Modified intramuscular transposition of the ulnar nerve.

The ulnar nerve passes posterior to the medial epicondyle and experiences longitudinal strain when subjected to elbow flexion. Furthermore, Osborne's ligament and the arcade of fibers between the 2 heads of the flexor carpi ulnaris (Osborne's fascia) compress the nerve during elbow flexion with narrowing of the cubital tunnel passageway. Some patients experience the added element of nerve subluxation over the posterior edge of the epicondyle. When changes to daytime ergonomic behavior and sleep posture prove insufficient to relieve a patient's symptoms, surgical treatment is warranted. A range

[Ulnar nerve compression in the area of the wrist].

Clinical investigation precisely determines the localization of ulnar nerve entrapment in Guyon's canal. The cause of compression can be due to internal (tumor, distal fibrous arch or proximal ligament arcade, nerve or muscle anomalies) or external influences (ulnar neuropathy of cyclists, thrombosis of ulnar artery by hypothenar hammer syndrome, fractures of pisiform or hook of hamate). Treatment involves splitting the roof of Guyon's canal, the ligaments or muscle anomalies, and excision of a tumor.

Recurrent ulnar-nerve dislocation at the elbow.

Recurring luxation of the ulnar nerve at the elbow is not uncommon (16.2%), occurring about equally in young and old, male and female, athletes and non-athletes but the greater mobility is usually at the dominant arm. The probable cause of such dislocation is congenital laxity of supporting ligaments. Being more vulnerable to injury than normally-positioned nerves, however, complicating neuritis can does occur. Subluxating nerves which stop on the tip of the medial humeral epicondyle upon 90 degrees or more of flexion at the elbow are more subject to direct trauma than completely displaced neu

Biomechanical analysis of ulnar nerve gliding and elongation: implications for nonsurgical ulnar nerve release in cubital tunnel syndrome.

BACKGROUND: Nonsurgical ulnar nerve release was developed for conservative treatment of cubital tunnel syndrome. Our objective in this study was to investigate the amount of ulnar nerve gliding and elongation during passive wrist, forearm, or elbow movements to determine the most effective nonsurgical ulnar nerve release technique. METHODS: Seven fresh-frozen cadaveric upper limbs were tested in an elbow-testing system. Ulnar nerve gliding (mobility) and elongation (stretching) were measured around the elbow joint using a three-dimensional digitizing system. Data were compared between arm pos

Anterior Interosseous Nerve to Ulnar Nerve Transfer: A Systematic Review.

BACKGROUND: Ulnar nerve injuries, especially high (proximal forearm) injuries, result in poor functional recovery. Peripheral nerve transfers have recently become a popular technique to augment nerve repairs and reduce the reinnervation distance before distal motor endplates irreversibly degenerate, leading to incomplete recovery. OBJECTIVES: To systematically review and analyse the recent literature regarding anterior interosseous nerve (AIN) to ulnar nerve transfers, including demographics, indications, outcomes, and complications. METHODS: A search was performed using PubMed, MEDLINE, EMB

[Entrapment of the ulnar nerve at the Guyon's canal].

ABSTRACT: The ulnar nerve runs at the level of the ulnar carpus through Guyon's canal. Due to the proximity to very rigid ligamentous and bony structures, space-occupying processes or fractures can easily cause compression of the ulnar nerve and its two distal branches. Pathologies of the adjacent ulnar artery can also irritate the nerve. ABSTRACT: After cubital tunnel syndrome, this is the second most common compression syndrome of the ulnar nerve. Overall, compression at Guyon´s canal is a very rare condition, which is considered challenging due to its variability of symptoms. Since the uln

Management of the Ulnar Nerve in Throwing Athletes.

Purpose of reviewDuring overhead throwing, there are tremendous forces placed on the medial elbow that can result in injury to the ulnar nerve. The purpose of this article is to review the anatomy, biomechanics, and appropriate clinical care of an overhead throwing athlete suffering from injury to their ulnar nerve. The authors preferred clinical work-up; non-operative and operative care are included in this review.Recent findingsMore recent findings support the use of subcutaneous transposition instead of the submuscular transpositions and only to perform a transposition in the setting of spe

Endoscopic decompression of the ulnar nerve at the elbow.

OBJECTIVE: Recently, several studies suggested that simple decompression is as effective as anterior transposition in ulnar nerve entrapment syndrome. Simple decompression might be performed with minimally invasive techniques. The authors present their technique and results with endoscopic decompression in ulnar nerve entrapment syndrome. MATERIAL AND METHODS: Between January 2005 and March 2008, 24 patients (mean age, 45.5 years; range, 26-67 years) underwent surgery for 26 ulnar nerve entrapment syndromes (2 bilateral). All patients presented with typical clinical signs and neurophysiologic

Anterior subcutaneous transposition of the ulnar nerve.

Anterior, subcutaneous ulnar nerve transposition decompresses the ulnar nerve and, by transposing anterior to the medial epicondyle, eliminates longitudinal traction forces applied to the nerve during elbow flexion. This article reviews the indications and contraindications of the technique and describes the surgical technique in detail.

Endoscopic decompression of the ulnar nerve at the elbow.

PURPOSE: The ideal operative treatment for cubital tunnel syndrome, the second most common form of peripheral compression neuropathy, remains controversial. We therefore reviewed our series of endoscopically assisted ulnar nerve decompression at the elbow to determine the effectiveness of the procedure, which was intended to minimize perioperative morbidity and scar discomfort. METHODS: In 36 patients (ages 22-76 years) with clinical McGowan grade I (4 patients), II (21 patients), and III (11 patients) and electrophysiologic signs of cubital tunnel syndrome (35 primary, 1 recurrent), 20 cm of

Arthroscopic neurolysis of the ulnar nerve at the elbow.

Cubital tunnel syndrome is a frequent form of neuropathy caused by entrapment of the upper limb. Conservative treatment and physical therapy are the treatment of choice for at least 6 months prior to any type of surgery. The surgical techniques proposed for treatment of this syndrome include simple decompression, transposition and neurolysis. It is the purpose of this study to demonstrate the advantages of arthroscopy for neurolysis of the ulnar nerve at the elbow in a specific cohort of patients. The patients were evaluated preoperatively and postoperatively based on subjective (VAS) and obje

Herpes zoster in the ulnar nerve distribution.

Varicella zoster is a ubiquitous virus which usually affects school-aged children as Chicken Pox. While the initial disease is self-limiting and seldom severe, the virus remains in the body. It lies dormant in the dorsal root ganglia and reactivation may occur years later with variable presentations as Herpes Zoster, or Shingles. While Shingles is common, it rarely presents exclusively in the upper extremity. It is important that hand surgeons recognize the possibility of zoster infection, with or without a rash, when evaluating the onset of neuralgia in a dermatomal distribution in the upper

Median and ulnar nerve conduction in pregnancy.

OBJECTIVE: A prospective, observational study assessed median and ulnar nerve conduction during pregnancy, to identify the optimum test for differentiating physiological effects of pregnancy from pathological carpal tunnel syndrome (CTS). METHODS: Pregnant women (n=18) and age- and parity-matched non-pregnant controls (n=13) were recruited. Symptomatic and neurophysiological evaluations were performed. Median and ulnar nerve latencies and intrapalmar latency (difference between median and ulnar nerve latencies) were computed. RESULTS: Median nerve distal latency is more prolonged in pregnant
