Morphology and morphometry of the ulnar head of the pronator teres muscle in relation to median nerve compression at the proximal forearm.

INTRODUCTION: The pronator syndrome is a rare compression neuropathy of the median nerve. Ulnar head of the pronator teres muscle may cause compression at proximal forearm. HYPOTHESIS: Detailed morphologic and morphometric studies on the anatomy of the ulnar head of pronator teres is scarce. MATERIAL AND METHODS: We dissected 112 forearms of fresh cadavers. We evaluated the morphology and morphometry of the ulnar head of pronator teres muscle. RESULTS: The average ulnar head width was 16.3±8.2mm. The median nerve passed anterior to the ulnar head at a distance of 50.4±10.7mm from the intere

The "hierarchical" Scratch Collapse Test for identifying multilevel ulnar nerve compression.

BackgroundThe Scratch Collapse Test (SCT) is used to assist in the clinical evaluation of patients with ulnar nerve compression. The purpose of this study is to introduce the hierarchical SCT as a physical examination tool for identifying multilevel nerve compression in patients with cubital tunnel syndrome.MethodsA prospective cohort study (2010-2011) was conducted of patients referred with primary cubital tunnel syndrome. Five ulnar nerve compression sites were evaluated with the SCT. Each site generating a positive SCT was sequentially "frozen out" with a topical anesthetic to allow determi

Regional anatomic structures of the elbow that may potentially compress the ulnar nerve.

HYPOTHESIS: Traumatic injuries to the ulnar nerve at the elbow are a frequent problem as it is vulnerable to stretching and compression with motion of the upper limb. The aim of the present study was to explore the course of the ulnar nerve at the elbow and forearm and to determine possible anatomical structures that may cause compression of this structure. MATERIALS AND METHODS: We examined 12 upper limbs from cadavers. The length of any fibrous bands, and if present, their distance to the medial epicondyle was recorded. RESULTS: On 5 sides a fibrous band originating from the medial intermu

Electrophysiological findings in entrapment of the median nerve at wrist and elbow.

In 117 consecutive patients with carpal tunnel syndrome and 11 patients with a compression syndrome of the median nerve at elbow, motor and sensory conduction along the median and ulnar nerves and quantitative electromyography were compared with findings in 190 normal controls of the same age. In 25% of patients with carpal tunnel syndrome in whom motor conduction and EMG were normal, the lesion was located from abnormalities in sensory conduction. The fact that conduction along the same fibres was moderately slowed from digit to palm, severely slowed across the flexor retinaculum, and normal

[Electroneurographic findings in cyclist's palsy (author's transl)].

The electromyographic and electroneurographic findings of a patient with cyclist's palsy of both hands are reported. The electrophysiological results show a compression syndrome of the deep palmar motor branch of the ulnar nerve without involvement of the hypothenar branches (Fig. 2a, b). Two more types of cyclist's palsy should be differentiated reviewing the literature: 1. a compression of the ulnar nerve at the wrist a lesion of the superficial branch, and 2. a compression of the deep motor branch including the hypothenar branches. As in the literature described probably exists a further ty

Ulnar tunnel syndrome.

Compression neuropathies are a significant source of pain in the upper extremity. Although ulnar tunnel syndrome occurs much less frequently than cubital tunnel syndrome, compression of the nerve at this level is a readily treatable condition. Ulnar tunnel syndrome should be kept in the differential diagnosis when the patient complains of numbness of the ring and small fingers, hand weakness, and pain on the ulnar side of the wrist.

[Unusual complications of median nerve neurolysis].

Two cases of acute post-operative compression of the ulnar nerve after, nerve release of the median nerve in the wrist are reported. The ulnar, nerve was free of any lesion on the preoperative clinical or electrical examination, at the elbow or the wrist. Stenosis of this nerve occurred during the month following the median nerve release as confirmed by EMG. The compression was treated by ulnar nerve release. Such a complication, representing 0.2% in a series of 982 cases of carpal tunnel syndrome, has never been mentioned in the literature. The authors suggest an etiopathogenic hypothesis whi

Importance of the ultrasound in cubital tunnel syndrome.

We report a case of a patient with cubital tunnel syndrome caused by presence of a non-thrombotic vein in the cubital tunnel. Cubital tunnel syndrome is a symptom complex caused by the compression of the ulnar nerve at the elbow. It is the second most common peripheral compression neuropathy. Ulnar compression at the elbow can be clinically diagnosed; however, an electroneurographic examination is necessary to confirm the diagnosis. Sonography and MRI may be required to locate nerve lesions, but primarily to evaluate the causes of compression. We decided to report this case because it shows th

In situ decompression of the ulnar nerve at the cubital tunnel.

Cubital tunnel syndrome is a clinical entity that has been described for more than a century. Numerous conservative and surgical treatments have evolved to address this condition, but a consensus has yet to emerge regarding optimal treatment. Evidence suggests a limited but potentially valuable role for in situ decompression of the ulnar nerve at the cubital tunnel. Future research will undoubtedly clarify this role and contribute to the development of a standard treatment protocol.

Ulnar nerve entrapment neuropathy at the elbow: simple decompression.

Ulnar nerve entrapment neuropathy at the elbow, or the cubital tunnel syndrome, is frequently encountered in neurosurgical practice as the second most common peripheral nerve entrapment after carpal tunnel syndrome. Patients typically present with weakness or atrophy of the hand as well as paresthesias in the ulnar nerve distribution. The diagnosis can be confirmed with a careful clinical examination and electrophysiological studies. Patients who have failed conservative therapy are considered for surgery. Although a number of surgical options are available, simple decompression of the ulnar n

Congenital constriction band syndrome causing ulnar nerve palsy: early diagnosis and surgical release with long-term follow-up.

Three children with congenital constriction band syndrome affecting their upper extremities demonstrated clinical and electrophysiologic signs of a complete ulnar nerve palsy. Two of the children were diagnosed immediately postpartum with the subtle findings of an intrinsic minus posture of their hand and inability to actively extend their fingers at the proximal interphalangeal joints. One child had at least 5.5 months of intrauterine compression of the ulnar nerve detected by ultrasound examination at 18 weeks. Despite early release of the constriction bands, at 3 months in 2 children and at

Peripheral Nerve Compression Syndromes in Children.

Nontraumatic neuropathies of the upper limb are rare in children. In this paper, we present the latest updates in this field and also critically review the diagnosis and treatment of nerve compression syndromes in children. This review describes the most common manifestations including idiopathic carpal tunnel syndrome (CTS), CTS occurring as a component of lysosomal storage diseases (mucopolysaccharidosis) and hereditary neuropathies, ulnar nerve neuropathy, and atraumatic radial nerve neuropathies.

Neurolymphomatosis of the Ulnar Nerve Presenting as Cubital Tunnel Syndrome: A Case Report.

Lymphoma is a malignant tumour of the lymphatic system. Neurolymphomatosis occurs when lymphoma cells invade the peripheral nervous system (PNS). Neurolymphomatosis has been reported involving the sciatic nerve, nerve roots and within the axilla and the upper arm. It can cause nerve dysfunction depending on the involved nerve. Neurolymphomatosis involving the ulnar nerve is rare. We report a patient with neurolymphomatosis of the ulnar nerve around the elbow that presented with features of cubital tunnel syndrome. A high index of suspicion is necessary in patients with cubital tunnel syndrome

Syndrome of canal of Guyon - definition, diagnosis, treatment and complication.

Syndrome of canal of Guyon is the second after carpal tunnel syndrome, compression syndrome in the wrist. Opposite to median nerve compression, ulnar nerve compression is not very popular. However it impairs functioning of the hand even more than median nerve lesion. Authors deal with definition, possible diagnostic methods, treatment and most frequent complication.

Median nerve compression in the palm of the hand by an anomalously enlarged ulnar artery.

Median nerve compression in the carpal tunnel by a thrombosed persistent median artery and a large aberrant artery substituting for the radial artery has been described but there have been no reports of median nerve compression in the palm of the hand by an anomalously enlarged ulnar artery. A 46 year old man is described who presented with clinical and electrophysiological features consistent with a median neuropathy at the wrist but surgical exploration revealed median nerve compression in the palm of the hand by an anomalously enlarged palmar branch of the ulnar artery. This case highlights

Simultaneous Carpal Tunnel Syndrome and Guyon's Canal Syndrome Caused by a Lipoma Arising From the Flexor Tenosynovium: A Case Report.

Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. While most cases are idiopathic, secondary CTS due to space-occupying lesions such as tumors is relatively uncommon. We report a rare case of simultaneous compression of the median nerve and the dorsal cutaneous branch of the ulnar nerve by a small lipoma arising from the flexor tenosynovium. Magnetic resonance imaging clearly demonstrated the lesion within the carpal tunnel and near Guyon's canal. Surgical excision led to resolution of median nerve symptoms, whereas ulnar sensory disturbance persisted due to inv

Modified in situ decompression of the ulnar nerve at the elbow.

Controversy continues to surround the treatment of ulnar nerve compression at the elbow in the cubital tunnel. By performing extensive segmental electrodiagnostic studies of the ulnar nerve through the cubital tunnel, accuracy of diagnosis is improved. In those patients with documented compression where the ulnar nerve enters the flexor carpi radialis, decompression of this area alone yields excellent results with rapid recovery.

Ulnar nerve compression at the elbow caused by the epitrochleoanconeus muscle: a case report and surgical approach.

Cubital tunnel syndrome is the second most common peripheral nerve compression syndrome. It is the most common peripheral neuropathy of the ulnar nerve. The surgical treatment of the cubital tunnel syndrome is widely described in the literature, however the variations of the standard muscular anatomy in the medial humeral epicondyle region may create technical difficulties during surgical management. The epitrochleoanconeus muscle, which is an aberrant muscle of this region, is a rare cause of cubital tunnel syndrome. A case with ulnar nerve compression at the elbow caused by an uncommon etiol

Common nerve decompressions of the upper extremity: reliable exposure using shorter incisions.

UNLABELLED: Considering that several different specialties perform nerve decompressions in the upper extremity, universal technical standards do not exist. Many of these procedures are performed via incisions that are made unnecessarily long to achieve adequate exposure of the nerves and their known anatomical compression points. The purpose of this article is to introduce reproducible techniques that reliably allow the necessary anatomical exposure while minimizing the length of required skin incisions. METHODS: The senior author's surgical approach to the most common nerve compression syndr
