This single-center retrospective case series assessed early ulnar artery patency by Doppler ultrasonography (DUS) after coronary angiography via distal ulnar access (DUA), described flow evolution in patients with follow-up, and reported access-related complications. Thirty-eight consecutive patients with a day-3 DUS after coronary angiography via DUA between September 2023 and March 2024 were included. All procedures were performed by 1 operator using a short 6F sheath and 5000 IU intra-arterial heparin. Flow was categorized as thrombosis/no flow, monophasic, or triphasic; the primary endpoin

BACKGROUND: Ulnar artery access may provide an alternative access route for chronic total occlusion percutaneous coronary intervention (CTO PCI), but data is limited.
AIMS: To assess the utilization, patient characteristics, and in-hospital outcomes of ulnar artery access compared with radial access and non-ulnar access in patients undergoing CTO PCI.
METHODS: We analyzed patients who underwent CTO PCI between 2012 and 2024 at 51 centers within the PROGRESS-CTO Registry. Patients were stratified by access: (1) no radial/ulnar, (2) radial without ulnar, and (3) ulnar. The primary endpoint was

BACKGROUND: Compartment syndrome (CS) is a rare complication from ulnar artery access for coronary intervention.
CASE SUMMARY: An 84-year-old patient presented with acute coronary syndrome. Coronary intervention was performed using ulnar artery access after radial access was found to be unsuitable. Post-procedure, upper extremity hematoma progressed to CS. Surgical fasciotomy and ulnar artery repair was successful, but neurologic impairment remained.
DISCUSSION: This patient represents a unique case of coronary intervention using ulnar artery access with resultant complications of hematoma a

Ulnar artery aneurysm (UAA) is an uncommon cause of digital ischemia. It is most often encountered in patients with repetitive injuries to the hypothenar region. UAA can cause significant morbidity, such as pain, nerve compression, and even acute digital ischemia if it is not identified early and treated accordingly. We present a case of a professional hockey player who presented with signs of Raynaud's phenomenon in the right-side ring finger, which was treated with UAA excision and primary end-to-end repair. A brief literature review of the literature is furthermore discussed.

Marfan syndrome (MFS) is a connective tissue disorder caused by mutations of the FBN1 gene encoding fibrillin-1, which leads to overexpression of transforming growth factor-β, increased hyaluronan deposition, and matrix metalloproteinase activity in the media of the aorta and other muscular arteries. Marfan syndrome patients present with connective tissue laxity and aneurysmal changes to muscular arteries. Successful medical and surgical intervention has prolonged the life expectancy of MFS patients, which can allow atypical presentations of the syndrome to manifest. We present a case of a 49-

After arising from the brachial artery in the cubital fossa the ulnar artery usually passes deep into the superficial flexor muscles of the forearm. In the lower two-thirds, it typically follows a sub-fascial course. In the present case, during a routine undergraduate course dissection of a cadaver, it was found that the ulnar artery arose normally as a terminal branch of the brachial artery in the cubital fossa, followed a sub-fascial course by lying superficial to the flexor muscles then completed the superficial palmar arch in hand. This artery gave only minute muscular branches in the fore

The aim of our study was to describe a dorsal distal radius vascularized bone graft pedicled on the posterior interosseous artery (PIA), and its clinical application in 2 cases of ulnar nonunion. We studied the surgical technique in 5 freshly injected cadavers. The 4th extensor compartment artery originates from the anastomotic arch between the posterior division of the anterior interosseous artery and the PIA and provides periosteal branches to supply the dorsal distal radius metaphysis. A 2-cm vascularized bone graft can be harvested from the radius, and dissection of the PIA enables a long

A 36-year-old female with known bilateral carpal tunnel syndrome was admitted to hospital and given a steroid injection on the ulnar side in her right hand. She suffered immediate ischaemia of the 3rd, 4th, and 5th fingers. Imaging showed decreased flow in the 4th and 5th phalageal arteries. Treatment with iloprost infusion commenced 7 days after the injury, with moderate improvement and further managed with a carpal tunnel release. This case report sheds light on an unusual yet very important complication of carpal tunnel management.

Naturally formed arteriovenous fistula (AVF) causing local vascular aneurysm dilatation in the forearm ulnar artery region is rare and is exceedingly uncommon in any age group. Presented is a case of AVF in the left ulnar artery of a 39-year-old man in whom there was no history of trauma; the deformity had been noted since childhood. The AVF had become tortuous and enlarged in size as the patient aged. As a result, aneurysm dilatation formed on the base of the AVF and that of the ulnar artery origin. Despite normal preoperative Allen test result and normal preoperative finger pressure measurem

Ulnar artery aneurysms (UAAs) are rare vascular lesions, most commonly associated with repetitive hand trauma and hypothenar hammer syndrome. Simultaneous involvement of both the ulnar and radial arteries is exceptionally uncommon. We report the case of a 61-year-old female patient with a true distal UAA in the dominant hand and concomitant ipsilateral radial artery occlusion, possibly associated with prolonged occupational repetitive trauma from sewing machine use. The patient presented with a progressively enlarging pulsatile wrist mass and sensory symptoms in the ulnar nerve distribution. C

Axillary artery is a continuation of subclavian artery, extending from the outer border of first rib to the lower border of teres major muscle. During routine dissection for the undergraduate medical students, a rare variations was seen in an approximately 55-year-old male cadaver. This case showed a variation in branching pattern of right axillary and subscapular arteries. The subscapular artery originated from 2(nd) part of axillary artery, gave origin to posterior circumflex humeral and lateral thoracic arteries in addition to its normal branches. The ulnar artery originated from the 3(rd)

PURPOSE OF REVIEW: Radial artery access for catheter-based procedures has demonstrated improved margins of safety and patient comfort without a loss of efficacy compared with femoral access. Unfortunately, radial access is not always available, so a review of the alternatives such as the ulnar artery and distal radial artery has been completed to understand alternative access sites that may preserve the benefits of distal forearm access without losing the efficacy of traditional transradial access.
RECENT FINDINGS: Several different trials have demonstrated the utility of ulnar access as a po

The anatomic relationship between the ulnar artery and transverse carpal ligament (TCL) as an aid in planning for minimally invasive carpal tunnel surgery was investigated. The anatomic course of the ulnar artery and its branches toward the TCL and the location of the median nerve were determined in 24 fresh cadaver hands perfused with a silicone compound. The ulnar artery coursed from 7 mm ulnar to 2 mm radial to the hook of hamate. The average distance between the superficial palmar arch and the distal margin of the TCL was 12 mm as measured along the flexor tendon of the ring finger. The lo

Pediatric craniosynostosis repair with cranial vault reconstructive surgery can be associated with significant blood loss. Tranexamic acid (TXA), an antifibrinolytic agent, has been shown to decrease blood loss and transfusion volume in craniofacial surgery. Nonetheless data regarding the safety of TXA remains limited. The authors describe a case of ulnar artery thrombosis following ulnar arterial line placement in a patient who received TXA for cranial vault reconstructive surgery.

Based on anatomical and dye injection observations, since 1984 42 cases of hand tissue defects (due to injury or surgery) have been reconstructed using a reverse flow unlar artery forearm island flap. Of the 42 cases, aetiology has included extensive crushing (25 cases), electric saw injuries (7 cases), burn scars (4 cases) and tissue defects following tumour or chronic ulcer resection (6 cases). Only one flap developed necrosis. The survival rate was 97.6%, including 4 cases of distal marginal necrosis. This procedure, therefore, offers a useful alternative in the repair and reconstruction of

Neurofibromatosis type 1 (NF1) is a genetic disorder involving an abnormality on chromosome 17, resulting in the production of the protein neurofibromin. Neurofibromin inhibits cell proliferation, and abnormalities in its encoding gene are hypothesized to trigger signals for proliferation, resulting in various lesions. Vascular fragility is a rare complication of NF1; however, ruptures of various vessels have also been reported. Here, we present a case of ulnar artery rupture treated endovascularly by puncturing the ipsilateral brachial artery, achieving excellent results.

Two cases are reported in which a fasciocutaneous island flap was employed supplied by the ulnaris dorsalis artery after the method proposed by Becker and Gilbert. The original technique has been modified by the authors, and this produces a better venous outflow. The vascular pedicle includes, besides the ascending branch of the artery and the venae comitantes, one of the superficial veins together with its respective subdermal band. A technique is also described that provides an optimal length for the vascular pedicle.

A rare variation of the ulnar artery is presented on routine anatomical dissection in one male cadaver. The superficial ulnar artery was found to originate from the axillary artery. The free forearm flap is increasingly popular among plastic surgeons. As a result of this case, and a review of the literature to gain further knowledge of anatomical variations, it is advised that more attention should be paid clinically to this artery to prevent its injury.
