosis including its drug resistant variety. Causes of aggravation of the situation are varied and ought to be examined separately in case of any particular region. One of the major ones are bad programmes of fighting against the disease or their inadequate realization, ignoring a problem of tuberculosis in developed countries, lack of money for treatment in developing countries and coincidence with HIV virus

We review the most interesting articles on respiratory infections published in the last trimester of 2009 and in 2010. Notable publications in bronchiectasis were the Guidelines of the British Thoracic Society, as well as several articles on the natural course of the process, the impact of exacerbations on the course of the disease, and treatment with inhaled antibiotics. Other notable publications were the SEPAR-SEIMC consensus document for the management of tuberculosis and articles on the use of interferon-gamma in the diagnosis of tuberculosis infection.
